We do not know how much clinical physicians carrying out clinical trials in oncology and haematology struggle with ethical concerns. To our knowledge, no empirical research exists on these questions in a Nordic context. Therefore, this study aims to learn what kinds of ethical challenges physicians in Sweden, Denmark and Finland (n = 29) face when caring for patients in clinical trials; and what strategies, if any, they have developed to deal with them. The main findings were that clinical cancer (...) trials pose ethical challenges related to autonomy issues, unreasonable hope for benefits and the therapeutic misconception. Nevertheless, some physicians expressed that struggling with such challenges was not of great concern. This conveys a culture of hope where health care professionals and patients uphold hope and mutually support belief in clinical trials. This culture being implicit, physicians need opportunities to deliberately reflect over the characteristics that should constitute this culture. (shrink)

ZusammenfassungEinhergehend mit den Fortschritten in der Reanimationsmedizin beschäftigen sich seit den 80er Jahren viele Studien mit der prognostischen Güte von reanimations- und patientenbezogenen Akutmarkern. Generell sollen Prognosemarker die Vorhersagbarkeit des Heilungspotenzials erlauben, sie besitzen dadurch eine Steuerfunktion für das weitere Therapiekonzept. Die Wertigkeit dieser Prädiktoren verändert sich jedoch, wenn sie als Determinanten bei Non-Treatment-Entscheidungen herangezogen werden. Dies aus zwei Gründen: 1. Nach Sichtung der Literatur und eingedenk eigener Studienresultate fehlt bislang ein 100%- sicherer Prognose-Algorithmus für Patienten im posthypoxischen Koma. 2. (...) Der komatöse Patient kann an der Beurteilung der Prognoseaussagen selbst nicht teilnehmen. Es stellt sich somit die Frage: Dürfen Prognosemarker unser moralisches Handeln an der Grenze des Lebens leiten? Die Autoren bejahen diese Frage und sehen eine adäquat moralische Handlungsoption in der Durchführung einer diskurs-pragmatischen Patientenkonferenz. Den Prognosemarkern kommt hier eine Stellvertreterfunktion im Sinne eines „stillen Patientenargumentes“ zu, wodurch die potenzielle Dialogfähigkeit des Patienten als moralisches Anspruchsobjekt gewahrt wird. Im Rahmen einer diskursiven Erörterung wird dann der Einfluss der Prognosedaten an der Entscheidung gewichtet. (shrink)

Cheyne and Girard characterize felt presence during sleep paralysis attacks as a pre-hallucinatory expression of a threat-activated vigilance system. While their results may be consistent with this interpretation, they are nonetheless correlational and do not address a parsimonious alternative explanation. This alternative stipulates that FP is a purely spatial, hallucinatory form of a common cognitive phenomenon—social imagery—that is often, but not necessarily, linked with threat and fear and that may induce distress among susceptible individuals. The occurrence of both fearful and (...) non-fearful FPs in a multiplicity of situations other than sleep paralysis attacks supports the notion that FPs are hallucinatory variants of social imagery and that they are not necessarily bound to threat-activated vigilance. Evidence linking FPs with anxiety disorders supports the notion that the distress they evoke may be mediated by a more general affective distress personality factor. To illustrate the predominantly spatial character of FP hallucinations, similarities between FP and phantom limbs are summarized and the possibility that these two phenomena are parallel expressions of a mirror neuron system is considered. This festschrift includes a dozen essays on issues that have been at the focus of Kai Nielsen's research, mainly issues in ethics and political philosophy. Among these are four essays on socialism and Marxism. There are also essays on philosophy of religion, epistemology, and meta-philosophy.

Will globalization promote or hinder social justice throughout the world? In this cogent analysis philosopher Kai Nielsen argues that in its present form capitalist globalization will only ensure that the rich get richer and the poor poorer. Noting that the ratio of the richest countries to the poorest has steadily grown larger under capitalism in the 20th century and that the total dollar value of the world economy has increased fivefold while the number of people living in poverty has (...) doubled, Nielsen clearly demonstrates that globalization has made and still is making a bad situation worse. While inveighing against capitalist globalization, he makes the important point that a globalization based on "market socialism" - to ensure both needed efficiency and an egalitarian conception of justice - would be a trend that people in all nations would welcome. Democratic socialism, despite historical betrayals and recent setbacks, Nielsen contends, is still humanity's best hope for achieving a classless, nonracist, and nonsexist world community. He devotes a number of chapters to a discussion of the critical theory that is the basis of this vision of a completely egalitarian international society, and he compares and contrasts his own position with that of such thinkers as Richard Rorty, John Rawls, Juergen Habermas, G. A. Cohen, and others. This well-argued critique of capitalist globalization and defense of democratic socialism as a viable alternative is essential reading for philosophers, political scientists, students of international relations, and anyone concerned about the future of democratic and egalitarian ideals. This study explores young students’ negotiation of their citizenship identities at the intersection of their class, gender, religious and ethnic identifications in the conflict-affected setting of Pakistan. While much of the global literature on global citizenship education (GCE) primarily takes into account the perspectives of middle-class or elite students located in richer economies, the current study is centred on a socio-demographically diverse group of young people in a low-income setting. With a specific focus on their negotiation of issues around diversity (...) and justice, students’ narratives generated important recommendations for a transformative and historically nuanced postcolonial/decolonial approach to global citizenship engagement that should be considered more broadly. The study illuminates the ways the global/local historical, cultural, political and economic factors influence individual relationship with GCE and offers useful pedagogical and policy implications for GCE ‘from below’. The purpose of this study is to explore and illuminate the phenomenon of busyness as experienced by nurses. The daily work of nursing practice is often characterized by a hectic pace in the execution of tasks. Previous research shows that busyness can potentially lead to a reduction in the quality of nursing. Little has been explored about nurses' own experiences of busyness. This study has a qualitative design. The method chosen is a phenomenological hermeneutical exploration of personal experiences. Results reveal (...) that busyness is experienced as a disparity between perceived necessary tasks and time available to accomplish them. Busyness has an outer dimension of events and a dimension of internal processes. Busyness is experienced as acceptable to some extent, but feels strongly uncomfortable if important tasks remain undone. The intolerable busyness raises negative emotions, steals energy and weakens health. Coping with busyness seems to be a personal and individual struggle, even though health service enterprises are a collective matter. (shrink)

Looking at responsibility within a Lévinasian sense, human beings are firstly seen not in the philosophically traditional sense, of being egocentric, but rather seen as ethical subjects based on “the other” (Lévinas & Hand, 1989). The purpose of this paper is to examine the notion of responsibility as Lévinas conceptualized in the idea that human beings are responsible for not only themselves but for others. Lévinas within “Ethics as First Philosophy” (Lévinas & Hand, 1989) states that before all other forms (...) of being in the world there must be responsibility to the Other (Lévinas & Hand, 1989). Lévinas defines this to mean that before freedom of the subject, and their subjectivity, before self-consciousness, before the consciousness of consciousness, before being, and before relativity there is “responsibility for the Other pre-exists any self-consciousness” (Lévinas & Hand, 1989, p.75). Perhaps, Lévinas is saying that a human beings obligation comes before all of what follows from our being in this world, our obligation to the Other beginning with the “face to face” (Lévinas & Hand, 1989, p.75), interaction, which is our first and foremost responsibility, the responsibility to the Other. And to be ethical beings who begin with responsibility we rely on the fundamental relationship of others. This notion of responsibility appears when we encounter, in the face-to-face interaction, the unknowable other. However, does this responsibility extend to more than two individuals and to those outside our immediate proximity? For example, Lévinas states that it is the “face to face” notion of seeing one in distress, yet what if it is a group of people, and what if they are not in close proximity but on the other side of the world? What if they are on the other side of a screen that can also be artificial intelligence in our modern era. Are we still responsible? And if we are responsible, how does Lévinas’s concept of responsibility fit into that situation or is it limited? I want to examine responsibility in terms of proximity through a Lévinasian ethical lens and plurality, defined as more than two individuals, by questioning responsibility in terms of relationality, and vulnerability. I will be looking mainly at five authors Emmanuel Lévinas, Ernst Van Alphen, Judith Butler, Kevin Quashi, and Lisa Guenther, to answer the question of whether Lévinas’s definition of responsibility includes proximity, and plurality. The ‘Practicum-3’ preservice teachers are those who are in their 3rd year of their degree studies and have prior teaching experience. The intent of this qualitative paper is to assess the challenges experienced by the preservice teachers and the tactics used by them to manage these challenges, during their first week of teaching practice. The challenges reported herein were thematically analysed after extrapolating from the participants’ narratives and presented within the context of the Gibbs’ Reflective Cycle. Findings from this study (...) reveal that there are several factors within and outside the control of the student teacher that affect the practicum experience. Further, the preservice teacher’s preparedness and emotional disposition significantly influence discharge of the duty. In the same vein, the mentor-mentee relations contribute a lot in making or breaking the practicum experience for student teachers in the UAE. The findings recommend further explorative assessments of the situation and necessary policy corrections in the preservice teacher education. (shrink)

Los análisis sobre la filosofía política hobbesiana suelen girar alrededor del estudio del miedo y de cómo este es el factor fundamental para explicar el surgimiento de la soberanía. Este artículo tiene como objetivo ir más allá del concepto de temor como base articuladora de la política y centrar el análisis en un terreno fundamental sin el cual no se explica el Leviatán: la alteridad entendida como enemistad. Un análisis de la específica otredad que pone en juego Hobbes a través (...) de su perspectiva mecanicista permitirá entender los diferentes matices que esta visión incorpora, así como las consecuencias que derivan de una filosofía política basada en una concepción negativa de la compañía. El texto explorará las líneas de articulación del concepto de enemistad hobbesiana tanto desde el punto de vista del interior del Estado como desde una perspectiva interestatal. (shrink)

Many consider play a natural part of childhood, and although there is disagreement in the literature on what essentially defines “play” in childhood, philosophical theories of play tend to support this initial consideration. But is childhood play also something we owe each other within a framework of educational justice? This is a question yet to be addressed. In this paper, I answer this question affirmatively. I take off from a generic account of educational justice and argue that childhood play should (...) be considered a central entitlement of this account. I then argue in line with the capability approach that if we ought to protect childhood play, it should be children’s capability for play rather than the functioning of play that needs protection. I end by offering an account of the capability for childhood play. (shrink)

The experience of fear in the Contemporaneity is mostly determined by a type of enmity that, unlike that which prevailed in Modernity, shows itself untraceable, virtual, rarely visible, given the delocalization structures of globalization. All this refers to a specific epistemology, linked to the loss of the classic coordinates of finitude and to the opening of a paradigm where the lack of representation and the apparent depoliticization will turn adversaries and combatants into mere ghosts, thus increasing their terrifying effect. The (...) article tries to review these notions taking the dread that contemporary terrorism causes as a starting point to build a more global reflection that includes the specific type of gear generated in times of pandemic. La experiencia del miedo en la Contemporaneidad viene mayormente determinada por un tipo de enemistad que, a diferencia de la que imperaba en la Modernidad, se muestra ilocalizable, virtual, pocas veces visible, dadas las estructuras de deslocalización propias de la globalización. Todo ello remite a una epistemología concreta, ligada a la pérdida de las coordenadas clásicas de la finitud y a la apertura de un paradigma donde la falta de representacionalidad y su aparente despolitización convertirán a los adversarios y combatientes en meros fantasmas, aumentando así su efecto terrorífico. El artículo pretende revisar estas nociones tomando el pavor que ocasiona el terrorismo contemporáneo como punto de partida para construir una reflexión más global que incluye el específico tipo de miedo generado en tiempos de pandemia. In what ways is students’ participation in school related to their participation and becoming subjects across the school context and other contexts in which they participate? This is the question analyzed in this paper, based on observations of, and narratives and perspectives provided by, three 15-year-old ethnic minority boys and their teachers at a school in Denmark. Drawing upon Davies’ concept of teaching-as-usual, I explore exclusions and marginalization inside school before exploring how these can be seen as connected to, and (...) seemingly having an impact on, the lives the students lead outside school, with an understanding of this connection rooted in Holzkamp’s concept of conduct of everyday life. In this regard, I make use of concepts of recognition ) and viable life in order to point to how a lack of such within the school tends to drive the students away from the school context, while simultaneously driving them towards other contexts outside school, with gang related street communities being a particularly potential one. This paper discusses limitarianism in light of three popular objections to the redistribution of extreme wealth: (i) that such redistribution legitimizes envy, which is a morally objectionable attitude; (ii) that it disincentivizes the wealthy to invest and work, leading to a diminished social product, and, thereby, making everyone worse-off; and (iii) that it undercuts the pursuit and achievement of human excellence by depriving successful people of resources through which they may otherwise excel. We argue that these objections fail to undermine (...) limitarianism. An elaborated defence against them, rather, reveals how the Standard Defence of limitarianism may be expanded and strengthened by relying explicitly on the importance of preventing a society of envy, avoiding harm to non-rich citizens, and ensuring that humans are given the opportunities to excel in ways that go beyond those that allow for the amassment of wealth. Metaphilosophy is itself philosophy about philosophy. It is not something before or independent of philosophy. Both Kai Nielsen and Richard Rorty are deeply concerned (someone might say obsessively preoccupied) with metaphilosophy. They both are thoroughly historicist and contextualist resolutely rejecting any form of a transcendental or metaphysical turn. They argue against claims to absolute validity (as well as against absolutism in any form) and a natural order of reasons: some 'Reason' to which any rational agent must be committed. They (...) both see philosophy as a transitional genre first (historically speaking) from religion then metaphysics and more latterly from scientistic conceptions of the world. But they differ about what philosophy is transitional to. For Rorty it is historical narrative and utopian proposals; for Nielsen it is critical theory. Rorty claims this, Nielsen's intentions to the contrary notwithstanding, commits him to enlightenment rationalism. Nielsen replies that his form of critical theory is deeply historicist and contextual without being resolutely atheoretical. This plays out in political orientation to Nielsen's being a socialist while Rorty is a social democrat. (shrink)

The best accuracy arguments for probabilism apply only to credence functions with finite domains, that is, credence functions that assign credence to at most finitely many propositions. This is a significant limitation. It reveals that the support for the accuracy-first program in epistemology is a lot weaker than it seems at first glance, and it means that accuracy arguments cannot yet accomplish everything that their competitors, the pragmatic (Dutch book) arguments, can. In this paper, I investigate the extent to which (...) this limitation can be overcome. Building on the best arguments in finite domains, I present two accuracy arguments for probabilism that are perfectly general—they apply to credence functions with arbitrary domains. I then discuss how the arguments’ premises can be challenged. We will see that it is particularly difficult to characterize admissible accuracy measures in infinite domains. (shrink)

Epistemic decision theory produces arguments with both normative and mathematical premises. I begin by arguing that philosophers should care about whether the mathematical premises (1) are true, (2) are strong, and (3) admit simple proofs. I then discuss a theorem that Briggs and Pettigrew (2020) use as a premise in a novel accuracy-dominance argument for conditionalization. I argue that the theorem and its proof can be improved in a number of ways. First, I present a counterexample that shows that one (...) of the theorem’s claims is false. As a result of this, Briggs and Pettigrew’s argument for conditionalization is unsound. I go on to explore how a sound accuracy-dominance argument for conditionalization might be recovered. In the course of doing this, I prove two new theorems that correct and strengthen the result reported by Briggs and Pettigrew. I show how my results can be combined with various normative premises to produce sound arguments for conditionalization. I also show that my results can be used to support normative conclusions that are stronger than the one that Briggs and Pettigrew’s argument supports. Finally, I show that Briggs and Pettigrew’s proofs can be simplified considerably. (shrink)
