BackgroundThe Supreme Court of Canada removed the prohibition on physicians assisting in patients dying on 6 February 2015. Bill C-14, legalising medical assistance in dying (MAID) in Canada, was subsequently passed by the House of Commons and the Senate on 17 June 2016. As this remains a divisive issue for physicians, the Canadian Neurosurgical Society (CNSS) has recently published a position statement on MAID.MethodsWe conducted a cross-sectional survey to understand the views and perceptions among CNSS members regarding MAID to inform (...) its position statement on the issue. Data was collected from May to June 2016.ResultsOf the 300 active membes of the CNSS who recevied the survey, 89 respondents completed the survey, 71% of whom were attending neurosurgeons and 29% were neurosurgery residents. Most respondents,74.2%, supported the right of physicians to participate in MAID with 7.8% opposing. 37% had current patients in their practice fitting the criteria for MAID. 23.6% had been asked by patients to assist with MAID, but only 11% would consider personally providing it. 84% of neurosurgeons surveyed supported the physicians’ right to conscientious objection to MAID while 21% thought attending surgeons should be removed from the inquiry and decision-making process. 43.8% agreed that the requirment to refer a patient to a MAID service should be mandatory. Glioblastoma multiforme (65%), quadriplegia/quadriparesis secondary to spinal tumour/trauma (54%) and Parkinson’s disease (24%) were the most common suggested potential indications for MAID among the neurosurgical population.ConclusionsOur results demonstrate that most neurosurgeons in Canada are generally supportive of MAID in select patients. However, they also strongly support the physicians’ right to conscientious objection. (shrink)

It is widely held that episodic memory is constitutively connected with space and time. In particular, many contend that episodic memory constitutively has spatial and/or temporal content: for instance, necessarily representing a spatial scene, or when a given event occurred, or at the very minimum that it occurred in the past. Here, I critically assess such claims. I begin with some preparatory remarks on the nature of episodic memory. I then ask: How, if at all, is episodic memory constitutively spatial? (...) And, how, if at all, is episodic memory constitutively temporal? In answer, I argue that episodic memory need not have any spatial content, nor (in any substantial sense) need it represent when its events occur, nor even that they occur in the past. Instead, only a relatively modest connection holds between episodic memory and time in virtue of the temporal structure of its objects. Finally, I critically assess whether considerations concerning the organization and encoding of episodic memory in creatures like us provide stronger reason to posit a constitutive link between our episodic memories and space or time. Examining the role of dispositions (potentials and propensities) in both physics and psychology reveals that they are commonly derivative dispositions, so called because they derive from other dispositions. Furthermore, when they act, they produce further propensities. Together, therefore, they appear to form discrete degrees within a structure of multiple generative levels. It is then constructively hypothesized that minds and physical nature are themselves discrete degrees within some more universal structure. This gives rise to an effective dualism of mind and nature, (...) but one according to which they are still constantly related by causal connections. Keith Reader's brief, unfinished article ‘Freeze Peach’ situates contemporary controversies surrounding free speech in relation to material and economic concerns. Ian James's response draws attention to the way Keith does this by bringing together four key figures of late twentieth-century philosophy and theory: Louis Althusser, Jean-François Lyotard, Terry Eagleton and Stanley Fish. Ian argues that the conjugation of Marx-inspired theory with thinkers associated with the postmodern would have allowed Keith to develop a uniquely perceptive and productive insight into the present (...) moment of political disorientation, one which demands a renegotiation of the legacies of both modernity and postmodernity. The teaching of critical thinking in public schooling is a central aim. Yet, despite its widespread acceptance in curriculum documents, critical thinking is rarely taught. Motivated by Onosko (1991), and by the efforts of some post-secondary instructors of critical thinking to get critical thinking taught in schools, I look at the recent literature on (a) critical thinking in the social studies, (b) definitions of, and programs in critical thinking, (c) teachers beliefs, and (d) the milieus in which teachers work. This article explores the concept of corporate identity from a moral perspective. In it we argue that the reification and personification involved in attributing an identity to an organization has moral repercussions. Through a discussion of 'intentionality' we suggest that it is philosophically problematic to treat an abstraction of the corporation as possessing identity or acting as a conscious moral agent. The article moves to consider practical and ethical issues in the areas of organizational commitment, of health and safety, and (...) corporate social responsibility, and finds that the notion of identity can be abused, although it will no doubt continue to be used as it does have some practical utility. In conclusion, we argue that despite being meaningless from a philosophical stance, the concept of corporate identity need not be discarded, however, it is far from benign and intense moral scrutiny is necessary wherever it is applied. An exploration of the possible place and purpose of a postmodernizing planning, in the pursuit of ecological wisdom - defined, in Ken Wilber's terms, as how to get people to agree on how to live in accord with nature. Placemaking - conceived as a form of applied Integral Ecology - is hypothesized as an appropriate planning response, driven by a more explicit "spirit-friendly" outlook, with an associated critique of contemporary conventional notions of growth and sustainability. In lieu of an abstract, here is a brief excerpt of the content:Foucault’s Renaissance Episteme Reassessed: An Aristotelian CounterblastIan MacleanThere seem to me to be two good reasons for looking at Foucault’s Renaissance episteme again, even though specialists of the Renaissance have given it short shrift and Foucault himself does not seem to have set great store by it in his later writings. 1 The first is that in general books on Foucault accounts of it are still given in a (...) neutral or even approving way, as though neither the concept of episteme (which suggests both that mind-sets or paradigms or epistemes are all-constraining) nor the account of the Renaissance episteme in particular (which is given a predominantly Platonic and hermetic character) require modification, if not refutation. 2 I shall suggest that although in some sense we are all prisoners of our mind-sets, there are ways in which we can escape from this prison 3 ; and with respect to the Renaissance in particular, I shall claim that Aristotelianism, not Platonism, is the appropriate context in which to see the dominant universe of discourse of the day and that this philosophy can be shown to possess the resources for escaping its limitations. To do this I shall [End Page 149] give a brief resume of Foucault’s episteme in general and the Renaissance episteme in particular. Then I shall show first, that the Platonist features which he cites as typical are actively refuted at that time and cannot be described as dominant without severe qualification; second, that the versions of Aris-totelianism widely taught in universities and schools give quite different accounts of these features; and third, that this form of thinking can be shown to have the freedom of self-critique in the very areas in which Foucault chooses to see Renaissance philosophy as both constraining and constrained.In 1969 Foucault produced his “theoretical work” L’Archéologie du savoir as the methodological systematization of his earlier historical writings, L’Histoire de la folie à l’âge classique (l961), La Naissance de la clinique (l963), and Les Mots et les choses (1966). In this work man is described as a modern self-invention, fashioned from a particular configuration of knowledge and destined soon to disappear. Foucault’s history of man is a story about the way he constituted himself as a subject and then made his body, his behavior, his reason, and his consciousness the object of his study and a field to be controlled, regulated, trained, and coerced. In this analysis man’s “rationality” becomes not a faculty but an authoritarian straitjacket. What Foucault sets out to investigate is a very fundamental form of ontology but not one associated with profound knowledge; it is a superficial and finite, if largely invisible, field, which is discontinuous, not defined by the agenda of the modern state of any associated discipline, and peopled by “utterances” (énoncés) which are only graspable in their differences and their dispersion. If coagulations of these “utterances” (formations discursives) occur at all, they appear only in the form of consistent relations: the objects to which they “refer” are no more than objectifications whose scientific status cannot be established and whose meaning cannot be recovered by semantic or rhetorical analysis. In calling his previous book Les Mots et les choses, he was being explicitly ironic, for neither the words nor the things of the title have any substance to them: they are not presences but absences. The description made by the historian is remote from his object, indirect, incomplete; it describes a “preconceptual field” in which the “preconcepts” are no more than organizing operators or functions revealing relationships of dependence, processes of validation, contingent theories of truth. The “utterances” which these “preconcepts” inform come together in “discourses” which are neither systems nor mere aggregations of knowledge; archaeological history as practiced by Foucault uncovers not a “mentality” but an “episteme,” which he defines as follows:By episteme we mean... the total set of relations that unite, at a given period, the discursive practices that give rise to epistemological figures, sciences, possibly formalized systems.... It appears that the attitudes of health professionals differ towards suicide and voluntary active euthanasia. An acceptance of, if not an agreement with, voluntary active eutha nasia exists, while there is a general consensus that suicide should be prevented. This paper searches for a working definition of suicide, to discover ethical reasons for the negative value that suicide assumes, and also to provide a term of reference when comparing suicide with euthanasia. On arriving at a working definition of suicide, it (...) is compared with voluntary active euthanasia. An analysis of utilitarian and deontological considerations is provided and proves to be inconclusive with respect to the ethical principles informing the attitudes of professionals. Therefore, a search for other influences is attempted; this indicates that psychological influences inform attitudes to a greater degree than ethical principles. (shrink)

A growing interest in affect holds much promise for anthropology by providing a new frame to examine and articulate subjective and intersubjective states, which are key parts of human consciousness and behavior. Affect has its roots in the social, an observation that did not go unnoticed by Durkheim and since then has been kept in view by those social scientists interested in the emotions, feelings, and subjectivity. However, the challenge for ethnographers has always been to articulate in words and conceptualize (...) theoretically what is only felt and sensed. What we are calling “evocative ethnography” is an ethnography that meets this challenge to make room for, and hold onto, feelings and affect in its description and explanation. Peer review of manuscripts has recently become a subject of academic research and ethical debate. Critics of the review process argue that it is a means by which powerful members of the scientific community maintain their power, and achieve their personal and communal aspirations, often at others' expense. This qualitative study aimed to generate a rich, empirically‐grounded understanding of the process of manuscript review, with a view to informing strategies to improve the review process. Open‐ended interviews were carried out with (...) 35 journal editors and peer reviewers in the United Kingdom, the USA and Australia. It is clear from this research that relations of power and epistemic authority in manuscript review are complex and dynamic, and may have positive and negative features, and that even where power is experienced as controlling, restrictive and illegitimate, it can also be resisted. In conclusion, the manuscript review process is best thought of not in terms of simple dominance of reviewers and editors over authors, but rather as a shifting “net” of power relations. These complex power relations need to be understood if reviewers are to be encouraged to participate in the process and to do so in the the most ethical and effective manner. (shrink)

In the discipline of practical theology, one finds a long history of linking the name of the field to diaconiology, in which you find the Greek word diaconia, directly translated as 'service'. For good and scientific reasons, the field changed its name to practical theology in some Faculties of Theology but that does not take away the fact that this field of research is still very much engaged in the broad area of 'service of all kinds'. The purpose of this (...) article was to do a descriptive investigation into the problems and challenges of service delivery in the suburb of Belhar (Cape Town), specifically looking at the role of leadership during a (new) time of struggle. As part of the normative task, the researcher took a closer look at the Belhar Confession as a prophetic stance in this regard. In an ironic twist, we see in these events the struggle between the real and the ideal that is typical of many places in the country facing problems with service delivery. The Church often thinks it knows what the world wants from it - inspiration, 'leadership,' clear teaching. But surely what the world is most hungry for is the simple knowledge that healing is possible - not by forgetting or minimizing outrages and sufferings but by sheer, obstinate, costly commitment to work with the grain of grace. This book takes Edmund Husserl's phenomenology and applies it to help psychotherapy practitioners formulate complex psychological problems. The reader will learn about Husserl's system of understanding and its concepts that point to first-person lived experience, and about the work of Husserl scholars who have developed a way to be precise about the experiences that clients have. Through exploring the connection between academic philosophy of consciousness and mental health, themes of biopsychosocial treatment planning, psychopathology of personality and psychological disorders, and the (...) treatment of complex psychological problems become clear. The author shows that Husserlian phenomenology can be used in the design of interventions for clients in a process called formulation. Once the intentionality of consciousness is understood, by asking simple questions, it becomes possible to define problematic experiences. This is a means of creating informed consent for treatment and it makes it clear to clients what is happening for them, so helping them understand themselves and how they see the world. We also see how Husserl's phenomenology is a vehicle for psychotherapists to present their knowledge about the research literature of what has been found to be effective care. This volume applies the concepts and practices of phenomenology in a concrete way, relating them to the practice of therapy and showing the value of a qualitative approach to understanding mental processes and the nature of human beings as motivated by values, meanings and other conscious experiences. This is a readable text in simple language that condenses key aspects of Husserl's thinking in relation to the theory and practice of psychotherapy, and it is suitable for philosophers and practitioners of psychology, psychiatry, and the psychotherapies, including psychoanalysis. Peter Mandler’s Return from the Natives examines Margaret Mead mid-career when she devoted much energy to promoting anthropology and anthropologists to government and industry and positioned herself as a prominent social commentator. By the time she returned to the field after an interlude of 14 years, something had happened to her professionally: she was treated as a bit of an embarrassment, no longer a scientific heavyweight, and much of this stems from the rather hare-brained “culture cracking” she engaged in during (...) the war. So while honors and kudos enhanced her official image, behind the scenes, she complained of being neglected and shunted aside. Her discipline became radicalized, and her balanced and accommodating approach was rejected. (shrink)

Carnap's early system of inductive logic make degrees of confirmation depend on the languages in which they are expressed. They are sensitive to which predicates are, in the language, taken as primitive. Hence they fail to be ‘linguistically invariant’. His later systems, in which prior probabilities are assigned to elements of a model rather than sentences of a language, are sensitive to which properties in the model are called primitive. Critics have often protested against these features of his work. This (...) paper shows how to make his systems independent of any choice of primitive predicates or primitive properties.The solution is related to another criticism of inductive logic. It has been noticed that Carnap's systems are too all-embracing. Hisc(h, e) is defined for all sentencesh ande. Modern scientific medicine is increasingly challenged by complementary and alternative therapies. Reviewing policy options for contemporary healthcare development, the World Health Organization's first global strategy on traditional and alternative medicine, released in May 2002, advocates integration. However, experience in East Asia, the only part of the world where state of the art modern scientific facilities are commonly found alongside thriving traditional practices, reveals that medical integration can take several forms. To clarify the available policy options, this article categorizes those forms, (...) identifying three types of integration (unification, equalization and subjugation), plus one type of non-integration (marginalization). It marks out a zone of balanced healthcare development that cuts across two of the integrationist types, and comprises non-discriminatory state treatment of separate but linked sectors of traditional and modern medicine. The article closes by exploring arguments for and against locating state policy in this zone, and holds that policy should be situated here for medical practices that can meet broadly acceptable professional standards, demonstrate an existing social demand, and generate an adequate supply of medical practitioners, possibly through some state subsidy.
