The University of Pittsburgh Medical Center (UPMC) "Policy for the Management of Terminally Ill Patients Who May Become Organ Donors after Death" proposes to take organs from certain patients as soon as possible after expected cardiopulmonary death. This policy requires clear understanding of the descriptive state of the donor's critical cardiopulmonary and neurologic functional capacity at the time interventions to sustain or harvest organs are undertaken. It also requires strong consensus about the moral and legal status of the donor during (...) dying and confirmation of death. There has been no need for the clarity and precision that this policy relies upon, and thus the needed research and conceptual work has not previously been generated. The empiric base and societal consensus are both too frail to provide justification for this policy at this time. (shrink)

The author has previously shown that for a certain class of structures $\mathcal {I}$, $\mathcal {I}$-indexed indiscernible sets have the modeling property just in case the age of $\mathcal {I}$ is a Ramsey class. We expand this known class of structures from ordered structures in a finite relational language to ordered, locally finite structures which isolate quantifier-free types by way of quantifier-free formulas. This paper examines, on the basis of a longitudinal corpus of 50 early modern authors, how change at the aggregate level of the community interacts with variation and change at the micro-level of the individual language user. In doing so, this study aims to address the methodological gap between collective change and entrenchment, that is, the gap between language as a social phenomenon and the cognitive processes responsible for the continuous reorganization of linguistic knowledge in individual speakers. Taking up the (...) case of the prepositional passive, this study documents a strong community-wide increase in use that is accompanied by increasing schematicity. A comparison of the 50 authors reveals that regularities arising at the macro-level conceal highly complex and variable individual behavior, aspects of which may be explained by studying the larger (social) context in which these individuals operate (e. g., age cohorts, community of practice, biographical insights). Further analysis, focusing on how authors use the prepositional passive in unique and similar ways, elucidates the role of small individual biases in long-term change. Overall, it is demonstrated that language change is an emergent phenomenon that results from the complex interaction between individual speakers, who themselves may change their linguistic behavior to varying degrees. Some commentators have recently proposed that “clinical equipoise,” although widely accepted, is not necessary for morally acceptable research on human subjects. If this concept is rejected, however, we may find that trials not in the best medical interests of their subjects—”bad deal trials”—could be justified. To avoid exploiting participants, we must find a way to distribute the risks fairly, even if it means embracing radical changes in the way clinical research is conducted.

: "Clinical pragmatism" is an important new method of moral problem solving in clinical practice. This method draws on the pragmatic philosophy of John Dewey and recommends an experimental approach to solving moral problems in clinical practice. Although the method may shed some light on how clinicians and their patients ought to interact when moral problems are at hand, it nonetheless is deficient in a number of respects. Clinical pragmatism fails to explain adequately how moral problems can be solved experimentally, (...) it underestimates the relevance and importance of judgment in clinical ethics, and it presents a questionable account of the role that moral principles should play in moral problem solving. For a long time, linguists more or less denied the existence of individual differences in grammatical knowledge. While recent years have seen an explosion of research on individual differences, most usage-based research has failed to address this issue and has remained reluctant to study the synergy between individual and community grammars. This paper focuses on individual differences in linguistic knowledge and processing, and examines how these differences can be integrated into a more comprehensive constructionist theory of grammar. The examination is (...) guided by the various challenges and opportunities that may be extracted from scattered research that exists across disciplines touching on these matters, while also presenting some new data that illustrate how differentiation between individuals can improve models of long-term language change. Can one be fooled into believing that one intended an action that one in fact did not intend? Past experimental paradigms have demonstrated that participants, when provided with false perceptual feedback about their actions, can be fooled into misperceiving the nature of their intended motor act. However, because veridical proprioceptive/perceptual feedback limits the extent to which participants can be fooled, few studies have been able to answer our question and induce the illusion to intend. In a novel paradigm addressing this (...) question, participants were instructed to move a line on the computer screen by use of a phony brain–computer interface. Line movements were actually controlled by computer program. Demonstrating the illusion to intend, participants reported more intentions to move the line when it moved frequently than when it moved infrequently. Consistent with ideomotor theory, the finding illuminates the intimate liaisons among ideomotor processing, the sense of agency, and action production. (shrink)

BackgroundFor children with life-limiting conditions who are unable to participate in decision-making, decisions are made for them by their parents and paediatricians. Shared decision-making is widely recommended in paediatric clinical care, with parents preferring a collaborative approach in the care of their child. Despite the increasing emphasis to adopt this approach, little is known about the roles and responsibilities taken by parents and paediatricians in this process. In this study, we describe how paediatricians approach decision-making for a child with a (...) life-limiting condition who is unable to participate in decision-making for his/herself.MethodsThis qualitative phenomenological study involved 25 purposively sampled paediatricians. Verbatim transcripts from individual semi-structured interviews, conducted between mid-2019 and mid-2020, underwent thematic analysis. Interviews were based around a case vignette matched to the clinical experience of each paediatrician.ResultsTwo key themes were identified in the exploration of paediatricians' approach to decision-making for children with life-limiting conditions: there is a spectrum of paediatricians’ roles and responsibilities in decision-making, and the specific influences on paediatricians’ choice of approach for end-of-life decisions. In relation to, analysis showed four distinct approaches: non-directed, joint, interpretative, and directed. In relation to, the common factors were: harm to the child, possible psychological harm to parents, parental preferences in decision-making, and resource allocation.ConclusionsDespite self-reporting shared decision-making practices, what paediatricians often described were physician-led decision-making approaches. Adopting these approaches was predominantly justified by paediatricians’ considerations of harm to the child and parents. Further research is needed to elucidate the issues identified in this study, particularly the communication within and parental responses to physician-led approaches. We also need to further study how parental needs are identified in family-led decision-making approaches. These nuances and complexities are needed for future practice guidance and training around paediatric decision-making.Trial registration: Not applicable. (shrink)

Usage-based linguistics postulates that multi-word expressions constitute a substantial part of language structure and use, and are formed through repeated chunking and stored as exemplar wholes. They are also re-used to produce new sequences by means of schematization. While there is extensive research on multi-word expressions in many spoken languages, little is known about the status of multi-word expressions in the mainstream U.S. variety of American Sign Language. This paper investigates recurring multi-word expressions, or sequences of multiple signs, that involve (...) a high-frequency sign of visual perception glossed as look and the family of ‘look’ signs. The look sign exhibits two broad functions: look/‘vision’ references literal or metaphorical vision and look/‘reaction’ signals a person’s reaction to a visual stimulus. Data analysis reveals that there are recurring sequences in distinct syntactic environments associated with the two functions of look, suggesting that look is in the process of grammaticalization from a verb of visual perception to a stance verb. The sequences demonstrate the emergence of linguistic structure from repeated use through the domain-general cognitive process of chunking in ASL. (shrink)

This article revisits the "harmony doctrine" school of economics and its distinctive understanding of how ethics and economics intersect. Harmony doctrine thinkers staked out a “natural” understanding of economic phenomena that in many ways fused the classical political economy of Adam Smith with the earlier French Physiocratic School. Their metaphysically grounded interpretation was largely eclipsed by the developments of utilitarian and marginalist schools by the end of the nineteenth century. Yet harmony doctrine thinking adhered to a distinct understanding of how (...) ethical processes and outcomes play out within the economic sphere that has to date gone under examined by business ethicists. This paper outlines three characteristics of harmony doctrine views: the belief in curative forces empowered to "heal" the economic order, a dichotomous interpretation of the economic realm that distinguishes the “natural” from the “artificial,” and an overarching belief in a harmonious social order actualized by cooperative interdependencies within economic enterprises. Despite falling into obscurity in the economic discipline, all these ideas found new life in intellectual currents of organizational and political thought at the turn of the twentieth century. After surveying these historical developments, this article concludes by demonstrating how harmony doctrine thinking helpfully illuminates persisting metaphysical dimensions of contemporary business ethics. This essay takes a first step in comparative ethics by looking to Aristotle and the Aztec's conceptions of the good life. It argues that the Aztec conception of a rooted life, neltiliztli, functions for ethical purposes in a way that is like Aristotle's eudaimonia. To develop this claim, it not only shows just in what their conceptions of the good consist, but also in what way the Aztecs conceived of the virtues (in qualli, in yectli).

A curious feature of Aztec philosophy is that the basic metaphysical question of the “Western” tradition cannot be formulated in their language, in Nahuatl. This did not, however, prevent the Aztecs from developing an account of 'reality', or whatever it is that might exist. The article is the first of its kind to compare the work of Aristotle on ousia (being) and the Aztecs on teotl and ometeotl. Through this analysis, it suggests that both of the Nahuatl terms are fundamental (...) for expressing the basic character of reality as process for the Aztecs. It also defends their notion as prima facia reasonable, despite its wide divergence from Aristotle's view. (shrink)

Source credibility has been thought to bean important determinant of peoples‘ reactions toinformation about technology. There has also been muchdebate about the need to communicate effectively withthe public about genetic engineering, particularlywithin the context of food production. Questionnaireswere used to investigate the impact of sourcecredibility, admission of risk uncertainty, andinitial attitude towards genetic engineering onattitudes of respondents after information provision.120 respondents with positive attitudes towardsgenetic engineering in food production were providedwith persuasive information about the technology,where both source attribution and admission (...) of riskuncertainty were systematically varied within theexperimental design. Impact on perceptions of sourcecredibility, informational qualities, and postintervention attitudes were examined, and compared toa second group of respondents who held initiallynegative attitudes towards genetic engineering, andwho had been exposed to similar informationinterventions. As predicted by Social Judgment Theory,initial attitude was found to be an importantdeterminant of post-intervention attitude. However,admission of risk uncertainty was also found to beinfluential in increasing acceptance and reducingrejection of the technology, possibly through thefacilitation of elaborative processing. Contrary toprevious research, prior attitudes had an impact onperceptions of both source credibility andinformational quality. In terms of effectiveinformation provision about genetic engineering, itwas concluded that scientific honesty is the bestpolicy, and that lay understanding of scientificprocess is probably greater than hitherto assumed byexperts. Recent empirical work on the concept of intentionality suggests that people’s assessments of whether an action is intentional are subject to uncertainty. Some researchers have gone so far as to claim that different people employ different concepts of intentional action. These possibilities have motivated a good deal of work in the relatively new field of experimental philosophy. The findings from this empirical research may prove to be relevant to medical ethics. In this article, we address this issue head on. We (...) first describe a study we conducted on intention ascription. Drawing on recent work in experimental philosophy, we investigated the possibility that the ascription of intentions to clinical actors in clinical settings is influenced by prior judgments about the goodness or badness of the consequences of the action in question. Our study was modeled on experimental studies in other contexts that have shown that people, when presented with a range of scenarios, are more likely to classify a side effect of an action as intended if the side effect is negative or reflects poorly on the actor than if it is positive or reflects well on the actor. We investigated whether this asymmetry in intention ascriptions was also present among physicians who were asked to ascribe intentions to clinical actors in certain well-defined clinical scenarios. The cultural importance of this new machine, however, remains less fully analyzed. This essay explores one facet of that history through a discussion of the commodification of launches in France and Great Britain. These two countries, which have larger middling classes as well as a higher degree of commercialization in general, provided (...) a fertile environment for aeronauts seeking to instruct and entertain an audience willing to fund ballooning. Balloonists had to invent ways to market this scientific discovery and determine how best to attract paying customers. Whilst there has been much debateregarding the importance of public acceptance ofgenetic engineering and its applications, there isevidence to indicate that objections to the technologyare likely to focus on specific applications of thetechnology, rather than genetic engineering per se.Thus it becomes important to examine the extent ofobjections associated with individual applications,rather than to assess public feeling regarding thetechnology overall. Survey data were collected from200 respondents regarding their objections to generalapplications of genetic engineering (where thetangible benefits were not obvious). Similar objectiondata (...) were collected from 200 different respondents,who were presented with specific applications withmore obvious tangible benefits. Overall patterns ofobjection to different applications were identifiedusing a novel method of objection mapping, inconjunction with analysis of variance to identifyindividual differences in the samples. For generalapplications, the results indicate that mostrespondents object less to applications involvingplants and microorganisms than to those involvinganimals or human genetic material. Individualdifferences in objection focus on applicationsinvolving animals or human genetic material, withwomen and those who are very concerned with theenvironment having greatest objections to theseapplications. Individual differences tend to reducewhen specific applications are used as stimuli,although the focus of concern is still on applicationsinvolving animals and human genetic material. However,gender differences were not statistically significant,and those respondents who have high levels ofenvironmental concern are differentiated by increasedobjections to large-scale agricultural applications.It is argued that effective communication regardingthe technology should focus on specific applications,and address issues of environmental impact within thecontext of these applications, if the public is tomake an informed choice regarding their acceptance ofthe products of the technology. In the traditional fix-it model of medical decision making, the identified problem is typically characterized by a diagnosis that indicates a deviation from normalcy. When a medical problem is multifaceted and the available interventions are only partially effective, a broader vision of the health care endeavor is needed. What matters to the patient, and what should matter to the practitioner, is the patient's future possibilities. More specifically, what is important is the character of the alternative futures that the patient could (...) have and choosing among them so as to achieve the best future possible, with the ranking of outcomes determined by the patient's preferences. This paper describes the fix-it model, presents and defends the outcomes-based model, and demonstrates that the latter is useful in developing normative conceptions of informed consent and decision making and in establishing a basis for societal involvement in the decision making process. Following the highly publicized New Bedford rape case, in which a young woman was raped by several men on a pool table in New Bedford, Massachusetts, on March 6, 1983, a segment of the local Portuguese community responded with great hostility to the rape victim and with sympathy for the rapists. The victim was blamed for the ethnic prejudice that erupted after the rape and culminated in the trial of six rapists in 1984. This article's purpose is to analyze the (...) Portuguese community's response, particularly the negative reactions of Portuguese women to another Portuguese woman who had been raped, and the conflict that developed between ethnic and sexual forms of prejudice. In educational research that calls itself empirical, the relationship between validity and reliability is that of trade-off: the stronger the bases for validity, the weaker the bases for reliability. Validity and reliability are widely regarded as basic criteria for evaluating research; however, there are ethical implications of the trade-off between the two. The paper traces a brief history of the concepts, and then describes four ethical issues associated with the validity–reliability trade-off in educational research: bootstrapping, stereotyping, dehumanization, and determinism. This paper examines the consequences for agency that Foucault’s historiographical approach constructs. The analysis begins by explaining the difference between ‘legislative history’ and ‘exemplary history,’ drawing parallels to similar theoretical distinctions offered in the works of Max Weber, J.L. Austin, and Zygmunt Bauman. The analysis continues by reading Habermas’s critique of Foucault through the tropological lenses suggested by White [Metahistory. Baltimore, MD: Johns Hopkins University Press, 1973]; it argues that Habermas’s critique misrecognizes the tropes of Foucaultian genealogy. The paper draws (...) implications for education by articulating possibilities for praxis and agency in terms of pedagogy specifically related to the distinction between didactics and modeling. This paper is about women nurse veterans from the Royal Australian Army Nursing Corps (RAANC) who served in Vietnam. I aim to develop an understanding about these nurses that might place their experiences into a wider context. My conclusions provide starting points for future studies on myth, remembering and oral history.

The new discourse of the Internet has political uses in spreading neo-Nazi ideology and action. I look at two kinds of online neo-Nazi discourse: hate speech itself, including text, music, online radio broadcasts, and images that exhort users to act against target groups; and persuasive rhetoric that does not directly enunciate but ultimately promotes or justifies violence. The online location of these (...) discourses poses urgent questions. Does information technology make the re-emergence of prejudicial messages and attitudes swifter and more likely? Does the Internet's wide range of distribution make for more followers and finally more persuasion? (shrink)

The book asks how we can make sense of career paths for PhD graduates, something that has rarely been systematically studied. It offers a coherent synthesis of the empirically-based insights that arose from the experiences of 48 early career researchers, who were participants in a 10-year qualitative longitudinal research program. The book has the power to inform other researchers’ conceptual and methodological approaches to the study of post-PhD career trajectories. The authors draw on the conceptual lens of ‘identity-trajectory’, which emerged (...) from their research program, to examine the decision-making processes underpinning the careers of PhD graduates, whether contingent researchers and teachers, assistant professors within the academy or professionals elsewhere. Florence Nightingale on Society and Politics, Philosophy, Science, Education and Literature, Volume 5 in the Collected Works of Florence Nightingale, is the main source of Nightingale’s work on the methodology of social science and her views on social reform. Here we see how she took her “call to service” into practice: by first learning how the laws of God’s world operate, one can then determine how to intervene for good. There is material on medical statistics, the census, pauperism and Poor (...) Law reform, the need for income security measures and better housing, on crime, gender and the family. Her comments on a new edition of The Dialogues of Plato are given, with their impact on the revision of the next edition. We see Nightingale’s condemnation of Plato’s “community of wives,” with her stirring approval of love (even outside marriage!), marriage and the family. In this volume also her views on natural science, education and literature are reported. Nightingale was an astute behind-the-scenes political activist. Society and Politics publishes (much of it for the first time) her correspondence with such leading political figures as Queen Victoria, W.E. Gladstone and J.S. Mill. There are notes and essays on public administration and personal observations on various members of royalty, prime ministers and ministers, and Indian viceroys. Nightingale’s support of the vote for women (contrary to much in the secondary literature) is here shown. Correspondence and notes on British general elections from 1834 to 1900 is reported, with letters to and for (Liberal) political candidates and fierce condemnations of Conservatives.
