Exploratory studies employing volunteer subjects are especially vulnerable to race and class bias. This article illustrates how inattention to race and class as critical dimensions in women's lives can produce biased research samples and lead to false conclusions. It analyzes the race and class background of 200 women who volunteered to participate in an in-depth study of Black and White professional, managerial, and administrative women. Despite a multiplicity of methods used to solicit subjects, White women raised in middle-class families who (...) worked in male-dominated occupations were the most likely to volunteer, and White women were more than twice as likely to respond to media solicitations or letters. To recruit most Black subjects and address their concerns about participation required more labor-intensive strategies involving personal contact. The article discusses reasons for differential volunteering and ways to integrate race and class into qualitative research on women. (shrink)

Healthcare workers experience moral injury (MI), a violation of their moral code due to circumstances beyond their control. MI threatens the healthcare workforce in all settings and leads to medical errors, depression/anxiety, and personal and occupational dysfunction, significantly affecting job satisfaction and retention. This article aims to differentiate concepts and define causes surrounding MI in healthcare. A narrative literature review was performed using SCOPUS, CINAHL, and PubMed for peer-reviewed journal articles published in English between 2017 and 2023. Search terms included (...) “moral injury” and “moral distress,” identifying 249 records. While individual risk factors predispose healthcare workers to MI, root causes stem from healthcare systems. Accumulation of moral stressors and potentially morally injurious events (PMIEs) (from administrative burden, institutional betrayal, lack of autonomy, corporatization of healthcare, and inadequate resources) result in MI. Individuals with MI develop moral resilience or residue, leading to burnout, job abandonment, and post-traumatic stress. Healthcare institutions should focus on administrative and climate interventions to prevent and address MI. Management should ensure autonomy, provide tangible support, reduce administrative burden, advocate for diversity of clinical healthcare roles in positions of interdisciplinary leadership, and communicate effectively. Strategies also exist for individuals to increase moral resilience, reducing the impact of moral stressors and PMIEs. (shrink)

The movement advocating the formal certification of clinical ethics consultants may result in major changes to the field of clinical ethics consultation by creating a new standard of care. The actual certification process is still in the development phase, but unanswered questions include: What will certification cost, and, Who will pay? Currently there is little salary support for ethics consultants and no regulation requiring healthcare institutions to offer clinical ethics consultation. Without the support of healthcare administrators and accreditation bodies, this (...) may remain unchanged.Healthcare administrators may be unwilling to pay for certification or professional services if accreditation bodies do not require healthcare institutions to provide certified ethics consultants’ services. If consultants will not be reimbursed or paid, they may not seek certification. If certified consultants are required, healthcare administrators may look for ways to cover the costs for providing this service, including insurance or third-party reimbursement and direct billing of patients for consultations, which may affect who performs and who participates in ethics consultation. However, this is less than ideal, as bioethicists believe ethics consultation should be available to all as part of providing safe, quality ethical care and support and guidance for patients, families, and healthcare staff.Going forward, bioethicists should study quality improvement, patient safety, and cost-savings resulting from certification-eligible clinical ethics consultants’ activities. Administrators and financial personnel can be surveyed regarding their support for the certification process. Bioethicists should enlist the help of patient rights and safety advocacy groups, professional medical associations, and healthcare administrators. Bioethicists should invite accreditation bodies, healthcare administrators, and financial personnel to collaborate in the development of the certification process. Without their support, certification may be of value only to the bioethics community, and may have little standing in actual clinical healthcare institution settings. (shrink)

Background Since the enactment of the euthanasia law in the Netherlands, there has been a lively public debate on assisted dying that may influence the way patients talk about euthanasia and physician-assisted suicide with their physicians and the way physicians experience the practice of EAS. Aim To show what developments physicians see in practice and how they perceive the influence of the public debate on the practice of EAS. Methods We conducted a secondary analysis of in-depth interviews with 28 Dutch (...) physicians who had experience with a complex case of EAS. Respondents were recruited both by the network of physicians working for SCEN as well as via a national questionnaire, wherein participating physicians could indicate their willingness to be interviewed. Results Three themes came up in analysing the interviews. First, the interviewed physicians experienced a change in what patients would expect from them: from a role as an involved caregiver to being the mere performer of EAS. Second, interviewees said that requests for EAS based on non-medical reasons came up more frequently and wondered if EAS was the right solution for these requests. Last, respondents had the impression that the standards of EAS are shifting and that the boundaries of the EAS regulation were stretched. Conclusions The perceived developments could make physicians less willing to consider a request for EAS. Our results also raise questions about the role of physicians and of EAS in society. (shrink)

Over the last decade, numerous interventions and techniques that aim to engender, strengthen, and expand compassion have been created, proliferating an evidence base for the benefits of compassion meditation training. However, to date, little research has been conducted to examine individual variation in the learning, beliefs, practices, and subjective experiences of compassion meditation. This mixed-method study examines changes in novice meditators’ knowledge and contemplative experiences before, during, and after taking an intensive course in CBCT®, a contemplative intervention that is increasingly (...) used for both inter- and intrapersonal flourishing. The participants in this study were Christian healthcare chaplains completing a 1-year residency in Clinical Pastoral Education who learned CBCT as part of their professional chaplaincy training curriculum. Prior to and upon completion of training, we surveyed participants to assess their beliefs about the malleability of compassion, types of engagement in compassion meditation, and perceptions of the impact of taking CBCT. We also conducted in-depth interviews with a subset of participants to gain a qualitative understanding of their subjective experiences of learning and practicing compassion meditation, a key component of CBCT. We found that participants reported increases in the extent to which they believed compassion to be malleable after studying CBCT. We also found high levels of variability of individual ways of practicing and considered the implications of this for the study of contemplative learning processes. This multi-methodological approach yielded novel insights into how compassion practice and compassion-related outcomes interrelate, insights that can inform the basic scientific understanding of the experience of learning and enacting compassion meditation as a means of strengthening compassion itself. Background Financial relationships between physicians and industry are extensive and public reporting of industry payments to physicians is now occurring. Our objectives were to describe physician recipients of large total payments from these seven companies, and to examine discrepancies between these payments and conflict of interest (COI) disclosures in authors’ concurrent publications. Methods The investigative journalism organization, ProPublica, compiled the Dollars for Docs database of payments to individuals from publically available data from seven US pharmaceutical companies during the period 2009 (...) to 2010. We examined the cohort of 373 physicians in this database who each received USD $100,000 or more in the reporting period 2009 to 2010. Results These physicians received a total of $52,600,624 during this period (mean payment per physician $141,020). The predominant specialties were internal medicine and psychiatry. 147 of these physicians authored a total of 134 publications in the first quarter of 2011 and 77% (103) of these publications provided a COI disclosure. 69% of the 103 publications did not contain disclosures of the payment listed in the Dollars for Docs database. Conclusions With increased public reporting of industry payments to physicians, it is apparent that large sums are being paid for services such as consulting and peer education. In over two-thirds of publications where COI disclosures were provided, the disclosures by physician authors did not include industry payments that were documented in the Dollars for Docs database. Ainsi dessine-t-elle une interrogation sur la place du plaisir, de la quête qu’il engendre, de l’attention réceptive et quasi co-créative qu’il sollicite. Pour mieux le montrer, l’analyse se centre sur le De musica, en revalorisant le lien (...) nécessaire, mais trop souvent mésestimé, entre les 5 premiers livres et le dernier et en montrant l’unité du projet indissociablement anagogique et esthétique. The paper suggests that Augustine anticipates in his own way « aesthetics ». Indeed his reflection does not only deal with sensible beauty, nor with art alone, but with the whole of subjective experience involving body and soul, sensibility and reason. It sets forth an interrogation on the place of pleasure, on the quest it generates, on the attention it requires both at the receptive and the co-creative level. In order to strengthen the argument, the analysis focuses on the De musica, emphasizing anew the link — necessary but too often overlooked — between the first five books and the last one and demonstrating the unity of a project that is indissolubly anagogical and aesthetic. Formalised knowledge systems, including universities and research institutes, are important for contemporary societies. They are, however, also arguably failing humanity when their impact is measured against the level of progress being made in stimulating the societal changes needed to address challenges like climate change. In this research we used a novel futures-oriented and participatory approach that asked what future envisioned knowledge systems might need to look like and how we might get there. Findings suggest that envisioned future systems will need (...) to be much more collaborative, open, diverse, egalitarian, and able to work with values and systemic issues. They will also need to go beyond producing knowledge about our world to generating wisdom about how to act within it. To get to envisioned systems we will need to rapidly scale methodological innovations, connect innovators, and creatively accelerate learning about working with intractable challenges. We will also need to create new funding schemes, a global knowledge commons, and challenge deeply held assumptions. To genuinely be a creative force in supporting longevity of human and non-human life on our planet, the shift in knowledge systems will probably need to be at the scale of the enlightenment and speed of the scientific and technological revolution accompanying the second World War. This will require bold and strategic action from governments, scientists, civic society and sustained transformational intent. (shrink)

Résumé L’analyse quantitative de l’histoire culturelle a été ouverte par la mise à disposition de corpus de masse tel que celui de Google fbooks (500 milliards de mots, 5 millions d’ouvrages, soit environ 4% de la littérature mondiale) et a été popularisé sous le nom de « culturonomics ». Elle s’ouvre désormais aux chercheurs, en promettant un accès profond aux faits culturels et à leurs évolutions qui affleurent à travers leurs traces textuelles dans les corpus textuelles numérisées. Encore faut-il pouvoir (...) interroger ces corpus dont la taille et la nature posent des problèmes scientifiques nouveaux, leur dimension les rendant illisibles directement et mettant échec les méthodes de fouille et les outils traditionnels d’analyse statistique des données en imposant des méthodes statistiques nouvelles et le saut vers des formes d’intelligence visuelles originales. Dans le cadre d’un projet mené entre le Labex « Obvil » de Paris-Sorbonne et le Literary Lab de Stanford sur l’histoire de l’idée de littéraire (la définition de la littérature comme mot, comme concept et comme champ), et visant à produire une histoire empirique de la littérature, nous avons mené depuis deux ans des expériences de fouille d’un corpus de critique littéraire de 1618 titres, 140 millions de mots (dont plus de 50 000 occurrences du lemme « littérature ») de la fin de l’Ancien Régime à la Seconde Guerre mondiale. En présentant des exemples développés dans cette première expérimentation à grande échelle de mesure de l’histoire des idées, on présentera les méthodes de text mining contemporaines en essayant d’éprouver leur pertinence heuristique et de leur capacité à faire remonter des données signifiantes pour l’histoire et la théorie littéraire. On fera l’hypothèse que toute enquête quantitative sérieuse mobilise désormais non une échelle intermédiaire standard et immédiatement lisible, mais le maniement d’outils statistiques dont l’interprétation en sciences humaines pose des problèmes particuliers qui, paradoxalement, ne peuvent être résolus que par leur articulation étroite à du close reading et à des mesures fines. Introduction - This paper is aimed at gaining more insight into the effects of camera-surveillance on behavior. This study investigates the effects of three different ways of ‘framing’ camera presence on cheating behavior and pro-social behavior. First, we explore the effect of presenting the camera as the medium through which an intimidating authority watches the participant. Second, we test the effect of presenting the camera as being a neutral, non-intimidating viewer. Third, we investigate whether a participant watching themselves via a (...) camera has an effect on their behavior. In contrast to most studies of camera surveillance, we will conduct our experiments in an indoor setting, enabling us to investigate behavior to a much greater detail. We will also explore possible moderating effects of personality traits. Methods - 86 students participated in this experiment, distributed over four conditions: three different ways of framing the camera presence, plus a control condition. Our main dependent variables were various kinds of cheating and pro-social behavior. We established the participant’s relevant personality traits in a separate session. Results - For cheating behavior, findings showed that the ‘authorative’ way of framing camera presence and the situation in which participants viewed themselves, differed significantly from a situation without camera-surveillance. We did not find significant effects of camera surveillance on pro-social behavior during the experiment. Looking at personality traits, we found an indication that people with an internal locus of control are more inclined to cheat when there is no camera present compared to people with an external locus of control. However, the effects of our manipulations were stronger. Conclusion - These results support the idea that the framing of a camera’s presence does indeed influence cheating behavior, adding to the preventive effects of camera-surveillance. Additionally, this study provides some valuable insights into the influence of camera presence on behavior in general. (shrink)

Si les lettres d’André Schaeffner n’ont pas été retrouvées, celles de Denise Paulme constituent une source essentielle pour observer les débuts d’une carrière scientifique féminine dans l’entre-deux-guerres. Elles nous révèlent que l’expérience de terrain, désormais située au cœur du parcours de l’ethnologue professionnel, a permis à Denise Paulme d’asseoir les (...) bases d’une légitimité qui ne lui était pas acquise. Mais elles mettent également en évidence la nécessité, pour une femme, de penser conjointement sa vie affective et sa vie scientifique, pour conquérir une place dans celle de ces deux vies où elle n’était pas attendue. How to engage farmers that have limited formal education is at the foundation of environmentally-sound and equitable agricultural development. Yet there are few examples of curricula that support the co-development of knowledge with farmers. While transdisciplinary and participatory techniques are considered key components of agroecology, how to do so is rarely specified and few materials are available, especially those relevant to smallholder farmers with limited formal education in Sub-Saharan Africa. The few training materials that exist provide appropriate methods, such as (...) compost making, but do not explain relationships and synergies between nutrition, social inequalities, climate change and agroecology. Some food sovereignty and agroecology courses aim at popular political education for those with more formal education. Here we describe the process of development of an innovative curriculum, which integrates agroecology, nutrition, climate change, gender and other dimensions of social equity across 2 weeks of training explicitly for smallholders in southern Africa with limited formal education. The curriculum is highly participatory; we use concepts in popular education, transformative and experiential-based learning, and theatre. It is also integrative; we link agroecology with climate change, human and soil nutrition, gender, and related components of social equity. Developed in partnership with Malawian farmers, community development experts and academics from five countries, the curriculum was piloted with 520 smallholder farming households in Malawi and Tanzania, and evaluated using qualitative techniques. Clashes of language, cultural norms, and terminology were as great of a challenge as agreeing on and conveying technical information, to weave into a coherent whole. However, farmers who participated in the curriculum training demonstrated high interest, comprehension of material and interest in immediate application to their lives. (shrink)

Depuis la nuit des temps, les relations homme-animal ont reposé sur un principe d'inégalité et de domination. Pourtant, la place et le statut des animaux ont récemment connu de grandes évolutions. L'animalisme, à l'instar de l'antispécisme ou de l'abolitionnisme est progressivement entré dans le débat public, poussé par une actualité brûlante (scandales des abattoirs, vidéos L214, émergence d'un véganisme de masse, etc.). A l'origine, ce courant d'éthique proposant de défendre les droits des animaux était considéré comme une mouvance violente et (...) radicale. Comment cette doctrine a-t-elle séduit un public de plus en plus large? Que nous raconte cet engouement quant à nos valeurs, aux croyances de notre société? Cet ouvrage constitue la première analyse sociologique de ce phénomène émergent. En embrassant les points de vues politiques, écologiques, religieux ou encore celui des contre-cultures, Marianne Celka jette un regard neuf et, pour la première fois, neutre et objectif sur cette question aussi profonde que troublante du rapport entre l'homme et l'animal. Ainsi entendu, l’exercice en art pourrait être comparé à un « exercice spirituel ». Pour autant, on invitera ultimement à nuancer une telle comparaison, en prenant appui sur les textes de P. After many policy attempts to tackle the persistent rise in the costs of health care, physicians are increasingly seen as potentially effective resource stewards. Frameworks including the quadruple aim, value-based health care and choosing wisely underline the importance of positive engagement of the health care workforce in reinventing the system–paving the way to real affordability by defining the right care. Current programmes focus on educating future doctors to provide ‘high-value, cost-conscious care’ (HVCCC), which proponents believe is the future of sustainable (...) medical practice. Such programmes, which aim to extend population-level allocation concerns to interactions between an individual doctor and patient, have generated lively debates about the ethics of expanding doctors’ professional accountability. To empirically ground this discussion, we conducted a qualitative interview study to examine what happens when resource stewardship responsibilities are extended to the consulting room. Attempts to deliver HVCCC were found to involve inevitable trade-offs between benefits to the individual patient and (social) costs, medical uncertainty and efficiency, and between resource stewardship and trust. Physicians reconcile this by justifying good-value care in terms of what is in the best interest of individual patients–redefining the currency of value from monetary costs to a patient’s quality of life, and cost-conscious care as reflective medical practice. Micro-level resource stewardship thus becomes a matter of working reflexively and reducing wasteful forms of care, rather than of making difficult choices about resource allocation. (shrink)

L’objet de cet article est de l’étayer et de la préciser, en lisant les textes épicuriens sur le temps à la lumière des trois grandes objections antiatomistes de la Physique. Notre analyse conclut que l’atomisme du (...) temps permet de répondre aux objections d’Aristote, mais doit être limité au seul niveau des atomes, par opposition à celui des agrégats. Il permet alors en effet de justifier l’isotachie des atomes, de rendre concevable la communauté de structure de la grandeur, du mouvement et du temps, et de garantir le caractère fini de la vitesse atomique. Deux lignes argumentatives se superposent : une conception proprement atomiste du mouvement et une reprise des termes de la physique aristotélicienne destinée à mieux en réfuter les objections. Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. En soulignant (...) le lent déplacement notionnel qui ramène l’inspiration du ciel à la terre et de l’extraordinaire à l’ordinaire, on plaidera pour la puissance d’idées « vagabondes ». From the 1960s, mathematical and computational tools have been developed to arrive at human population trees from various kinds of serological and molecular data. Focusing on the work of the Italian-born population geneticist Luigi Luca Cavalli-Sforza, I follow the practices of tree-building and mapping from the early blood-group studies to the current genetic admixture research. I argue that the visual language of the tree is paralleled in the narrative of the human diasporas, and I show how the tree was actually (...) mapped onto the surface of the earth. This visual and textual structure is mirrored in the liberal discourse of unity in diversity that has been criticized as running counter to the socio-political effects of human population genetics. From this perspective, one may ask how far the phylogenetic diagram in its various forms is a manifestation of the physics of power that according to Michel Foucault consists in mechanisms that analyse distributions, movements, series, combinations, and that uses instruments to render visible, to register, to differentiate and to compare. It is one among other disciplinary technologies that ensure the ordering of human diversity. In the case of intra-human phylogenetic trees, population samples and labels are one issue. Another is that the separated branches seem to show groups of people, who have in reality been interacting and converging, as isolated. Often based on so-called isolated peoples, molecular tree diagrams freeze a hierarchical kinship system that is meant to represent a state before the great historical population movements. (shrink)

Résumé Les missions et transmissions qui s’engagent dans les liens sociaux et familiaux ont prise sur le corps, ici défini dans sa double composition de corps biologique et de corps érotique. Les messages du sexuel inconscient infiltrent l’architecture psychosomatique et subvertissent les relations familiales, sociales, intersubjectives. Après quelques rappels théoriques, l’article aborde plusieurs situations cliniques prises dans la littérature psychanalytique (cas de Dora pour Freud) et dans la pratique clinique récente avec des patients présentant des pathologies somatiques diverses : pelade (...) totale décalvante, infarctus du myocarde, suivi post-greffe cardio-pulmonaire. Cet article propose une conceptualisation du deuil périnatal chez l’homme, sujet peu étudié jusqu’ici. Le deuil périnatal est particulier, du fait à la fois du manque de différenciation de l’objet perdu, de la force avec laquelle il était investi et d’une hémorragie narcissique renforcée. Dans la famille bouleversée, le deuil du « devenant père » présente certaines spécificités. L’article s’attache à développer la question de la symbolisation de la perte chez le père. Une réflexion sur le continuum narcissico-objectal de l’investissement (...) du bébé in utero est engagée, qui mènera à une proposition de conceptualisation du deuil périnatal centrée sur la notion de relation d’objet virtuelle – l’« enfant virtuel » n’étant pas à opposer à l’enfant réel, mais à l’enfant actuel. Le Commentarium in Convivium Platonis de Marsile Ficin célèbre le Banquet de Platon. Mais dans cette reprise laudative, de notables infléchissements témoignent d’une réinterprétation décisive du legs du platonisme. En se centrant sur la seule réécriture du portrait de Socrate par Alcibiade, l’article met ainsi en évidence des modifications troublantes et répétées de détail en détail – la laideur du silène, l’atopie du philosophe et la puissance corybantique de ce Marsyas disparaissent au profit de l’éloge d’un Socrate qui « charme (...) » par son éloquence supérieure et apparaît comme un verus amator. Loin d’être mineures, ces modifications répondent en fait à un remaniement métaphysique de la conception de l’amour et, conséquemment, de la « philo-sophie », où se perd la dimension inquiétante de la mania platonicienne. L’effroi de la dépossession est désormais circonscrit dans le circuitus spiritualis normé d’une harmonie cosmique. (shrink)
