Newborn bloodspot screening programs are some of the longest running population screening programs internationally. Debate continues regarding the need for parents to give consent to having their child screened. Little attention has been paid to how meanings of consent-related terminology vary among stakeholders and the implications of this for practice. We undertook semi-structured interviews with parents, healthcare professionals and policy decision makers in two Canadian provinces. Conceptions of consent-related terms revolved around seven factors within two broad domains, decision-making and information (...) attainment. Decision-making comprised: parent decision authority; voluntariness; parent engagement with decision-making; and the process of enacting choice. Information ascertainment comprised: professional responsibilities ; parent responsibilities; and the need for discussion and understanding prior to a decision. Our findings indicate that consent-related terms are variously understood, with substantive implications for practice. We suggest that consent procedures should be explained descriptively, regardless of approach, so there are clear indications of what is expected of parents and healthcare professionals. Support systems are required both to meet the educational needs of parents and families and to support healthcare professionals in delivering information in a manner in keeping with parent needs. In this article, I will explore how the underlying research values of ‘openness’ and ‘mutual responsiveness’, which are central to open science practices, can be integrated into a new ethos of science. Firstly, I will revisit Robert Merton's early contribution to this issue, examining whether the ethos of science should be understood as a set of norms for scientists to practice ‘good’ science or as a set of research values as a functional requirement of the scientific system to produce knowledge, (...) irrespective of individual adherence to these norms. Secondly, I will analyse the recent codification of scientific practice in terms of ‘scientific integrity’, a framework that Merton did not pursue. Based on this analysis, and illustrated on the case of COVID-19 as a case in which the institution of science was challenged to deliver urgently on societal desirable outcomes, I will argue that promoting open science and its core norms of collaboration and openness requires broader governance of the institution of science in its relationship with society at large, rather than relying solely on self-governance within the scientific community through a new ethos of science. This conclusion has implications for re-evaluating research assessments, suggesting that the evaluation of the scientific system should take precedence over evaluating individual researchers, and that incentives should be provided to encourage specific research behaviour rather than solely focusing on individual research outputs. Moral distress has been identified as an occupational hazard for clinicians caring for vulnerable populations. The aim of this systematic review was (i) to summarize the literature reporting on prevalence of, and factors related to, moral distress among nurses within acute mental health settings, and (ii) to examine the efficacy of interventions designed to address moral distress among nurses within this clinical setting. A comprehensive literature search was conducted in October 2022 utilizing Nursing & Allied Health, Embase, CINAHL, PsychInfo, and (...) PubMed databases to identify eligible studies published in English from January 2000 to October 2022. Ten studies met inclusion criteria. Four quantitative studies assessed moral distress among nurses in acute mental health settings and examined relationships between moral distress and other psychological and work-related variables. Six qualitative studies explored the phenomenon of moral distress as experienced by nurses working in acute mental health settings. The quantitative studies assessed moral distress using the Moral Distress Scale for Psychiatric Nurses (MDS-P) or the Work-Related Moral Stress Questionnaire. These studies identified relationships between moral distress and emotional exhaustion, depersonalization, cynicism, poorer job satisfaction, less sense of coherence, poorer moral climate, and less experience of moral support. Qualitative studies revealed factors associated with moral distress, including lack of action, poor conduct by colleagues, time pressures, professional, policy and legal implications, aggression, and patient safety. No interventions targeting moral distress among nurses in acute mental health settings were identified. Overall, this review identified that moral distress is prevalent among nurses working in acute mental health settings and is associated with poorer outcomes for nurses, patients, and organizations. Research is urgently needed to develop and test evidence-based interventions to address moral distress among mental health nurses and to evaluate individual and system-level intervention effects on nurses, clinical care, and patient outcomes. The way Leibniz applied his philosophy to mathematics has been the subject of longstanding debates. A key piece of evidence is his letter to Masson on bodies. We offer an interpretation of this often misunderstood text, dealing with the status of infinite divisibility innature, rather than inmathematics. In line with this distinction, we offer a reading of the fictionality of infinitesimals. The letter has been claimed to support a reading of infinitesimals according to which they are logical fictions, contradictory in (...) their definition, and thus absolutely impossible. The advocates of such a reading have lumped infinitesimals with infinite wholes, which are rejected by Leibniz as contradicting the part–whole principle. Far from supporting this reading, the letter is arguably consistent with the view that infinitesimals, as inassignable quantities, arementis fictiones, i.e., (well-founded) fictions usable in mathematics, but possibly contrary to the Leibnizian principle of the harmony of things and not necessarily idealizing anything inrerum natura. Unlike infinite wholes, infinitesimals—as well as imaginary roots and other well-founded fictions—may involve accidental (as opposed to absolute) impossibilities, in accordance with the Leibnizian theories of knowledge and modality. Objective: Quality of Life is an important predictor of patient's recovery and survival in lung cancer patients. The aim of the present study is to identify 1-year trends of lung cancer patients' QoL after robot-assisted or traditional lobectomy and investigate whether clinical and sociodemographic variables may predict these trends.Methods: An Italian sample of 176 lung cancer patients undergoing lobectomy completed the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire—Core 30 at the pre-hospitalization, 30 days, 4 months, (...) 8 months, and 12 months after surgery. Sociodemographic and clinical characteristics were also collected. The individual change over time of the 15 dimensions of the EORTC QLQ-C30 and the effects of pre-surgery scores of QoL dimensions, type of surgery, perioperative complications, and age on patients' QoL after surgery were studied with the individual growth curve models.Results: Patients had a good recovery after lobectomy: functioning subscales improved over time, while most of the symptoms became less severe over the care process. Perioperative complications, type of surgery, pre-surgery status, and age significantly affected these trends, thus becoming predictors of patients' QoL.Conclusion: This study highlights different 1-year trends of lung cancer patients' QoL. The measurement of pre- and post-surgery QoL and its clinical and sociodemographic covariables would be necessary to better investigate patients' care process and implement personalized medicine in lung cancer hospital divisions. Ao apurar os ouvidos sobre a "polêmica do samba" observa-se que as composições revelam tensões inexoráveis entre um mundo festivo do "malandro" e os limites da realidade da época. Desse modo, com suporte em estudos que tratam deste (...) tema, busca-se trazer à tona um insight sobre este "duelo" musical que está, certamente, muito mais disseminado na sociedade que restrito a questões puramente estéticas.ABSTRACTThe purpose of this paper is to analyze the meanders of composition and their intertextuality in the samba controversy between Noel Rosa and Wilson Batista. We observe dialogism in the verbal and musical interaction between the samba writers and several texts of the 1930s. When ears are in tune with the "samba controversy," one observes that the compositions reveal inexorable tensions between the festive world of the "malandro" and the limits of the reality of the time. Accordingly, based on studies that address this theme, we aim to present an insight into this musical "duel," which is not restricted to purely aesthetics issues, but certainly disseminated in society. Background Pragmatic randomized controlled trials are designed to evaluate the effectiveness of interventions in real-world clinical conditions. However, these studies raise ethical issues for researchers and regulators. Our objective is to identify a list of key ethical issues in pragmatic RCTs and highlight gaps in the ethics literature. Methods We conducted a scoping review of articles addressing ethical aspects of pragmatic RCTs. After applying the search strategy and eligibility criteria, 36 articles were included and reviewed using content analysis. Results Our (...) review identified four major themes: 1) the research-practice distinction; 2) the need for consent; 3) elements that must be disclosed in the consent process; and 4) appropriate oversight by research ethics committees. 1) Most authors reject the need for a research-practice distinction in pragmatic RCTs. They argue that the distinction rests on the presumptions that research participation offers patients less benefit and greater risk than clinical practice, but neither is true in the case of pragmatic RCTs. 2) Most authors further conclude that pragmatic RCTs may proceed without informed consent or with simplified consent procedures when risks are low and consent is infeasible. 3) Authors who endorse the need for consent assert that information need only be disclosed when research participation poses incremental risks compared to clinical practice. Authors disagree as to whether randomization must be disclosed. 4) Finally, all authors view regulatory oversight as burdensome and a practical impediment to the conduct of pragmatic RCTs, and argue that oversight procedures ought to be streamlined when risks to participants are low. Conclusion The current ethical discussion is framed by the assumption that the function of research oversight is to protect participants from risk. As pragmatic RCTs commonly involve usual care interventions, the risks may be minimal. This leads many to reject the research-practice distinction and question the need for informed consent. But the function of oversight should be understood broadly as protecting the liberty and welfare interest of participants and promoting public trust in research. This understanding, we suggest, will focus discussion on questions about appropriate ethical review for pragmatic RCTs. (shrink)

This paper is about syllogistic reasoning, i.e., reasoning from such pairs of premises as, All the chefs are musicians; some of the musicians are painters. We present a computer model that implements the latest account of syllogisms, which is based on the theory of mental models. We also report four experiments that were designed to test this account. Experiments 1 and 2 examined the strategies revealed by the participants' use of paper and pencil as aids to reasoning. Experiment 3 used (...) a new technique to externalize thinking. The participants had to refute, if possible, putative conclusions by constructing external models that were examples of the premises but counterexamples of the conclusions. Experiment 4 used the same techniques to examine the participants' strategies as they drew their own conclusions from syllogistic premises. The results of the experiments showed that individuals not trained in logic can construct counterexamples, that they use similar operations to those implemented in the computer model, but that they rely on a much greater variety of interpretations of premises and of search strategies than the computer model does. We re‐evaluates current theories of syllogistic reasoning in the light of these results. (shrink)

Les nombreux débats conflictuels entourant l’application du principe de grève de masse au sein du Parti social-démocrate allemand témoignent de la lente agonie des idéaux marxistes au sein de ce parti de masse. La fragile victoire des idéaux prônés par la faction radicale du Parti contre ceux des réformistes, entre 1900 et 1906, connaît un retour de balancier sans appel après la première révolution russe. De 1906 à 1910, la logique légalo-parlementaire allait progressivement devenir la seule voie légitime aux yeux (...) des dirigeants du Parti. Ce changement de tactique et de stratégie visant à conquérir le pouvoir, sera grandement contesté par plusieurs militants influents du Parti. Le conflit intellectuel entre Rosa Luxemburg et Karl Kautsky, qui éclata publiquement en 1910 alors que les mouvements contestataires en Allemagne étaient très forts, nous permet de comprendre, par l’analyse du discours de ces penseurs marxistes, les raisons qui conduisirent le SPD à ne pas appliquer la grève de masse. Ceci, alors qu’une motion favorable à ce principe avait été votée en 1905 lors du Congrès d’Iéna. Du point de vue pseudo-sérendipien qui était le sien, Luxemburg croyait fermement que la temporalité était favorable à l’application de la grève de masse. L’interprétation temporelle et électoraliste de Kautsky l’amènera plutôt à critiquer les effets pseudo-sérendipiens, non contrôlés par le SPD, entourant ce type d’action politique. (shrink)

The diversity of bone proteoglycan (PG) structure and localisation (pericellular, extracellular in the organic bone matrix) reflects a broad spectrum of biological functions within a unique tissue. PGs play important roles in organizing the bone extracellular matrix, taking part in the structuring of the tissue itself as active regulators of collagen fibrillogenesis. PGs also display selective patterns of reactivity with several constituents including cytokines and growth factors, such as transforming growth factor‐β or osteoprotegerin thereby modulating their bio‐availability and biological activity (...) in the bone tissue. In this review, the complex PG composition in bone will be addressed together with the specific role played by PGs (or their GAGs chains) in bone biology, as regulatory molecules for bone resorption and their involvement in bone tumor development. These roles have been determined after modulation of PG expression or mutations in their corresponding genes, which revealed specific roles for these compounds in bone pathologies (e.g. perlecan or glypican‐3 mutations observed respectively in chondrodysplasia or dysmorphic syndrome). Finally, the potential therapeutic interest of PGs is discussed based on recent data, more particularly on bone tumor‐associated osteolysis as these molecules are involved both in bone resorption and tumor development. This paper examines folk theories of algorithmic recommendations on Spotify in order to make visible the cultural specificities of data assemblages in the global South. The study was conducted in Costa Rica and draws on triangulated data from 30 interviews, 4 focus groups with 22 users, and the study of “rich pictures” made by individuals to graphically represent their understanding of algorithmic recommendations. We found two main folk theories: one that personifies Spotify and another one that envisions it as a (...) system full of resources. Whereas the first theory emphasizes local conceptions of social relations to make sense of algorithms, the second one stresses the role of algorithms in providing a global experience of music and technology. We analyze why people espouse either one of these theories and how these theories provide users with resources to enact different modalities of power and resistance in relation to recommendation algorithms. We argue that folk theories thus offer a productive way to broaden understanding of what agency means in relation to algorithms. (shrink)

In this article I reconsider the issue of ?transfer? in education. Received views of learning transfer tend to rely upon a version of representation in which the world and the learner are held apart. The focus falls on how this gap can be closed; how learning can be transferred. A sociomaterial perspective, by contrast, puts learner and world back together, making each available to the other. Bringing the materialist sensibility of actor-network theory to bear and drawing on empirical data collected (...) as part of a small-scale qualitative study of the experience of graduate teachers when moving from education into work, it is argued that transfer, and by implication, learning, primarily concerns the practical and takes multiple forms: contingently composed of social, textual and material practices of knowledge production, learning transfer is a relational effect of the intersection of these practices. Empirical analyses point to the practice of two broad patterns of learning transfer, termed here the representational and the relational. Thinking learning transfer as performed through disparate agencies and practices challenges the self-evidences of perspectives on learning which characterize contemporary education. Here, learning is primarily seen in terms of the intrinsic capabilities of people, regardless of the object-dependent qualities of their learning and lives. This human-centricity raises significant epistemological and ethical issues which are addressed by way of a discussion of the embedded normativities and politics of the practices of representational and relational transfer. "Towards the Death of Humanity" is the endless demonstration of the disastrous side effects left on our environment, on life on this planet, on health and most of all on human dehumanization by a century of tremendous scientific and technological realizations and their material values. It illustrates how these unhealthy side effects are highly linked to the hasty and thoughtless decisions of scientists, intellectuals and governments to replace the humanities and the traditional methods of teaching with their own methods of (...) science, technology and their essentially material values. Unfortunately totally deprived of the smallest bit of humanity, this method of education has killed the rudiments of humanity acquired during the course of history, has dehumanized the people, their institutions and their societies. These side effects originate in the age-old desire of intellectuals and scientists to take hold of the power of religion over man and to satisfy their morbid need to build a new world, re-designed following Hegel' belief that the matter is eternal and God. Although these ideologies have lost all their legitimacy today because science has recently demonstrated that the matter is not eternal, scientists and intellectuals keep on teaching the falseness of their materialistic social system as if the logic of their reasoning were the only truth. This work promotes certain guidelines that individuals must follow to make the lucid and effective compromises needed to try and solve these destructive problems affecting man and society before the end of History. S'interrogeant sur l'usage stratégique et sur l'utilité théorique de la notion de « postféminisme », cet article développe une argumentation en trois temps. D'abord, il prend acte de la modification de la réalité sociale des femmes sous l'impact des luttes féministes. Ensuite, il évalue l'impact de ces transformations à la fois sur le mouvement féministe et sur les conditions de possibilité d'une politique féministe. Enfin, il traite du pluralisme sous deux angles, à savoir comme moyen de surmonter les apories des (...) perspectives de l'égalité, de la différence et de l'autonomie qui ont marqué le féminisme contemporain et pouvant se matérialiser dans la notion arendtienne de « paria rebelle » comme modèle d'action politique. Cette réflexion débouche sur le constat que le féminisme a toujours un sens et qu'il serait prématuré de s'en départir.What strategic device lie behind the idea of "postfeminism" ? Could it be of any use for feminist theory ? This article attemps to answer these questions in a threefold argument. First, it shows how feminism has succeeded in transforming women's life. Second, it analyzes the impact of these transformations both on the women's movement and on the very possibility of a feminist politics. Third, it deals with pluralism; on the one hand, pluralism is conceived as a way out of the dilemmas of the exclusive constructivist, essentialist or radical approach within contemporary feminism; on the other hand, using the arendtian figure of "the pariah as a rebel" it shows how it could provide a useful framework for feminism both on a theoritical and on a political level. It can only conclude that feminism still makes sense and that women cannot afford politically to get rid of it. (shrink)

O artigo analisa a ideologia da equipe econômica do governo Fernando Henrique Cardoso, que orientou sua ação por uma racionalidade instrumental. Foi feita uma análise comparativa com as equipes econômicas dos governos anteriores, mostrando que a equipe do governo Cardoso abandonou o modelo desenvolvimentista para retomar o modelo clássico liberal. No entanto, o neoliberalismo da equipe de Cardoso não entende o mercado como um meio para se promover o desenvolvimento econômico e social. Herdeiros da geração pós Consenso de Washington, vêem (...) o mercado como um fim em si. A relação que esses economistas estabeleceram com o mercado é quase religiosa, porque nele depositaram total confiança. Para estes economistas, era como se o mercado fosse uma entidade superior que naturalmente resolveria os problemas econômicos e sociais. Sob esse pretexto, colocaram em prática no país políticas típicas do neoliberalismo, que prevêem o desmantelamento do Estado e das grandes conquistas políticas e sociais. Na verdade, essa idolatria em relação mercado configura a irracionalidade desses economistas que defendem explicitamente os interesses de um segmento da classe dominante, a saber, a burguesia financeira internacional. O que leva à conclusão de que não houve autonomia da equipe para elaborar as políticas econômicas do governo, porque seguem as receitas dos organismos internacionais. A autonomia só se estabeleceu em relação às classes trabalhadoras, que não participaram de qualquer negociação que lhes dizem respeito. (shrink)

There is a variety of way to organize and manage spatially knowledge in literature. In this present inquire only a few type relating to the context of education will be analyzed: two of the most widespread and oft-used in the field of didactic pedagogy and one that has recently been scientifically affirmed—though its study dates back twenty years. The parallel—and sometimes intertwining—analysis of these three interpretive perspectives could serve as an epistemological scaffolding allowing us to look out on a holistic (...) experimental paradigm on the topic of spatialization in/of cognitive processes. The present study explores the idea that human linguistic communication co-opted a pre-existing population-wide behavioural system that was shared among social group members and whose structure reflected the structure of the environment. This system is hypothesized to have emerged from interactions among individuals who had evolved the capacity to imitate arbitrary, functionless behaviour. A series of agent-based computer simulations test the separate and joint effects of imitation, pattern completion behaviour, environment structure and level of social interaction on such a population-wide (...) behavioural system. The results support the view that a system that could be co-opted for linguistic communication might arise in a population of agents equipped with arbitrary imitation for the purposes of pattern completion interacting in certain kinds of structured environments. Such pre-linguistic behavioural system could have bootstrapped communication and paved the way for biological capacities widely believed to be necessary for communication, such as shared intentionality and symbolicity, to evolve. The promise of genomic medicine lies in the opportunity to improve health outcomes via a personalized approach to management, grounded in genetic and genomic variation unique to an individual. However, disparities and inequities mar this remarkable landscape of genomic innovation. Prior efforts to understand these inequities have focused on populations for which genetic testing is relatively protocolized or where test utility varies greatly by ancestry groups, where equitable outcomes are more clearly defined. We therefore consider the current landscape of rare (...) disease genomics, in which diagnostic approaches vary widely and utility remains to be fully understood, and suggest a path forward: how ecosocial theory may be used to guide novel equity‐focused initiatives that incorporate illness narratives to improve population health. We present examples of narrative medicine in rare disease and reimagine the role this discipline may play in genomic sequencing studies, toward incorporation of the unique illness narrative into clinical genetics and genomics practice. Approaches that broaden the definitions of disease and of outcomes of interest will force the field to grapple with its racist history and begin to advance health equity and promote justice so that genomic medicine may truly deliver on its promise. To date, COVID-19 has spread across the world, changing our way of life and forcing us to wear face masks. This report demonstrates that face masks influence the human ability to infer emotions by observing facial configurations. Specifically, a mask obstructing a face limits the ability of people of all ages to infer emotions expressed by facial features, but the difficulties associated with the mask’s use are significantly pronounced in children aged between 3 and 5 years old. These findings are (...) of essential importance, as they suggest that we live in a time that may potentially affect the development of social and emotion reasoning, and young children’s future social abilities should be monitored to assess the true impact of the use of masks. This essay examines how early moderns used birth horoscopes (genitures) to assess the compatibility of prospective spouses before marriage. Astrologers could probe the horoscope of an individual to investigate his or her present and future physical and moral qualities or compare charts to reveal the personal compatibility of a couple and help establish the best time to consummate their marriage. These practices aimed at ensuring a fruitful marriage and the harmony and happiness of the couple and their families. Focusing on (...) a few key examples, the essay outlines both the astrological theories and the social politics that propelled astrological matching, suggesting that its appeal lay in the promise of informed choice while also preserving free will. Finally, the author suggests ways in which astrological practices and the use of vast amounts of astronomical data share affinities with data-driven matching in our own time. (shrink)

Discussions of the framework and terminology associated with the right to health tend to treat the indeterminacy of ‘health’ as conceptual noise that the construction of effective policy must not focus on, but find ways of bracketing out. On this basis, the right to health is broadly regarded as a social and economic, rather than a civil and political right. This article draws critically on literature about the implications of developments in medical biotechnologies, to argue that a positive acknowledgement of (...) the indeterminate character of health should transform, rather than simply hinder, the quality of debate over what is to be understood and expected in connection with a right to health. A focus on indeterminacy allows for the perception and the formulation of health-related demands that may not stem from the scarcity of material resources or technical means, but from the misplaced authority of particular voices in defining what possibilities are to be seriously envisaged. This proposition only becomes politically effective, it is argued, when ‘indeterminacy’ (and the capacity for normativity) is referred to life itself and not merely to social and moral judgements about life. Although more immediately pertinent to the concerns of relatively privileged populations, the focus on indeterminacy provides a key to generating a certain symmetry and complementarity of interest, across the privileged/underprivileged divide, in promoting health as a (human) right. The issues involved in deciding whether to use artificial methods of delivering hydration and nutrition are often very difficult for patients, families, and health care providers. Once private and personal matters, these decisions now frequently involve the judicial system. Five years ago, Hospice of Washington recognized the need for a written policy and wrote the one published here. Its goal is to respect individual preferences and family concerns while addressing the nutrition and hydration needs of dying patients.
