Background This manuscript explores the pervasive issue of moral distress among nurses and its impact on their well-being and professional satisfaction. Focusing on diverse factors contributing to moral distress, the review spans various experience levels and patient care settings. Method Utilizing integrative reviews and sourcing from PubMed, CINAHL, SCOPUS, PsycINFO, and ProQuest, the study synthesizes findings from studies worldwide. The conceptual framework by Whittemore & Knafl is employed to comprehensively analyze nurses’ experiences. Results Key factors were identified as contributing to (...) moral distress, including concerns about care quality, team dynamics, and insufficient support. Interventions range from light-touch approaches like mentorship programs to resource-intensive strategies such as staff wellness initiatives. The impact of the COVID-19 pandemic on nurses’ moral distress is also explored. Conclusion Moral distress in nurses leads to burnout and, in some cases, prompts professionals to leave the field. The study emphasizes the need for organizational-level initiatives, support networks, and interventions to address moral distress. Identified gaps in the literature underscore opportunities for future research to better prepare clinicians and advance understanding across experience levels and settings. (shrink)

Tests of economic theory often focus on choice outcomes and find significant individual differences in these outcomes. This variability may mask universal psychological processes that lead to different choices because of differences across cultures in the information people have available when making decisions. On this view, decision making research within and across cultures must focus on the processes underlying choice.

Evolutionary theory guides an investigation of foster parent selection in two northern Thai villages with different biosocial environments: one village has high levels of labour migration and divorce, and growing numbers of parental death due to HIV/AIDS, while the other village has lower migration, divorce and parental mortality levels. Focus groups examine mothers motivations and ideals regarding foster caretaker selection, and quantitative family surveys examine real fostering outcomes: specifically, the laterality (matrilateral versus patrilateral) and genetic distance of the foster caretakers (...) of all ever-fostered children in these two villages. As predicted, in environments of high marital stability and paternity certainty, parents seem to prefer close genetic kin from either side as foster parents for their children. In low marital stability and paternity certainty environments, parents trust their own lateral kin, regardless of genetic distance, over close genetic kin from the other side. The striking exception to this pattern, however, occurs in the case of parental death, in which case children are fostered to the deceased parents sex or other factors. In general, the foster parents for girls are selected with more care, reflecting the daughter/female preference expected in traditionally matrilineal, matrilocal societies. An ordered decision-making pathway for foster parent selection is proposed, taking into consideration the key factors of (a) the circumstances driving the fostering decision, (b) the gender of the child, (c) the gender of the key decision-making parent, and (d) the degree of marital and population fluidity (and subsequent paternity certainty) in the village. Ethical climate (EC) has been broadly described as how well institutions respond to ethical issues. Developing a tool to study and evaluate EC that aims to achieve sustained improvements requires a contemporary framework with identified relevant drivers. An extensive literature review was performed, reviewing existing EC definitions, tools and areas where EC has been studied; ethical challenges and relevance of EC in contemporary paediatric intensive care (PIC); and relevant ethical theories. We surmised that existing EC definitions and tools designed to (...) measure it fail to capture nuances of the PIC environment, and sought to address existing gaps by developing an EC framework for PIC founded on ethical theory. In this article, we propose a Paediatric Intensive Care Ethical Climate (PICEC) conceptual framework and four measurable domains to be captured by an assessment tool. We define PICEC as the collective felt experience of interdisciplinary team members arising from those factors that enable or constrain their ability to navigate ethical aspects of their work. PICEC both results from and is influenced by how well ethical issues are understood, identified, explored, reflected on, responded to and addressed in the workplace. PICEC encompasses four, core inter-related domains representing drivers of EC including: (1) organisational culture and leadership; (2) interdisciplinary team relationships and dynamics; (3) integrated child and family-centred care; and (4) ethics literacy. Future directions involve developing a PICEC measurement tool, with implications for benchmarking as well as guidance for, and evaluation of, targeted interventions to foster a healthy EC. (shrink)

Measuring outcomes using quantitative analytic methods is the hallmark of scientific research in healthcare. For clinical ethics support services (CESS), tangible outcome metrics are lacking and literature examining CESS quality is limited to evaluation of single cases or the influence on individual healthcare professional’s perceptions or behaviour. This represents an enormous barrier to implementing and evaluating ethics initiatives to improve quality. In this context, Kok _et al_ propose a theoretical framework for how moral case deliberation (MCD) can drive quality at (...) an organisational-level, termed _morisprudence_ 1. By offering mechanistic insights into how MCD can lead to organisational learning and quality improvement (QI), they provide a roadmap for actionable items to measure quality. As conceptualised, _morisprudence_ follows accumulation of organisational practical wisdom from a repertoire of ‘paradigmatic’ MCDs, aiding identification of patterns of morally important features. Human behavioral ecology (HBE) began as an attempt to explain human economic, reproductive, and social behavior using neodarwinian theory in concert with theory from ecology and economics, and ethnographic methods. HBE has addressed subsistence decision-making, cooperation, life history trade-offs, parental investment, mate choice, and marriage strategies among hunter-gatherers, herders, peasants, and wage earners in rural and urban settings throughout the world. Despite our rich insights into human behavior, HBE has very rarely been used as a tool to help the people (...) with whom we work. This article introduces a special issue of Human Nature which explores the application of HBE to significant world issues through the design and critique of public policy and international development projects. The articles by Tucker, Shenk, Leonetti et al., and Neil were presented at the 104th annual meeting of the American Anthropological Association (AAA) in Washington, D.C., in December 2005, in the first organized session of the nascent Evolutionary Anthropology Section (EAS). We conclude this introduction by summarizing some theoretical challenges to applying HBE, and ways in which evolutionary anthropologists can contribute to solving tough world issues. (shrink)

Background: Facial proportionality and symmetry are positively associated with perceived levels of facial attractiveness. -/- Objective: The aims of this study were to confirm and extend the association of proportionality with perceived levels of attractiveness and character traits and determine differences in attractiveness and character ratings between "anomalous" and "typical" faces using a large dataset. -/- Methods: Ratings of 597 unique individuals from the Chicago Face Database were used. A formula was developed as a proxy of relative horizontal proportionality, where (...) a proportionality score of "0" indicated perfect proportionality and more negative scores indicated less proportionality. Faces were categorized as "anomalous" or "typical" by 2 independent reviewers based on physical features. -/- Results: Across the ratings for all faces, Spearman correlations revealed greater proportionality was associated with attractiveness ( ρ = 0.292, P < 0.001) and trustworthiness ( ρ = 0.193, P < 0.001), while lesser proportionality was associated with impressions of anger (ρ = 0.132, P = 0.001), dominance (ρ = 0.259, P < 0.001), and threateningness ( ρ = 0.234, P < 0.001). Mann-Whitney U tests revealed the typical cohort had significantly higher levels of proportionality (-13.98 versus -15.14, P = 0.030) and ratings of attractiveness (3.39 versus 2.99, P < 0.001) and trustworthiness (3.48 versus 3.35, P < 0.001). -/- Conclusions: This study demonstrated that facial proportionality is not only significantly associated with higher ratings of attractiveness, but also associated with judgements of trustworthiness. Background: Facial attractiveness influences our perceptions of others, with beautiful faces reaping societal rewards and anomalous faces encountering penalties. The purpose of this study was to determine associations of visual attention with bias and social dispositions toward people with facial anomalies. -/- Methods: Sixty subjects completed tests evaluating implicit bias, explicit bias, and social dispositions before viewing publicly available images of preoperative and postoperative patients with hemifacial microsomia. Eye-tracking was used to register visual fixations. -/- Results: Participants with higher implicit (...) bias scores fixated significantly less on the cheek and ear region preoperatively (P = 0.004). Participants with higher scores in empathic concern and perspective taking fixated more on the forehead and orbit preoperatively (P = 0.045) and nose and lips (P = 0.027) preoperativel. -/- Conclusions: Participants with higher levels of implicit bias spent less visual attention on anomalous facial anatomy, whereas participants with higher levels of empathic concern and perspective taking spent more visual attention on normal facial anatomy. Levels of bias and social dispositions such as empathy may predict layperson gaze patterns toward those with facial anomalies and provide insights to neural mechanisms underlying the "anomalous is bad" paradigm. Background: This study tested the core tenets of how facial scars are perceived by characterizing layperson response to faces with scars. The authors predicted that scars closer to highly viewed structures of the face (i.e., upper lip and lower lid), scars aligned against resting facial tension lines, and scars in the middle of anatomical subunits of the face would be rated less favorably. Methods: -/- Volunteers aged 18 years and older from the United States were recruited through Amazon’s Mechanical Turk (...) to complete a face rating survey. Scars were digitally added in different locations and orientations for a total of 14 unique scars added to each face. Each participant rated 50 different faces on confidence, friendliness, and attractiveness. Data were analyzed using linear mixed effects models. Results: -/- A total of 88,850 ratings [82,990 scarred (93.4 percent)] for attractiveness, friendliness, and confidence were analyzed. In univariate linear mixed effects models, the presence of a facial scar did not significantly impact attractiveness (β = 0.016, SE = 0.014, z = 1.089, p = 0.276). A second set of linear mixed effects models identified interactions between location, subunit placement, and orientation to facial tension lines. Scars located on the lower lid mid subunit perpendicular to facial tension lines were rated less attractive (β = −0.065, SE = 0.028, z = −2.293, p = 0.022). Conclusions: -/- On average, a single well-healed facial scar does not negatively affect first impressions of attractiveness, confidence, or friendliness. Specific scar location and orientation combinations, however, such as a perpendicular scar at the mid-lower eyelid, may result in lower perceived attractiveness, confidence, and friendliness. Paediatric Intensive Care Units (PICU) are complex interdisciplinary environments where challenging, high stakes decisions are frequently encountered. We assert that appropriate decisions are more likely to be made if the decision-making process is comprehensive, reasoned, and grounded in thoughtful deliberation. Strategies to overcome barriers to high quality decision-making including, cognitive and implicit bias, group think, inadequate information gathering, and poor quality deliberation should be incorporated. Several general frameworks for decision-making exist, but specific guidance is scarce. In this paper, we provide (...) specific guidance on collaborative complex decision-making for PICUs. The proposed approach is on principles of procedural justice and pragmatic hermeneutics. The process encompasses set-up/planning, information gathering, question formulation, analysis (perspectives, values, and principles), action plan development, decision documentation, and a review and appeal mechanism. The process can be adapted to suit other clinical contexts. Research evaluating the process, exploring how best to develop education for clinicians, and how to build a culture that values high quality deliberation, is worthwhile. (shrink)

It remains unclear whether such genetic variants are associated with disease aggressiveness. The NCI-SPORE Genetics Working Group retrospectively collected clinicopathologic information and genotype data for 36 SNPs which at the time had been validated to be associated with PC risk from 25,674 cases with PC. Cases were grouped according to race, Gleason score and aggressiveness. Statistical analyses were used to compare the frequency (...) of the SNPs between different disease cohorts. After adjusting for multiple testing, only PC-risk SNP rs2735839 was significantly and inversely associated with aggressive and high-grade disease in European men. Similar associations with aggressive and high-grade disease were documented in African-American subjects. The G allele of rs2735839 was associated with disease aggressiveness even at low PSA levels in both European and African-American men. Our results provide further support that a PC-risk SNP rs2735839 near the KLK3 gene on chromosome 19q13 may be associated with aggressive and high-grade PC. Future prospectively designed, case-case GWAS are needed to identify additional SNPs associated with PC aggressiveness. (shrink)

The two-factor theory (Davies, Coltheart, Langdon & Breen 2001; Coltheart 2007; Coltheart, Menzies & Sutton 2010) is an influential account of delusion formation. According to the theory, there are two distinct factors that are causally responsible for delusion formation. The first factor is supposed to explain the content of the delusion, while the second factor is supposed to explain why the delusion is adopted and maintained. Recently, another remarkable account of delusion formation has been proposed, in which the notion of (...) “prediction error” plays the central role (Fletcher & Frith 2009; Corlett, Krystal, Taylor & Fletcher 2009; Corlett, Taylor, Wang, Fletcher & Krystal 2010). According to this account, the prediction-error theory, delusions are formed in response to aberrant prediction-error signals, those signals that indicate a mismatch between expectation and actual experience. -/- In this chapter, we examine the relationship between the two-factor theory and the prediction-error theory in some detail. Our view is that the prediction-error theory does not have to be understood as a rival to the two-factor theory. We do not deny that there are some important differences between them. However, those differences are not as significant as they have been presented in the literature. Moreover, the core ideas of the prediction-error theory may be incorporated into the two- factor framework. For instance, the aberrant prediction-error signal that is posited by prediction-error theorists can be (or underlie) the first factor contributing to the formation of some delusions, and help explain the content of those delusions. Alternatively, the aberrant prediction-error signal can be (or underlie) the second factor, and help explain why the delusion is adopted and maintained. (shrink)

Much of the history of Western ethical thought has revolved around debates about what constitutes a good life, and claims that a good life is achievable only by certain human beings. In Feminist Philosophies of Life, feminist, new materialist, posthumanist, and ecofeminist philosophers challenge this tendency, approaching the question of life from alternative perspectives. Signalling the importance of distinctively feminist reflections on matters of shared concern, Feminist Philosophies of Life not only exposes the propensity of discourses to normalize and exclude (...) differently abled, racialized, feminized, and gender nonconforming people, it also asks questions about how life is constituted and understood without limiting itself to the human. The standard analysis of quantification says that determiner quantifiers (such as every) take an NP predicate and create a generalized quantifier. The goal of this paper is to subject these beliefs to crosslinguistic scrutiny. I begin by showing that in St'á'imcets (Lillooet Salish), quantifiers always require sisters of argumental type, and the creation of a generalized quantifier from an NP predicate always proceeds in two steps rather than one. I then explicitly adopt the strong null hypothesis that the denotations of (...) quantifiers should be crosslinguistically uniform. Since the Salish data cannot be captured by the usual analysis of English, I pursue the idea that English is reducible to the Salish pattern. Reanalysis of many English constructions is required. I argue that the reanalysis has advantages over the standard analysis for partitives, as well as for non-partitive all- and most-phrases, which I analyze as containing bare plurals of argumental type. Even where the new analysis faces some challenges (for example, with every), the attempt still leads to fruitful results. It forces us to view familiar constructions in a new light, and to redefine, I believe correctly, which quantificational constructions are ‘basic’ and which stand in need of further explanation. This article examines how the ideology of white injury both conceals and sustains inequitable social relations in turn‐of‐the‐millenium California. Understanding the political and economic context of California in the early 1990s in relation to media, law, and culture helps explain why Californian citizens passed the unconstitutional initiative, Proposition 187, in 1994. Targeting undocumented Mexican immigrants, this ‘color‐blind’ Proposition functioned to conflate economic insecurities with racial anxieties. An analysis of culture, law, and media discloses how racial anxieties limit our understandings of (...) exploitative capitalist relations, serving to artificially augment the white middle‐class ‘ wealth, opportunities, and power, while making vulnerable populations in the United States even more open to exploitation. NHS Blood and Transplant is responsible for the procurement and allocation of human organs in the United Kingdom. Its main role is to “ensure that organs donated for transplant are matched and allocated to patients in a fair and unbiased way.” NHSBT’s liver allocation policies are underpinned by the National Liver Transplant Standards, a document published by the Department of Health in 2005 to oversee patient care, patient assessment, liver allocation and transplantation, education and training, and research and development. As part of the effort to promote policies and practices ensuring the ethical (...) conduct of research, innovation, and health care delivery, the bioethics commission staff, in consultation with commission members, has used the bioethics commission reports to develop pedagogical materials for traditional and nontraditional educational settings. Recent social and cultural theory has emphasized that in risk culture the achievement of a reflexive self-identity is a key resource, for example, in terms of employment, citizenship and intimacy. Commentators on shifts in the organization of health have also stressed the significance of achieving a self-reflexive identity. So, for example, knowing, self-monitoring subjects have emerged as optimal citizens in relation to health. While there is certainly some critical commentary on these kinds of moves, nevertheless reflexive sexual subjects in relation (...) to health have received less critical attention. In this article, through an analysis of HIV antibody testing and, in particular, by looking at the making of reflexivity through the practice of testing, I consider the emergence of a heterosexuality defined in terms of reflexivity. In so doing I suggest that reflexivity should not be understood - as many commentators suggest - as an effect but rather as constitutive of risk culture. (shrink)

She wants to reaffirm the traditional unity of sex, love and parenthood, not as an absolute norm, but a guiding framework. The book also develops the significance of New Testament models of community and of moral formation, to argue that the human values associated with sex and family should be embodied in a context of concern for society's poor and marginalized. This article situates Hannah Arendt’s On Revolution in the traditions of French and American revolutionary historiography to demonstrate that Arendt’s ‘fable’ of the American Revolution was at odds with her argument about the council form. I argue that had Arendt really wanted to inspire a resurrection of the council form in the present, she would have done better to orient her readers to the French Revolution, specifically to the experiments in democratic republicanism of the group known as the Girondins.

Mandarin Chinese exhibits two paradigms of conditionals with indefinite wh-words that have the semantics of donkey sentences, represented by ‘bare conditionals’ on the one hand and ruguo- and dou-conditionals on the other. The bare conditionals require multiple occurrences of wh-words, disallowing the use of overt or covert anaphoric elements in the consequent clause, whereas the ruguo- and dou-conditionals present a completely opposite pattern. We argue that the bare conditionals are cases of unselective binding par excellence (Heim 1982, Kamp 1981) while (...) the ruguo- and dou-conditionals are most naturally accounted for with the traditional E-type pronoun strategy of Evans (1980). We thus argue partly for a return to the E-type strategy (along with Heim 1990) but maintain the need for unselective binding in UG (cf. Kratzer 1989, Chierchia 1992). It is further shown that these two paradigms do not differ with respect to the proportion problem and the distribution of symmetric and asymmetric readings of Kadmon (1987), though they differ with respect to ∀ and ∃ readings (discussed in Chierchia 1992) in a non-trivial way that provides further support for the proposed approach. Finally, evidence is given that the bare conditionals should be kept apart from correlative constructions in languages like Hindi, and treated differently from the latter. (shrink)
