Diabetes mellitus (DM) is a colossal burden in global health, its prevalence increases steadily throughout the world. This systematic review explores the pathophysiology and epidemiology of diabetes, which describes the interactions that implicate that is the result of the play between genetic, environmental, and lifestyle factors in the development and progression of the disease. The review also describes a plethora of pharmacological treatments available to manage diabetes, with the oral agent, insulin regimens, and new approaches to therapy. Integrating (...) the wisdom of the best clinical guidelines, those are from American diabetes association (ADA), European association for the study of diabetes (EASD), and International Diabetes Federation (IDF). Since these reviews are consistent with normative standards for evidence-based recommendations of optimal care of diabetes. The review goes further to cover technological advances in the management of diabetes. They include Continuous Glucose Monitoring, advanced insulin delivery systems, and Artificial Pancreas Systems. Such technologies hold more promising routes to better glucose control and patient quality of life. All this comes along with challenges that their deployment and maintenance that run through economic, adherence issues, and unequal access to healthcare. The future research and innovation in the battle against DM will include the application of personalized medicine and the development of advanced digital health solutions. The two significant zoonotic bacterial infections that have remained a concern due to the complex dynamics involved in transmission and global prevalence are anthrax and brucellosis. The present paper attempts to address some of the most important zoonotic pathogens, highlighting their epidemiology, clinical manifestations, diagnosis, treatment, and prevention strategies. Anthrax is largely transmitted through direct contact with the infected animals or their products resulting in cutaneous, inhalational, and gastrointestinal forms, all with specific clinical outcomes and approaches for treatment. Similarly, the (...) genus Brucella primarily transmits brucellosis to humans by occupational exposure and consumption of contaminated animal products. Its presentation may be acute, chronic, or in relapse. Diagnosis is made by a combination of serological, molecular, and culture-based techniques, while the treatment focuses on antimicrobial regimens specific to the pathogen. However, this increases the problem of antimicrobial resistance, which makes their effective management challenging, necessitating an integrated approach. Mitigating risks through biosecurity measures, vaccination, public health education, and a One Health approach that includes human, animal, and environmental health, all facilitate prevention and control of an outbreak. Advanced surveillance and diagnostic technologies further support the prompt interventions to contain outbreaks. The rapid development of digital technologies has significantly impacted governance structures globally, with e-governance emerging as a crucial tool for enhancing governmental efficiency, transparency, and public participation. This study explores the role of e-governance in shaping political ideologies within online media in Indonesia, focusing on the influence of social media platforms on political engagement and public discourse. Despite Indonesia's advancements in e-governance, as evidenced by its rising E-Government Development Index (EGDI) and E-Participation Index scores, challenges such as disinformation, digital divides, (...) and the polarization of online content persist. This paper used a literature review methodology to analyze the intersection of e-governance and online media, examining both the opportunities and obstacles faced by Indonesia in providing social media for political engagement. The findings highlight the dual nature of social media, which can both facilitate political participation and exacerbate political distrust. The study concludes that while e-governance in Indonesia has made significant strides, addressing the challenges of digital literacy, content regulation, and inclusive access is essential for fully realizing the potential of digital platforms to enhance political participation and shape a more democratic society. In this modern world, a massive amount of data is processed and broadcasted daily. This includes the use of high energy, massive use of memory space, and increased power use. In a few applications, for example, image processing, signal processing, and possession of data signals, etc., the signals included can be viewed as light in a few spaces. The compressive sensing theory could be an appropriate contender to manage these limitations. “Compressive Sensing theory” preserves extremely helpful while signals are sparse (...) or compressible. It very well may be utilized to recoup light or compressive signals with less estimation than customary strategies. Two issues must be addressed by CS: plan of the estimation framework and advancement of a proficient sparse recovery calculation. The essential intention of this work expects to audit a few ideas and utilizations of compressive sensing and to give an overview of the most significant sparse recovery calculations from every class. Individuals’ beliefs about the malleability of emotions have been theorised to play a role in their psychological distress by influencing emotion regulation processes, such as the use of emotion regulation strategies. We conducted a meta-analysis to test this idea across studies with a focus on the relationships between emotion malleability beliefs and five distinct emotion regulation strategies: cognitive reappraisal, suppression, avoidance, rumination, and acceptance. Further, using two-stage meta-analytic structural equation modelling (TSSEM), we examined whether the emotion regulation strategies mediate the (...) cross-sectional relationship between emotion malleability beliefs and psychological distress across studies. Thirty-seven studies were included in the meta-analyses and 55 cross-sectional studies were included in the TSSEM. Results demonstrated that, across studies, emotion malleability beliefs were significantly associated with greater use of putatively helpful strategies (particularly with cognitive reappraisal) and less use of putatively unhelpful strategies (particularly with avoidance). The use of cognitive reappraisal and avoidance partially mediated the relationship between emotion malleability beliefs and psychological distress. These results highlight the importance of considering beliefs about the malleability of emotions in the context of emotion regulation. These findings suggest the potential role of emotion malleability beliefs in interventions for individuals with emotion regulation-related difficulties and psychological distress. (shrink)

This paper presents a literature review on the ethics of self-tracking technologies which are utilized by users to monitor parameters related to their activity and bodily parameters. By examining a total of 65 works extracted through a systematic database search and backwards snowballing, the authors of this review discuss three categories of opportunities and ten categories of concerns currently associated with self-tracking. The former include empowerment and well-being, contribution to health goals, and solidarity. The latter are social harms, (...) privacy and surveillance, ownership control and commodification of data, autonomy, data-facilitated harm, datafication and interpretability of data, negative impact on relation to self and others, shortcomings of design, negative impact on health perception, and regulation and enforcement of rules. Childhood obesity is a serious problem for increasing numbers of children around the world. According to the International Obesity Task Force, 1 of 10 schoolaged children worldwide is overweight or obese, a number totaling 155 million; of these, 2-3% are obese. Prevalence is highest in the Americas and Europe, followed by the Near/Middle East, with smaller but growing numbers in the Asia-Pacific and Sub-Saharan regions of the world.In the United States, which provides the data for much of this report, prevalence (...) continues to rise. The most recent nationally representative data indicate that 33.6% of individuals aged 2-19 years were overweight and 17.1% were at risk for overweight in 2003-2004, compared to 28.2% and 13.9% in 1999-2000, respectively. In this age group, the prevalence of overweight or at risk for overweight was highest among Mexican Americans, followed by Non-Hispanic blacks and Non-Hispanic whites, and a higher percentage of males were overweight compared to females. There is now compelling evidence that motor imagery (MI) promotes motor learning. While MI has been shown to influence the early stages of the learning process, recent data revealed that sleep also contributes to the consolidation of the memory trace. How such “online” and “offline” processes take place and how they interact to impact the neural underpinnings of movements has received little attention. The aim of the present review is twofold: i) providing an overview of recent applied and fundamental (...) studies investigating the effects of MI practice on motor learning, and ii) detangling applied and fundamental findings in support of a sleep contribution to motor consolidation after MI practice. We conclude with an integrative approach of online and offline learning resulting from intense MI practice in healthy participants, and underline research avenues in the motor learning/clinical domains. The purpose of this research is to present the major factors that lead to ethical dissolution in an organization. Specifically, drawing from a wide spectrum of sources, this study explores the impact of organizational, individual, and contextual factors that converge to contribute to ethical dissolution. Acknowledging that ethical decisions are, in the final analysis, made by individuals, this study presents a model of ethical dissolution that gives insight into how a variety of elements coalesce to draw individuals into decisions that (...) result in the ethical undoing of an otherwise healthy organization. ENRON, TYCO and WorldCom did not happen in a vacuum. Nor can such debacles be explained as simply one or two individuals who were morally corrupt. The ethical breakdowns that occurred in these companies happened over a period of time, involved numerous individuals both inside and outside of the organization, and brought about the implosion of viable companies. Seeking to extend the work of previous researchers, this study attempts to tie together a disparate set of factors into a cohesive explanation of ethical breakdowns in organizations. (shrink)

IntroductionDeep brain stimulation (DBS) is increasingly used to treat the symptoms of various neurologic and psychiatric conditions. People can undergo the procedure during reproductive years but the safety of DBS in pregnancy remains relatively unknown given the paucity of published cases. We thus conducted a review of the literature to determine the state of current knowledge about DBS in pregnancy and to determine how eligibility criteria are approached in clinical trials with respect to pregnancy and the potential for pregnancy.MethodsA (...) literature review was conducted in EMBASE to identify articles involving DBS and pregnancy. Two reviewers independently analyzed the articles to confirm inclusion. Data extracted for analysis included conditions treated, complications at all stages of pregnancy, neonatal/pediatric outcomes, and DBS target. A second search was then conducted using www.clinicaltrials.gov. The same two reviewers then assessed whether each trial excluded pregnant individuals, lactating individuals, or persons of childbearing age planning to conceive. Also assessed was whether contraception had to be deemed adequate prior to enrollment.ResultsThe literature search returned 681 articles. Following independent analysis and agreement of two reviewers, 8 pregnancy related DBS articles were included for analysis. These articles described 27 subjects, 29 pregnancies (2 with subsequent pregnancies), and 31 infants (2 twin pregnancies). There was 1 preterm birth at 35 weeks, and 3 patients who experienced discomfort from the DBS battery (i.e., impulse generator) placement site. All 27 patients had a DBS device implanted before they became pregnant, which remained in use throughout their pregnancy. There was exclusion of pregnant individuals from 68% of 135 interventional trials involving DBS. Approximately 44% of these trials excluded persons of childbearing age not on “adequate contraception” or wishing to conceive in the coming years. Finally, 22% excluded breastfeeding persons.ConclusionThe data from 29 pregnancies receiving DBS treatment during pregnancy was not associated with unexpected pregnancy or post-partum complication patterns. Many clinical trials have excluded pregnant individuals. Documentation of outcomes in larger numbers of pregnancies will help clarify the safety profile and will help guide study designs that will safely include pregnant patients. (shrink)

The different kinds of cheating behaviors and the factors associated with them are delineated and described. Suggestions are provided on how to take corrective and proactive decisions to control and thereby reduce academic dishonesty and cheating.

One of the most important questions that educators try to answer is how to prepare new generations of students for an unpredictable future. Students need to learn several skills, such as creativity, critical thinking, collaboration, and communication. Creativity, especially, is an essential skill in a complex and unforeseeable world/era, and an important step in any effort to enhance creativity is to identify students’ creative strengths and relative weaknesses. This review aims to offer school psychologists and other educators such as (...) teachers, policymakers, and curriculum designers a comprehensive and practical guide to one of the most well-known creativity assessments—the Torrance Tests of Creative Thinking that was developed by E. Paul Torrance in the 1960s. The paper discusses the history, components, training, psychometric properties, and uses of the TTCT. Contrary to the notion that the TTCT is only a measure of divergent thinking skills, the current article presents its other uses. It is the authors’ hope that teachers, school psychologists, and other educators will find the information reported in this article useful to better understand the TTCT and use it most effectively. The autonomic nervous system is the other division of the human nervous system, which is in charge of most of the automatic processes in the body including heartbeat, digestion, breathing, dilation of the pupils, constriction of pupils, ejaculation, and micturition. This work focuses on the relations and mediators implied in these functions and on the principal regulatory models. Both sympathetic and parasympathetic systems are essential in creating flexibility, and whenever a shift occurs, many health issues may develop as a result. (...) For instance, an ANS activity level too high will cause hypertension or heart issues, while an ANS activity level too low will cause anxiety or panic disorders. The review also discusses the impact of ANS and other new advancements in genetics and genetic makeup regarding stress responsiveness. Even though some people’s genetic predisposes them to certain ANS disorders they can be faced with it due to their lifestyle and stress. Nevertheless, concise information is still scarce about ANS, underlining the importance of further research to evaluate pathogenic genes, and constituent the genetic treatments to enhance the disease's negative outlook. Measurements play a significant role in the scientific investigation of religious phenomena. The study of religion, in general, has attracted considerable attention from academia and literature. The aim of this study was to conduct a systematic analysis of the literature regarding scales that have been developed for measuring variables with Islamic relevance and importance, having a theoretical framework embedded in the primary sources of Islamic literature (the Quran and hadith) and validated on Muslim samples. Six well-known databases were selected—ScienceDirect, Web (...) of Science, Springer, Taylor & Francis, Emerald Insight, and Google Scholar—to conduct an in-depth search of existing literature on the topic. Results revealed three broad concepts that have been the focus of measurement: religiosity, spirituality, and morality. Based on our analysis, it can be suggested that although there has been an increase in measuring Islamic religiousness in recent years, there is scope for improvement. This analytical review can be a valuable resource for researchers interested in selecting a valid and trustworthy scale that meets the criteria of their study settings. (shrink)

A challenge of performing research in the paediatric emergency and acute care setting is obtaining valid prospective informed consent from parents. The ethical issues are complex, and it is important to consider the perspective of participants, health care workers and researchers on research without prospective informed consent while planning this type of research. We performed a systematic review according to PRISMA guidelines, of empirical evidence relating to the process, experiences and acceptability of alternatives to prospective informed consent, in the (...) paediatric emergency or acute care setting. Major medical databases and grey sources were searched and results were screened and assessed against eligibility criteria by 2 authors, and full text articles of relevant studies obtained. Data were extracted onto data collection forms and imported into data management software for analysis. Thirteen studies were included in the review consisting of nine full text articles and four abstracts. Given the heterogeneity of the methods, results could not be quantitatively combined for meta-analysis, and qualitative results are presented in narrative form, according to themes identified from the data. Major themes include capacity of parents to provide informed consent, feasibility of informed consent, support for alternatives to informed consent, process issues, modified consent process, child death, and community consultation. Our review demonstrated that children, their families, and health care staff recognise the requirement for research without prior consent, and are generally supportive of enrolling children in such research with the provisions of limiting risk, and informing parents as soon as possible. Australian data and perspectives of children are lacking and represent important knowledge gaps. (shrink)

Corruption prevention can be more effective if it does not rely merely on legal enforcement. This theoretical review aimed to propose a hypothetical psychological model capable of explaining the behavior of corruption. Moral disengagement is a variable that is considered ontologically closest in “distance” to the variable of corruption behavior. Counterfeit self, implicit self-theory, ethical mindset and moral emotion are taken into account as the pivotal factors of the corruption behavior and its mechanism of moral disengagement. Counterfeit self along (...) with some moderating variables are regarded to “set” one’s future corrupt behavior based on his/her past/prior ethical or unethical behavior and moral emotions. This review discovered a conjectural-theoretical model of the corruption psychology. It can be used to design a social intervention and training for individuals to manage the mindset and emotion that can buffer counterfeit self effect. In addition, the users of these research findings are recommended to be aware of the surroundings that consist of groups of people with particular ethical mindset, moral emotion proneness and self-theory. The past decades have seen an increase in the design and implementation of sexuality education programs deemed comprehensive within formal schooling. Qualitative research is gaining relevance in the field; however, few studies focus on youth perspectives. This study analyzes qualitative research through a literature review on the implementation of Comprehensive Sexuality Education (CSE) programs from the perspective of young people. A review of qualitative studies and a meta-synthesis were conducted to outline a state-of-the-art of qualitative research (...) on youth perceptions of CSE. Results point to two main areas of concern for youth: contents and pedagogy. There remains a general dissatisfaction over the depth and breadth of the CSE curriculum, leaving young people feeling unprepared for positive relationships and good sexual health. In terms of pedagogy, there is still a shortfall in the effective implementation of innovative teaching and learning strategies, pointing towards a gap regarding the pedagogical frameworks underlying CSE. The purpose of this study is to discuss the effects of audiovisual input on second language acquisition and the factors that influence the difficulty of audiovisual learning through a systematic literature review. Prior to this systematic review, in this paper, we searched papers on related topics for the past 10 years from 2012 to 2022, and found 46 journal papers that met the research criteria. They can basically represent the scholarly work in this field. The 46 studies were (...) published in journals indexed in Google Scholar, Eric, Scopus, and Wiley Library. Databases were selected according to a set of inclusion and exclusion criteria. The following conclusions are drawn from the literature review: Audiovisual input can provide more authentic language input and more adequate and richer multimodal cultural and situational contexts, which can better promote learners' understanding of the content and stimulate learners' interest in participating in listening comprehension tasks. The influencing factors of multimodal input on listening difficulty include subtitles, video type, and the relationship between the audio and visual input. (shrink)

The advent of generative artificial intelligence and the widespread adoption of it in society engendered intensive debates about its ethical implications and risks. These risks often differ from those associated with traditional discriminative machine learning. To synthesize the recent discourse and map its normative concepts, we conducted a scoping review on the ethics of generative artificial intelligence, including especially large language models and text-to-image models. Our analysis provides a taxonomy of 378 normative issues in 19 topic areas and ranks (...) them according to their prevalence in the literature. The study offers a comprehensive overview for scholars, practitioners, or policymakers, condensing the ethical debates surrounding fairness, safety, harmful content, hallucinations, privacy, interaction risks, security, alignment, societal impacts, and others. We discuss the results, evaluate imbalances in the literature, and explore unsubstantiated risk scenarios. (shrink)

In my book The Comprehensibility of the Universe (OUP, 1998), I argue for a new conception of science that construes science as adopting a hierarchy of increasingly contentless cosmological assumptions about the comprehensibility and knowability of the universe. This view, I argue, solves outstanding problems about science, such as problems of induction, simplicity and verisimilitude. A needs assessment survey of ethics review committees (ERCs) across Africa was conducted in order to establish their major needs and areas of weaknesses in terms of ethical review capacity. The response rate was 84% (31 of 37 targeted committees), and committees surveyed were located in 18 African countries. The majority of the responding committees (61%) have been in existence between 5 and 10 years; approximately 74% of the respondents were institutional committees, with the remainder being either national (...) (6/31) or regional (2/31). In terms of the ethical review process, nine of the 31 committees that responded did not have standard operating procedures (SOPs), and seven of the 22 that did have SOPs had never revised them after their initial development (an average period of three years). Of the 31 committees, 10 operated without any ethical guidelines. Many of the committees (13/30) met once per month, and the number of proposals reviewed annually varied, ranging from five to over 100. All respondents relied on paper-based data management and archiving systems. Overall, the survey identified the major constraints on ERCs as lack of office equipment, outdated or lack of SOPs, lack of electronic data management systems, inadequate resources, lack of or insufficient expertise on the committees, and poor recognition of the importance of the role of the committees. Consequently, the authors are addressing the identified needs and weaknesses through the Bill and Melinda Gates Foundation-funded capacity building project. This systematic review identifies and critically evaluates instruments that have been developed to measure clinical trial informed consent comprehension in non-cognitively-impaired adults. Literature searches were carried out on Medline (Ovid), PsycInfo, CINHAL, ERIC, ScienceDirect, and Cochrane Library for English language articles published between January 1980 and September 2008. Instruments were excluded if they focused on consent onto paediatric trials, the construct under study was primarily capacity or competency, or the instrument was developed specifically for psychiatric or cognitively-impaired populations. Instruments (...) selected for review were evaluated against the following criteria: (1) method of item generation; (2) type and format of test items; (3) administration and interpretation of test results; and (4) psychometric properties. Three instruments met our defined inclusion criteria: the Deaconess Informed Consent Comprehension Test (DICCT), the Quality of Informed Consent (QuIC) questionnaire and the Brief Informed Consent Protocol (BICEP). Each instrument varied in terms of content measured. Significantly, these are the first standardized instruments developed to assess comprehension in non-cognitively-impaired adults. Yet, each instrument had its own set of limitations such as the lack of generalizability and the absence of details pertaining to how test results should be used to guide clinical decision-making. Standardized clinical trial informed consent comprehension assessments have been developed to identify gaps in research participants' understanding and ensure that respect for patient autonomy is satisfied. Adolescents are increasingly at risk for sexually transmitted diseases (STDs), including human immunodeficiency virus (HIV) infection. The prolonged latency period, sometimes in excess of five years, and the incubation period of up to 10 years before the manifestation of symptoms, may foster adolescents’ false sense of invincibility and denial as they often do not see the devastating effects of the disease in their peers until they are older. In turn, their practice of safer sex may be hindered and thereby contribute (...) to the escalation of this public health crisis among sexually active adolescents. Prevention-focused recommendations were made in the USA as a result of this crisis. Recommendations were made to: (1) include STD/HIV education in the curricula of grades kindergarten to 12; (2) increase to at least 75% the proportion of primary care and mental health professionals who provide age-appropriate STD/HIV prevention counselling to adolescents; and (3) expand HIV prevention services to include age-appropriate HIV education curricula for students in grades 4-12 in 95% of schools. Yet, in the USA, the provision of school-based comprehensive STD/HIV education has been difficult to achieve owing to certain limitations and, in some instances, legal action. These limitations include: limited student access; restricted content; and the implementation of sporadic and/or brief educational programmes. Given these recommendations and the fact that adolescents are acquiring STDs and HIV infections at increasing rates, and despite the limitations and legal actions, do health care professionals not have an ethical obligation to provide adolescents with comprehensive STD/HIV prevention education? This ethical dilemma will be discussed using the ethical decision-making principles of ‘autonomy’ and ‘beneficence’, and a decision-making model proposed by Thompson and Thompson, and by Chally and Loric. Paperback, $34.95.This is the last book Rudolf Makkreel published before passing away in October 2021. No wonder, then, that it makes some strong points, one of which is truly fundamental: the time has come to recognize the (...) expediency of reading Kant along with Dilthey for a better understanding of the workings of human comprehension. Makkreel maintains that Kant endorsed “a more inclusive idea of philosophizing according to a world-concept (Weltbegriff ),” for Kant had realized that, because reason cannot simply extrapolate from the ways in which the understanding proceeds to a determinate lawful order, it is necessary to tame the speculative projections of reason by our own powers of orientation and judgment. Hence, Makreel concludes that “the overall worldview we can actually arrive at will need [End Page 511] to include supplementary modes of organizational order that are not determinant but reflective” (3).Throughout the book, Makkreel is pleading for Kant scholars to acknowledge the heuristic value of a set of concepts whose philosophical effectiveness has been to date underestimated. Readers will find many convincing analyses of the lexical cloud around judgment: determining judgment (bestimmende Urteilskraft) against reflecting judgment (reflektierende Urteilskraft) in its connection with the Weberian distinction between explaining (Erklären), understanding (Verstehen), and comprehending (Begreifen). What is at stake in contrasting the last two, understanding and comprehension, insists Makkreel, is the scope of what humans can claim to know, for while it is important to base knowledge claims on the empirical understanding, we must remain open to what could lie beyond the horizon of sense. Hence, critical philosophy is there to supplement “the limits of actual geographical” surveys of the earth with the “boundaries of all possible descriptions of the earth” (KrV A 759/B 787), whereby limits (Schranken) are negative empirical markers that do not take into account what could be beyond them, while boundaries (Grenzen) are positive markers that “presuppose a space existing outside a certain definite space and enclosing it” (Prolegomena, AA 4:352). Makkreel suggests we can infer that whereas earthly limits are imposed from without, worldly boundaries are in some sense self-imposed. Earthly limits would act as external negative restraints on the understanding, while worldly bounds work as internal positive constraints on reason as it attempts to comprehend both what we can and cannot know. Comprehension, argues Makkreel, can be “shown to be a mode of reason whose bounds must be judgmentally self-binding” (14).Let me take a step back. Makkreel intends to show that the task of understanding in its relation with comprehension is what we need to provide an overall characterization of Kant’s philosophical project. This is a bold stance, given Kant’s argument that the faculty of understanding (Verstand) proceeds discursively, namely one step at a time, which makes the project of a more holistic comprehension (Begreifen) highly problematic. However, Makkreel notes that if it is true that “concepts of reason serve for comprehension” (KrV A 311/B 367), it is also true they are purely inferential and at times sophistical (13). To help define what Kant ends up meaning by ‘comprehension,’ Makkreel provides exhaustive references to important discussions about this notion in Kant’s Lectures on Logic and the Critique of Judgment (40–46).Here comes to the foreground the notion of worldview (Weltanschauung) that stands alone in the title of his book. It is in the context of his discussion of the mathematical sublime that Kant broached the idea of an overall “worldview [Weltanschauung]” (KU AA 5:255), which nineteenth- and twentieth-century philosophers (e.g. Hegel, Dilthey, and Jaspers) took to denote a common or generic outlook on life. BackgroundLiterature on issues relating to comprehension during the process of obtaining informed consent (IC) has largely focused on the challenges potential participants can face in understanding the IC documents, and the strategies used to enhance comprehension of those documents. In this review, we set out to describe the factors that have an impact on comprehension and the strategies used to enhance the IC process in sub-Saharan African countries.MethodsFrom November 2021 to January 2022, we conducted a literature search using a (...) PRISMA tool. We searched electronic databases (PubMed, EMBASE, EBSCOHOST) to identify relevant peer reviewed studies. We then reviewed the references of these articles to find additional literature that might have been missed through the initial search. We were particularly interested in full text articles in English that focused on the IC process in SSA published between 2006 and 2020. We included systematic reviews, and studies from Western and Asian countries that included data about SSA. We excluded articles that focused on medical interventions and studies that did not require IC.ResultsOut of the 50 studies included most were multi-country (n = 13) followed by single country studies in South Africa (n = 12); Kenya, Tanzania, Uganda (n = 5) each; Gambia, Ghana and Nigeria (n = 2)each ; and one each for Botswana, Malawi, Mali, Mozambique. We identified three areas of focus: (1) socio-cultural factors affecting IC; (2) gaps in the ethical and legal frameworks guiding the IC process; and (3) strategies used to improve participants’ understanding of IC.ConclusionOur review showed wide recognition that the process of achieving IC in SSA is inherently challenging, and there are limitations in the strategies aimed at improving comprehension in IC. We suggest that there is a need for greater flexibility and negotiation with communities to ensure that the approach to IC is suited to the diverse socio-cultural contexts. We propose moving beyond the literal translations and technical language to understanding IC comprehension from the participants’ perspectives and the researchers’ views, while examining contextual factors that impact the IC process. Scientific peer reviewers play an integral role in the grant selection process, yet very little has been reported on the levels of participation or the motivations of scientists to take part in peer review. The American Institute of Biological Sciences developed a comprehensive peer review survey that examined the motivations and levels of participation of grant reviewers. The survey was disseminated to 13,091 scientists in AIBS’s proprietary database. Of the 874 respondents, 76% indicated they had reviewed grant (...) applications in the last 3 years; however, the number of reviews was unevenly distributed across this sample. Higher review loads were associated with respondents who had submitted more grant proposals over this time period, some of whom were likely to be study section members for large funding agencies. The most prevalent reason to participate in a review was to give back to the scientific community and the most common reason to decline an invitation to review was lack of time. Interestingly, few suggested that expectation from the funding agency was a motivation to review. Most felt that review participation positively influenced their careers through improving grantsmanship and exposure to new scientific ideas. Of those who reviewed, respondents reported dedicating 2–5% of their total annual work time to grant review and, based on their self-reported maximum review loads, it is estimated they are participating at 56–87% of their capacity, which may have important implications regarding the sustainability of the system. Overall, it is clear that participation in peer review is uneven and in some cases near capacity, and more needs to be done to create new motivations and incentives to increase the future pool of reviewers. Introduction Future HIV vaccine efficacy trials with adolescents will need to ensure that participants comprehend study concepts in order to confer true informed assent. A Hepatitis B vaccine trial with adolescents offers valuable opportunity to test youth understanding of vaccine trial requirements in general. Methods Youth reviewed a simplified assent form with study investigators and then completed a comprehension questionnaire. Once enrolled, all youth were tested for HIV and confirmed to be HIV-negative. Results 123 youth completed the questionnaire (mean age=15 (...) years; 63% male; 70% Hispanic). Overall, only 69 (56%) youth answered all six questions correctly. Conclusions Youth enrolled in a Hepatitis B vaccine trial demonstrated variable comprehension of the study design and various methodological concepts, such as treatment group masking. (shrink)
