Clinicians at quaternary centers see part of their mission as providing hope when others cannot. They tend to see sicker patients with more complex disease processes. Part of this mission is offering longshot treatment modalities that are unlikely to achieve their stated goal, but conceivably could. When patients embark on such a treatment plan, it may fail. Often treatment toward an initial goal continues beyond the point at which such a goal is feasible. We explore the progression of care from (...) longshot to fantasy using two pediatric cases. This progression may be differentiated into four distinct stages of care related to the potential of achieving the initial goals of care. Physicians are often ill prepared for the progression of treatments from a longshot hope to an unfeasible and, therefore, typically unjustified intervention. We present a structured approach to guide clinicians at referral institutions where these situations may be common. The transition of care from “longshot” to “fantasy” is an inherent part of quaternary care for the sickest of patients that has been underexplored. Physicians are often poorly equipped to approach that transition. We advocate this approach to the shift from longshot to fantasy with the belief that such a structured method will have multiple benefits, including: reduced suffering for the patient; decreased emotional burden on patient and family; decreased provider moral distress; increased likelihood of seeking high quality palliative care earlier; and provision of honest and straightforward information to patients and their families. The object of this paper is to examine half and full connexive extensions of the basic regular conditional logic CR. Extensions of this system are of interest because it is among the strongest well-known systems of conditional logic that can be augmented with connexive theses without inconsistency resulting. These connexive extensions are characterized axiomatically and their relations to one another are examined proof-theoretically. Subsequently, algebraic semantics are given and soundness, completeness, and decidability are proved for each system. The semantics is (...) also used to establish independence results. The practice of paternalism has changed along with developments in medicine, philosophy, law, sociology and psychology. Physicians have learned that a patient's values are a factor in determining what is best for that patient. Modern paternalism continues to be guided by the principle that the physician decides what is best for the patient and pursues that course of action, taking into account the values and interests of the patient. In the autonomy model of the doctor-patient relationship, patient values are decisive. (...) In the paternalistic model, they are but one among several factors the physician must consider in making a medical decision. Although difficult to practise because of limitations in empathising with another person, modern paternalism remains a way to achieve maximum patient benefit. (shrink)

Over the last decade, the international media has devoted increasing attention to operations that separate conjoined twins. Despite the fairly low odds that a child or adult will survive the operation with all of their vital organs intact, most people fail to question the urgency of being physically separated from one’s identical twin. The drive to surgically tear asunder that which was originally joined, I suggest, is motivated in part by a refusal to acknowledge intercorporeality as a basic condition of (...) human existence that doesn’t undermine identity but makes it possible in the first place. In his Outlines of Pyrrhonism 2.110–113, Sextus Empiricus presents four different accounts of the conditional, presumably all from the Hellenistic period, in increasing logical strength. While the interpretation and provenance of the first three accounts is relatively secure, the fourth account has perplexed and frustrated interpreters for decades or longer. Most interpreters have ultimately taken a dismissive attitude towards the fourth account and discounted it as being of both little historical and logical interest. We argue that this attitude is unwarranted (...) and demonstrate that the conditional expressed in the fourth account can profitably be understood as a precursor of analytic entailment, familiar from containment logics such as those developed by Parry and Angell. Exploiting recent work by Fine, we present a formal truthmaker semantics for this conditional and show how it sheds light on a number of longstanding issues in the interpretation of this passage. In this chapter, I present a systematic study of theories of entailment as developed in relevant modal logics of varying strength. I consider several relevant normal modal logics and the properties of entailment, as defined as necessitated relevant implication, in these systems. Fine-Urquhart style semantics are provided for these relevant modal logics and proofs of soundness and completeness are given. The semantics is used to prove admissibility results for certain weak relevant modal theories. Connections between some of these relevant modal (...) logics and preexisting theories of entailment are pointed out. (shrink)

Nietzsche sees westem philosophy and culture as dorninated by the metaphysical belief in opposites. The first and second sections of this paper spell out the basic assumptions underlying this belief and discuss the distinction between the “true” and the “apparent” world as the primary opposite by reference to which all opposites are determined. Section three employs Nietzsche’s idea of the will to power to analyze the belief in opposites as an expression of a weak and sickly type of Iife seeking (...) to revenge itself upon the natural world. Section four tums to Nietzsche’s denial of the belief in opposites and examines how he dissolves the distinction between the “true” and “apparent” world. The final section ofthe paper presents Nietzsche’s counter-analysis of metaphysical opposites within epistemology, religion, aesthetics, and morality. He regards “opposites” as contraries which exist along a continuum of natural phenomena, and he introduces the notion of sublimation to explain how one contrary can eventually give rise to its extreme. This edited volume brings together papers by both eminent and rising scholars to celebrate Saul Kripke’s singular contributions to modal logic. Kripke’s work on modal logic helped usher in a new semantic epoch for the field and made facility with modal logic indispensable not only to technically oriented philosophers but to theoretical computer scientists and others as well. This volume features previously unpublished work of Kripke’s as well as a brief intellectual biography recounting the story of how Kripke became interested (...) in, and made his first contributions to, modal logic. However, the majority of the volume’s contributions are forward-looking, and produce new philosophical and technical insights by engaging with ideas tracing back to Kripke. (shrink)

Challenging earlier cognitivist approaches, recent theories of embodied cognition argue that the human mind and its functions are best understood as intimately bound up with the human body and its physiological dimensions. Some scholars have suggested that such theories, in departing from some core assumptions of the Western philosophical tradition, display significant similarities to certain non-Western traditions of thought, such as Buddhism. This essay extends such parallels to the Jewish tradition and argues that, in particular, classical rabbinic thought presents a (...) profoundly nondualistic account of the body–soul relation in its connection to cognition, action, and embodiment. Classical rabbinic texts therefore model the possibility of engaging with ‘Western’ conceptions such as God and the soul, while doing so in a manner that resonates strongly with many aspects of contemporary scientific theories. Thus, beyond their value as historical documents, insight into the texts and concepts of classical rabbinic Judaism can contribute to the further development of new theories of intellect and cognition. Students of Hegel will soon have at their disposal a complete translation of the Encyclopaedia of the Philosophical Sciences, "the only complete, matured, and authentic statement of Hegel's philosophical system", and the last of the four major works published by Hegel in his lifetime to be fully translated into English. Early next year, the Clarendon Press at Oxford will issue a translation of the second part of the Encyclopaedia, The Philosophy of Nature, by A.V. Miller, recent translator of Hegel's Science (...) of Logic. The translation includes the valuable Zusätze, and was supervised throughout by Professor J.N. Findlay of Yale, who has also written an Introduction for it. With it will also be published a new edition of the Philosophy of Mind, the third part of the Encyclopaedia. The text of the paragraphs is the familiar Wallace translation, but added to this volume are the Zusätze newly translated by Mr. Millet, and a new Introduction by Professor Findlay. When the present edition of the first part of the Encyclopaedia, usually referred to as "the Lesser Logic" has been exhausted, if will also be republished with some revisions and additions. Professor Findlay adds that all the proofs were done last June, and that if will appear "at a price that students will not find prohibitive". (shrink)

This essay explores classical rabbinic literature's understanding of the prohibition of bloodshed alongside its understanding that "the image of God" corresponds to the physically embodied individual. This conception generates radical implications so that, apart from the narrow instance of a direct aggressor with intent to kill or rape, it is never legitimate to cause the death of any person, even in pursuit of a supposed "greater good." While notions of war and execution are retained in principle, the requirement of direct (...) divine sanction for such actions neutralizes them in practice, removing them from the domain of human judgment and justification. In Guide 2.32 Maimonides notes that just as there are three opinions concerning prophecy , so are there three opinions concerning cosmogony. Scholars have tended to assume that Maimonides, despite what he says, must have seen some more important correspondence between the two sets of opinions than their number. I argue that although for Maimonides what the two sets of opinions have in common is indeed their number, what he wishes to direct the careful reader's attention to is that the (...) number of opinions in both cases is actually two rather than three. Context: Josef Mitterer has become known for criticizing the main exponents of analytic and constructivist philosophy for their blind adoption of a dualistic epistemology based on an alleged ontological difference between world and words. Judith Butler, who has developed an influential model of (de)constructivist feminism and has been labeled a linguistic constructivist, has been criticized for sustaining exactly what, according to Mitterer, most modern philosophy fails to acknowledge: namely that there is no ontological difference between objective facts beyond language and (...) the discourse about these facts. Problem: In the scholarly discussion on non-dualism, two main questions have been raised: Where does Mitterer’s basic consensus, i.e., the starting-point description, come from? and: What does it mean, to say that further descriptions change their object? Method: Comparative analysis of the core concepts of Mitterer’s and Butler’s work in the context of the history of ideas. Results: Butler’s conception of a performative production of objectivity through discursive and non-discursive iterated practices can be interpreted as an illustration of Mitterer’s claim that descriptions change their object. The problem of where Mitterer’s starting-point descriptions come from can be solved by adopting Butler’s concept of culturally inherited practices. (shrink)

In his response to my article, “Historicism and Science: Thoughts on Quine” , Paul Bernays distinguishes between two aspects of the article: my criticism of Quine's theory of knowledge and my contention that the epistemological foundations of modern science lead to historicism. Bernays substantially accepts my criticism of Quine, particularly insofar as I reject Quine's behaviorism and “physicalism”. He opposes, however, my claim that historicism is implicit in the presuppositions of modern science. I argued that the historicism inherent in modern (...) science is due to the essentially changeable character of modern science. Quine's theory of knowledge tacitly takes account of this changeable character a) through his view of sense experience as being constituted by posits, b) through his recognition that individual scientific statements are dependent upon a broad linguistic‐conceptual framework that is itself subject to change, and c) through his pragmatic conception of reason and the concomitant emphasis upon the criterion of simplicity.
