Locked inpatient units are an increasing phenomenon, introduced in response to unforseen abscondences and suicides of patients. This paper identifies some value issues concerning the practice of locked psychiatric inpatient units. Broad strategies, practicalities and ethical matters that must be considered in inpatient mental health services are also explored. The authors draw on the published research and commentary to derive relevant information to provide to patients and staff regarding the aims and rationales of locked units. Further debate is warranted in (...) relation to best practice. Inpatient staff need to be aware of their practice values, be able to access education and supervision and negotiate apparent contradictions. Further patient/clinician focused enquiry is necessary to mitigate the negative and stigmatising effects of locked mental health units. (shrink)

In acute inpatient mental health services, patients commonly demonstrate extreme behaviours. A number of coercive practices, such as locked doors, enforced medication and seclusion, are used in these settings to control such behaviours. The aim of this report is to explore briefly some of the contemporary debates pertaining to seclusion. A perusal of the literature reveals a clarion call to end the practice of seclusion, without consideration of feasible alternatives. For well over two centuries now he has been a popular icon of unctuous self-righteousness, superstitious fanaticism, and dogmatic intolerance. Nor has endorsement of this stereotype been confined to casual laypersons who know of Mather only from lurid accounts of (...) Salem witch-hunting. Scholars who ought to know better often buy into it. Perry Miller, the doyen of early American studies, consistently wrote of Mather in a particularly unsympathetic manner. More odious, as Professor Solberg reminds us (p. xciii), was the treatment by historians at a Florida planetarium who in 1982 put together a short film on early European and American doomsday predictors which included segments on Mather the astronomer. When these segments appeared, a projector was programmed to flash the words “Feel Free to Boo and Hiss” on the planetarium’s dome—an invitation which audiences, presumably, gleefully accepted.Mather deserves better than this, and Winton Solberg’s critical edition of The Christian Philosopher goes a long way towards rehabilitating the Puritan divine. Along with recent studies such as Kenneth Murdock’s edition of Mather’s Magnalia Christi Americana (1977) and Kenneth Silverman’s masterful The Life and Times of Cotton Mather (1984), Solberg’s study painstakingly presents us with a detailed account of a man who undoubtedly was one of the most important literary figures in early America.Mather published The Christian Philosopher in 1721, seven years before his [End Page 167] death and eight years after being named to the Royal Society of London. The book was both the culmination of a lifetime of learning as well as a calculated effort on Mather’s part to show that his election to the prestigious Royal Society was deserved. The book is the first attempt by an American-born author to take a comprehensive look at the “new learning” associated with that period known since as the Enlightenment. Mather was an omnivorous reader and an equally thorough observer of natural phenomena, and he welcomed the scientific interpretations of reality offered by such savants as Isaac Newton, Robert Boyle, and John Ray. In the mind of Mather, such explanations posed no challenge to the Christian worldview. On the contrary, he concluded, they supported it by providing clear and indisputable evidence of design in nature. The Christian Philosopher (or, in more contemporary jargon, The Christian Scientist) is Mather’s attempt to defend this thesis. It is an exhaustive (and sometimes exhausting!) compendium of data gleaned from the natural realm which, in Mather’s judgment, irrefutably points to the existence of a grand Designer. Although Mather explicitly claimed that his primary goal in writing the book was to demonstrate that atheism is contrary to reason, he also penned one of the New World’s first systematic Enlightenment documents.One of the difficulties in reading The Christian Philosopher traditionally has been the sheer breadth of learning that went into its composition. Mather cites over four hundred books, includes hundreds of allusions to classical and scriptural sources, and so inundates the reader with evidence in support of his fundamental design argument that it is quite easy to lose one’s way. Professor Solberg has considerably lightened these burdens with his critical edition. He prefaces Mather’s text with a masterful 115-page introduction which places the work in its historical context as well as analyzes its methodology and arguments. Moreover, his edition includes 170 pages of endnotes, biographical notes, biblical references and a recapitulation of Mather’s own sources. Finally, Solberg supplies almost each page of Mather’s text with footnotes that identify literary or scriptural allusions and translate Mather’s liberal use of Greek and Latin quotations.Solberg’s edition of The Christian Philosopher will prove, I’m confident, to be the definitive text. It is an invaluable reference tool for the specialist, but it can also serve as a friendly and intriguing introduction for the layperson. Despite enthusiasm for patient-centered care, the practice of patient-centered care is proving challenging. Further, it is curious that the literature about this subject does not explicitly address patient autonomy, since patients guide care in patient-centered care, and respect for patient autonomy is a prominent health-care value. We argue that by explicitly adopting a relational conception of autonomy as an essential component, patient-centered care becomes more coherent, is strengthened, and could help practitioners to make better use of a principle of respect (...) for autonomy. Neuroimaging, psychosurgery, deep-brain stimulation, and psychopharmacology hold considerable promise for more accurate prediction and diagnosis and more effective treatment of neurological and psychiatric disorders. Some forms of psychopharmacology may even be able to enhance normal cognitive and affective capacities. But the brain remains the most complex and least understood of all the organs in the human body. Mapping the neural correlates of the mind through brain scans, and altering these correlates through surgery, stimulation, or pharmacological interventions can affect us in (...) both positive and negative ways. We need to carefully weigh the potential benefit against the potential harm of such techniques. Recently, scholars have argued that disclosure of personal information is an effective mechanism for building high-quality relationships. However, personal information can focus attention on differences in demographically diverse teams. In an experiment using 37 undergraduate teams, we examine how sharing personal information by ethnically similar and ethnically distinct newcomers to a team affects team perceptions, performance, and behavior. Our findings indicate that the disclosure of personal information by ethnically distinct newcomers improves team performance. However, the positive impact on team performance (...) comes at a cost to the newcomers, who are perceived as less competent by others and experience heightened social discomfort in team interactions. Ironically, what benefits the ethnically diverse team may undermine its ethnically distinct members. This study highlights how the management of diversity may sometimes require making trade-offs between individual interests and those of the team. (shrink)

By exploring the possibility of self-development in African female protagonists, this engrossing work augments existing feminist critiques that have analyzed the possibilities and potentials for the individuation of mostly Western, mostly European female characters. In this radical reinterpretation of Aristotle's Metaphysics, Walter E. Wehrle demonstrates that developmental theories of Aristotle are based on a faulty assumption: that the fifth chapter of Categories is an early theory of metaphysics that Aristotle later abandoned.

The author defends the anti-representationalist claim that the formation of the proper names (and as a consequence – scientific terms or notions) cannot happen through certain ostensive pointing at some objects given here and now (like in B. Russell’s theory) or through perceptions which are generalized inductively or by means of Kantian apperception or Anschauung. In order to answer the question about the concepts formation we have to take into account the historical and socio-cultural background of the genesis of proper (...) names which form the foundation and boundary of all classifications including the scientific ones. The author claims that there is an important difference between a personal belief or propositional knowledge and some implicit or background knowledge of the language community in its historical development. The first one could be evaluated on its truth / falseness. The second one however – being the foundation for the first one – cannot be evaluated in this manner. It simply is as it is. (shrink)

This article considers the development of nurse-led services as a part of a pilot study and explores the therapeutic nature of the role of the nurse. In particular it suggests a need for reconsideration of the fundamental values of nurse-led care in the context of changing organizational culture. Within the UK there has been pressure from policy makers to extend the role of the specialist nurse and create new nursing roles, shifting the boundaries between professional health groups. The philosophy of (...) nurse-led initiatives has therefore been driven mainly from a service redesign and clinical need standpoint rather than necessarily focusing on enhancing patients’ experience and the changes in organizational culture required to achieve this. While several studies have focused on the safety, comparative cost and comparative patient outcomes in nurse-led care in relation to traditional or doctor-led care, little attention has been given to the changing organizational values underlying the nursing role. Exploring this context is essential if new nursing roles are to provide more than relief for bottlenecks in the system and also meet their potential for providing patient centred and innovative models of care.
