Objective: Deep brain stimulation targeted to the ventral intermediate nucleus of the thalamus is effective for motor symptoms in essential tremor, but there is limited data on cognitive outcomes. We examined cognitive outcomes in a large cohort of ET DBS patients.Methods: In a retrospective analysis, we used repeated-measures ANOVA testing to examine whether the age of tremor onset, age at DBS surgery, hemisphere side implanted with lead, unilateral vs. bilateral implantations, and presence of surgical complications influenced the cognitive outcomes. Neuropsychological (...) outcomes of interest were verbal memory, executive functioning, working memory, language functioning, visuospatial functioning, and general cognitive function.Results: We identified 50 ET DBS patients; 29 males; the mean age of tremor onset was 35.84 years with a median age of 38 years. The mean age at DBS was 68.18 years. There were 37 unilateral 30 left, seven right, and 13 bilateral brain implantations. In the subgroup analysis, there was a significant interaction between assessment and age of tremor onset ; F = 4.47; p = 0.043 for working memory. The post hoc testing found improvements for younger onset ET. Similarly, there was a significant interaction between assessment and complications vs. no complications subgroups; F = 4.34; p = 0.043 for verbal memory with worsening scores seen for ET patients with complications. The remaining tests were not significant.Conclusion: In this large cohort of ET patients with, DBS was not accompanied by a significant decline in many cognitive domains. These outcomes were possibly related to the selection of patients with normal cognitive functioning before surgery, unilateral DBS implantations for the majority, and selection of patients with optimal response to DBS. IntroductionDeep brain stimulation (DBS) is increasingly used to treat the symptoms of various neurologic and psychiatric conditions. People can undergo the procedure during reproductive years but the safety of DBS in pregnancy remains relatively unknown given the paucity of published cases. We thus conducted a review of the literature to determine the state of current knowledge about DBS in pregnancy and to determine how eligibility criteria are approached in clinical trials with respect to pregnancy and the potential for pregnancy.MethodsA literature (...) review was conducted in EMBASE to identify articles involving DBS and pregnancy. Two reviewers independently analyzed the articles to confirm inclusion. Data extracted for analysis included conditions treated, complications at all stages of pregnancy, neonatal/pediatric outcomes, and DBS target. A second search was then conducted using www.clinicaltrials.gov. The same two reviewers then assessed whether each trial excluded pregnant individuals, lactating individuals, or persons of childbearing age planning to conceive. Also assessed was whether contraception had to be deemed adequate prior to enrollment.ResultsThe literature search returned 681 articles. Following independent analysis and agreement of two reviewers, 8 pregnancy related DBS articles were included for analysis. These articles described 27 subjects, 29 pregnancies (2 with subsequent pregnancies), and 31 infants (2 twin pregnancies). There was 1 preterm birth at 35 weeks, and 3 patients who experienced discomfort from the DBS battery (i.e., impulse generator) placement site. All 27 patients had a DBS device implanted before they became pregnant, which remained in use throughout their pregnancy. There was exclusion of pregnant individuals from 68% of 135 interventional trials involving DBS. Approximately 44% of these trials excluded persons of childbearing age not on “adequate contraception” or wishing to conceive in the coming years. Finally, 22% excluded breastfeeding persons.ConclusionThe data from 29 pregnancies receiving DBS treatment during pregnancy was not associated with unexpected pregnancy or post-partum complication patterns. Many clinical trials have excluded pregnant individuals. Documentation of outcomes in larger numbers of pregnancies will help clarify the safety profile and will help guide study designs that will safely include pregnant patients. (shrink)

We estimate that 208,000 deep brain stimulation (DBS) devices have been implanted to address neurological and neuropsychiatric disorders worldwide. DBS Think Tank presenters pooled data and determined that DBS expanded in its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. The DBS Think Tank was founded in 2012 providing a space where clinicians, engineers, researchers from industry and academia discuss current and emerging DBS technologies and logistical and ethical issues facing the field. (...) The emphasis is on cutting edge research and collaboration aimed to advance the DBS field. The Eighth Annual DBS Think Tank was held virtually on September 1 and 2, 2020 (Zoom Video Communications) due to restrictions related to the COVID-19 pandemic. The meeting focused on advances in: (1) optogenetics as a tool for comprehending neurobiology of diseases and on optogenetically-inspired DBS, (2) cutting edge of emerging DBS technologies, (3) ethical issues affecting DBS research and access to care, (4) neuromodulatory approaches for depression, (5) advancing novel hardware, software and imaging methodologies, (6) use of neurophysiological signals in adaptive neurostimulation, and (7) use of more advanced technologies to improve DBS clinical outcomes. There were 178 attendees who participated in a DBS Think Tank survey, which revealed the expansion of DBS into several indications such as obesity, post-traumatic stress disorder, addiction and Alzheimer’s disease. Background: Treating medication-refractory freezing of gait in Parkinson’s disease remains challenging despite several trials reporting improvements in motor symptoms using subthalamic nucleus or globus pallidus internus deep brain stimulation. Pedunculopontine nucleus region DBS has been used for medication-refractory FoG, with mixed findings. FoG, as a paroxysmal phenomenon, provides an ideal framework for the possibility of closed-loop DBS.Methods: In this clinical trial, five subjects with medication-refractory FoG underwent bilateral GPi DBS implantation to address levodopa-responsive PD symptoms with open-loop stimulation. Additionally, PPN (...) DBS leads were implanted for CL-DBS to treat FoG. The primary outcome of the study was a 40% improvement in medication-refractory FoG in 60% of subjects at 6 months when “on” PPN CL-DBS. Secondary outcomes included device feasibility to gauge the recruitment potential of this four-lead DBS approach for a potentially larger clinical trial. Safety was judged based on adverse events and explantation rate.Findings: The feasibility of this approach was demonstrated as we recruited five subjects with both “on” and “off” medication freezing. The safety for this population of patients receiving four DBS leads was suboptimal and associated with a high explantation rate of 40%. The primary clinical outcome in three of the five subjects was achieved at 6 months. However, the group analysis of the primary clinical outcome did not reveal any benefit.Interpretation: This study of a human PPN CL-DBS trial in medication-refractory FoG showed feasibility in recruitment, suboptimal safety, and a heterogeneous clinical effect in FoG outcomes. DBS Think Tank IX was held on August 25–27, 2021 in Orlando FL with US based participants largely in person and overseas participants joining by video conferencing technology. The DBS Think Tank was founded in 2012 and provides an open platform where clinicians, engineers and researchers can freely discuss current and emerging deep brain stimulation technologies as well as the logistical and ethical issues facing the field. The consensus among the DBS Think Tank IX speakers was that DBS expanded in (...) its scope and has been applied to multiple brain disorders in an effort to modulate neural circuitry. After collectively sharing our experiences, it was estimated that globally more than 230,000 DBS devices have been implanted for neurological and neuropsychiatric disorders. As such, this year’s meeting was focused on advances in the following areas: neuromodulation in Europe, Asia and Australia; cutting-edge technologies, neuroethics, interventional psychiatry, adaptive DBS, neuromodulation for pain, network neuromodulation for epilepsy and neuromodulation for traumatic brain injury. This collection of essays is the first to look closely at the phenomena of philosophy in a cafe. Since the tradition of philosophical dialogue in coffee-houses was revived in Paris in the 1990s, public venues for participatory philosophy have sprung up in numerous countries, taking many forms, all seeking to stimulate intellectual interest as well as meaningful democratic community engagement. Some of the earliest discussion series continue to this day. The simple activity of reasoning together in a cafe is of (...) interest to democratic theory, epistemology, social philosophy, philosophy of education. Although much has been written on the theory and practice of philosophy for children, little or no concerted exploration has taken place of participatory philosophy in the public sphere. This volume brings the international voices of numerous facilitators of engaged philosophical inquiry, including some of the most prominent, together with observers in allied fields, to explore practical, organizational issues, but also to bring critical and theoretical perspectives to bear on the Café Philosophy. The article takes in to account the influence of laws in the context of violence cases and it finishes reviewing certain notion taken from the juridical work. This is done beginning with a reading based on human sciences and psychoanalysis. To accept that cognition is embodied is to question many of the beliefs traditionally held by cognitive scientists. One key question regards the localization of cognitive faculties. Here we argue that for cognition to be embodied and sometimes embedded, means that the cognitive faculty cannot be localized in a brain area alone. We review recent research on neural reuse, the 1/f structure of human activity, tool use, group cognition, and social coordination dynamics that we believe demonstrates how the boundary between (...) the different areas of the brain, the brain and body, and the body and environment is not only blurred but indeterminate. In turn, we propose that cognition is supported by a nested structure of task-specific synergies, which are softly assembled from a variety of neural, bodily, and environmental components (including other individuals), and exhibit interaction dominant dynamics. Intuitively it has seemed to many that our concepts conscious state and conscious creature are sharp rather than vague, that they can have no borderline cases. On the other hand, many who take conscious states to be identical to, or realized by, complex physical states are committed to the vagueness of those concepts. In the paper I argue that conscious state and conscious creature are sharp by presenting four necessary conditions for conceiving borderline cases in general, and showing that some (...) of those conditions cannot be met with conscious state. I conclude that conscious state is sharp, and the conclusion is then extended to conscious creature. The paper ends with a brief discussion of some implications. (shrink)

This study aims to point out the disfunction produced in the application of organic law 10/ 2007, which regulates the police base of identifiers obtained from DNA, in relation to the police obtaining biological samples of detained persons. Because of the compulsory and authomatic nature of the registration of DNA database of suspected, detained or charged reached in the investigation of the crimes predetermined by law, it has been derived a similar automatism in the obtaining of biological samples from the (...) same subjects to obtain the registrable genetic profile, which mainly affects people who are detained. This action could lead to a distortion of the requirements related to informed consent for such obtaining, with infringement of the fundamental right to the protection of data or to informational self determination. SPEP 2022 Plenary Addresses, and we are grateful to be able to include their plenary papers in this special issue. Hardt's paper, "The Politics of Articulation and Strategic Multiplicities," treated SPEP members to an early peek at his newest book, The Subversive 70s (Oxford University Press, 2023). In this work, Hardt digs into the powerful resources for social movements that activists and theorists from the 1970s developed. As he argues, in many ways those activists and thinkers were ahead of their times—and also ahead of ours—in understanding how to analyze interwoven multiplicities of power and how to articulate and organize liberation struggles based on those multiplicities. Hardt brilliantly demonstrates how analyzing the progressive and revolutionary social [End Page 237] movements of the '70s can help us not only understand the roots of contemporary social and political struggles but also reclaim critical resources for those struggles.Patricia Pisters's paper, "Thinking with Fire: Elemental Philosophy and Media Technology," draws upon Gaston Bachelard's "fire complexes" to address a variety of pyrotechnical images appearing in contemporary cinema. Noting that elemental philosophy is on the rise in media studies and elsewhere, not least because of current environmental crises, fire is particularly engaging for its metaphorical, "matterphorical," and technological associations. While acknowledging fire as a material medium—cooking, heating, burning, etc.—Pisters's primary focus in her paper is on fire as an immaterial medium, and it is here that she turns to Bachelard's constellation of fire complexes—the Empedocles, Prometheus, and Novalis Complexes—to which she adds her own fourth complex, the Sita Complex. With these complexes, she provides readings of four cinematic productions that elucidate the annihilating, transgressive, sexual, and purifying qualities of fire, and she suggests that these entangled fire complexes present different kinds of combustive knowledge in which the element of fire manifests itself as material phenomenon of nature, the engine for modern life, and immaterial affective reverie of destruction, transgression, and sexuality.The other articles in this special issue have been organized according to five broad groupings. In "Two Versions of the Mestizo [End Page 238] Model: Toward a Theory of Anti-Blackness in Latin American Thought," Miguel Gualdron Ramirez criticizes the anti-Blackness that he sees at the heart of mestizo models of latinidad, or lo latinoamericano. Focusing on the concepts of liberation offered by Venezuelan theorist and politician Simón Bolívar and Mexican philosopher José de Vasconcelos, Ramirez reveals the exclusion and erasure of Black bodies, lives, and histories that ground their versions of Latinx identity and Latin American history. In this talk, I shall begin by considering alternative definitions of "naturalism", and by asking how the term is best understood in the present context. In answering this question, I shall distinguish between anti-naturalism on the one hand, and supernaturalism on the other. Next, I shall discuss the relation between science and supernaturalism, and I shall argue, first, that a commitment to scientific method does not in itself presuppose a rejection of supernaturalism, and secondly, that scientific investigation and theorizing could (...) in principle show either that supernaturalism is true, or that it is false. I shall then go on to survey a number of pro-supernaturalist arguments and discussions, both traditional and more recent. Given the number of such arguments and discussions, my critical commentary will necessarily be rather brief, though I shall comment in more detail upon some of the work of Michael Behe and William Dembski. The basic conclusion for which I shall argue is that there is no good reason for thinking that supernaturalism is true. (shrink)

We prove that the problem of determining the minimum propositional proof length is NP- hard to approximate within a factor of 2 log 1 - o(1) n . These results are very robust in that they hold for almost all natural proof systems, including: Frege systems, extended Frege systems, resolution, Horn resolution, the polynomial calculus, the sequent calculus, the cut-free sequent calculus, as well as the polynomial calculus. Our hardness of approximation results usually apply to proof length measured either by (...) number of symbols or by number of inferences, for tree-like or dag-like proofs. La narración se centra en la voz de un niño que crece en un Chile de postdictadura militar y, además, lo hace desde un hogar pinochetista. Esta voz se cruza con otros dos personajes, Andrés y Arnoldo, para formar un único relato. En este marco, la hipótesis de investigación sostiene que el encuentro con la mirada del animal posibilita la (...) emergencia de discursos alternos como los de un niño, un ebrio y un loco, a través de los cuales cada personaje puede reconocer a un otro y, al verse observado por ese otro, también puede descubrirse a sí mismo. To pose the question of evaluating political poetry is, of course, itself already a polemical move, since it insists on distinctions that command neither general critical consent nor methodological specificity. Repudiating the pertinence of such concerns to poetry has been, after all, the principal thrust of some of the most influential texts in modern literary theory. Indeed, considered historically, the struggle to separate aesthetic from both moral and political considerations can be seen as constituting the inaugural, grounding act of poetics (...) as a distinct discipline. In such a view, the words of a poem, by their very nature, are radically divorced from their usage in the quotidian world of shared human activities, so that although a text may contain political themes among its material poetica, insofar as it succeeds as a work of art these must function purely as internal and autonomous elements in the structure of the piece, not as arguments seeking to participate in a wider discourse. Because the language of poetry is unique and self-sufficient, thematic considerations are strictly irrelevant, and the issue of evaluation is identical regardless of the ostensible subject matter of the poem. Political poetry, in other words, is a meaningless term: a work is either a poem or it is not, and any attempt to include political concerns in its creation or evaluation is simply to abandon the domain of art for what Mallarmé dismissed as the debased idiom of “les journaux.”1Yet the very need to keep insisting on so categorical a distinction reveals that contamination is always possible, that the chasm may prove only a threshold habitually traversed by the words of any poem. And in fact, for every instance of a Mallarméan insistence upon the autonomy of the poem, there exists a counterpolemic stressing the link between word and world and, more pertinently still, between the language of verse and a search for values applicable to the communal experiences of both author and readers.2 But as I remarked earlier, the very heterogeneity of these arguments tends to deprive them of any methodological specificity, and all too often discussions of political poetry have done little more than catalog judgments about the ideological stance of a given work according to a critic’s fixed conception of which attitudes merit approval and which deserve censure. There is a crucial distinction between reading political poetry and reading poetry politically. Although America’s founders may have been inspired by the political thought of ancient Greece and Rome, the United States is more often characterized by its devotion to the pursuit of commerce. Some have even said that a modern commercial republic such as the United States unavoidably lowers its moral horizon to little more than a concern with securing peace and prosperity so that commerce can flourish. Michael Chan reconsiders this view of America through close readings of Aristotle and Alexander (...) Hamilton, showing that America at its founding was neither as modern nor as low as we have been led to believe. He challenges the virtue/commerce divide that dominates modern thought by demonstrating that the prevailing views of Aristotle and Hamilton on commerce reflect misleading half-truths. Chan first examines Aristotle’s views of economics as presented in the _Politics_, arguing that Aristotle was not as hostile to commerce as is commonly believed. He points out the philosopher’s belief in the value of commercial acquisition in the interest of supplying citizens with the “equipment of virtue,” citing Aristotle’s praise of commercial Carthage over agrarian but much-esteemed Sparta. Chan then turns to a detailed account of the political economy of Hamilton, a proponent of an advanced industrial republic modeled on Great Britain. While many take Hamilton’s advocacy of public credit, a national bank, and manufacturing as evidence of his rejection of classical republican thought in favor of modernity, Chan contends that Hamilton embraced a classically inspired economic statesmanship that transcended a concern with merely securing peace and prosperity. Leading the reader through the complexities of Hamilton’s thought, Chan shows that he intended commerce to pursue the wider classical goals of forming the character of citizens, establishing harmony and justice, and pursuing national greatness. Rather than attempting to brand Hamilton an Aristotelian, Chan seeks to incorporate into the study of Hamilton’s political economy what Aristotle himself regarded as the statesman’s characteristic virtue, prudence. By reflecting on Hamilton in the context of Aristotle’s own reflections on commerce, Chan casts him in a new light that cuts across the ongoing debate about liberal versus classical republican elements of the American founding. His cogent analysis also raises important questions regarding the American system as it is being challenged by conflicting worldviews. _Aristotle and Hamilton on Commerce and Statesmanship _makes a significant contribution to our understanding of both Hamiltonian thought and the moral worthiness of democratic capitalism. Philosopher Michael Boylan and theologian James A. Donahue provide a framework_compatible with humanist and theist beliefs_that will enable college and university professors to address a full range of ethical issues as they arise in classroom discussion, both in the academic disciplines and in professional education.

In this essay we respond to some criticisms of the guidance theory of representation offered by Tom Roberts. We argue that although Roberts’ criticisms miss their mark, he raises the important issue of the relationship between affordances and the action-oriented representations proposed by the guidance theory. Affordances play a prominent role in the anti-representationalist accounts offered by theorists of embodied cognition and ecological psychology, and the guidance theory is motivated in part by a desire to respond to the critiques of (...) representationalism offered in such accounts, without giving up entirely on the idea that representations are an important part of the cognitive economy of many animals. Thus, explorations of whether and how such accounts can in fact be related and reconciled potentially offer to shed some light on this ongoing controversy. Although the current essay hardly settles the larger debate, it does suggest that there may be more possibility for agreement than is often supposed. (shrink)

This paper discusses the notion of epistemic circularity, supposedly different from logical circularity, and evaluates Ernest Sosa’s claim that this specific kind of circular reasoning is virtuous rather than vicious. I attempt to determine whether or not the conditions said to make epistemic circularity a permissible instance of begging the question could make other instances of circular reasoning equally permissible.
