his article develops a set of recommendations for the psychiatric and medical community in the treatment of mental disorders in response to the recently published fifth edition of the Diagnostic and Statistical Manual of Mental Disorders, that is, DSM-5. We focus primarily on the limitations of the DSM-5 in its individuation of Complicated Grief, which can be diagnosed as Major Depression under its new criteria, and Post-Traumatic Stress Disorder (PTSD). We argue that the hyponarrativity of the descriptions of these disorders (...) in the DSM-5, defined as the abstraction of the illness categories from the particular life contingencies and personal identity of the patient (e.g., age, race, gender, socio-economic status), constrains the DSM-5's usefulness in the development of psychotherapeutic approaches in the treatment of mental disorders. While the DSM-5 is useful in some scientific and administrative contexts, the DSM's hyponarrativity is problematic, we argue, given that the DSMs are designed to be useful guides for not only scientific research, but also for the education of medical practitioners and for treatment development. our goal therefore is to offer suggestions for mental health practitioners in using the DSM-5, so that they can avoid or eliminate the problems that may stem from the limitations of hyponarrativity. When such problems are eliminated, we believe that effective psychotherapeutic strategies can be developed, which would be successful in repairing the very relationships that are strained in mental disorder: the patient's relationship to herself, her physical environment, and her social environment. Philosophers of science typically focus on the epistemic performance of scientific models when evaluating them. Analysing the effects that models may have on the world has typically been the purview of sociologists of science. We argue that the reactive (or “performative”) effects of models should also figure in model evaluations by philosophers of science. We provide a detailed analysis of how models in financial economics created the impetus for the growing importance of the phenomenon of “passive investing” in financial markets. (...) Considering this case motivates the position that we call contextualism about model evaluation, or _model contextualism_ for short. Model contextualism encompasses standard analyses of the epistemic performance of the model, but also includes their reactive aspects. It entails identifying the _epistemic and contextual import of the model_, the ways in which a model can engender change in the world (which we call the _channels of transmission_), and the _interactions between the epistemic and reactive import of a model_. During communication, we form assumptions about what our communication partners know and believe. Information that is mutually known between the discourse partners—their common ground—serves as a backdrop for successful communication. Here we present an introduction to the focus of this topic, which is the role of memory in common ground and language use. Two types of questions emerge as central to understanding the relationship between memory and common ground, specifically questions having to do with the representation of common ground in (...) memory, and the use of common ground during language processing. Background: International developments suggest that providing clinical ethics services to help clinicians negotiate ethical issues that arise in clinical practice is beneficial and reflects best practice in promoting high ethical standards and patient-centered care. The aim of this study was to explore the needs and experiences of clinical staff members to inform the development of future clinical ethics support. Methods: Health professionals at a large regional health service completed an online survey containing questions about the frequency of ethical and legal (...) issues encountered in clinical practice, the type of situations that gave rise to the ethical and/or legal uncertainty or concern, how clinicians currently address these issues, and what support would be welcome. Results: The survey was completed by 369 staff members, including 61% with more than 10 years in the profession and 51% in nursing/midwifery. Two-thirds (66%) indicated they often considered ethical implications of their clinical decisions, and half (49%) often considered the legal implications. More than half (58%) were often/occasionally concerned about the ethically right thing to do. Patient requests for borderline treatment (47%), staff disagreements about patient care (48%), and patients declining recommended treatment (54%) were the most frequent reasons given for ethical or legal uncertainty. Sixty-nine percent of respondents indicated that the way their department addressed ethical issues could be improved and 85% agreed that there is a need for more discussion on ethical issues. The majority (82%) of respondents had encountered ethically challenging clinical situations where additional support would have helped. Common approaches to issues were discussion with colleagues/supervisor (91.1%), discussion at handover/group forums (50.8%), and consultation with guidelines/protocols (60.9%). Conclusions: A majority of clinical staff members surveyed have encountered ethically/legally challenging cases daily for which they have often sought additional advice. This study indicates that many clinical staff members would welcome some form of additional clinical ethics support including advice and education. (shrink)

This paper considers whether individual companies have an ethical obligation to label their Genetically Modified (GM) foods. GM foods and ingredients pervade grocery store shelves, despite the fact that a majority of North Americans have worries about eating those products. The market as whole has largely failed to respond to consumer preference in this regard, as have North American governments. A number of consumer groups, NGO’s, and activist organizations have urged corporations to label their GM products. This paper asks whether, (...) in such a situation, individual corporations can be ethically required to take such unilateral action. We argue that they cannot. Given the lack of solid evidence for any risk to human health, and the serious market disadvantage almost surely associated with costly unilateral action, no individual company has an ethical obligation to label its GM foods. Objective Analyze racial differences in comfort with medical research using an alternative to the traditional approach that treats white people as a raceless norm.Methods Quantitative analysis of survey responses (n = 1,570) from Black and white residents of the US to identify relationships between perceptions of research as a right or a risk, and comfort participating in medical research.Results A lower proportion of white respondents reported that medical experimentation occurred without patient consent (p < 0.001) and a higher proportion of (...) white respondents reported that it should be their right to participate in medical research (p = 0.02). Belief in one’s right to participate was significantly predictive of comfort (b = 0.37, p < 0.001). Belief in experimentation without consent was significantly predictive of comfort for white respondents but not for Black respondents in multivariable analysis.Conclusions A rights-based orientation and less concern about the risks of medical research among white respondents demonstrate comparative advantage. Efforts to diversify medical research may perpetuate structural racism if they do not (1) critically engage with whiteness and its role in comfort with participation, and (2) identify and respond specifically to the needs of Black patients. The American Society for Bioethics and Humanities promulgated standards for clinical ethics consultants and is currently developing a national Quality Attestation in Clinical Ethics Consultation to assist facilities in ensuring that those performing clinical ethics consultations meet minimum standards. As the field moves towards such professionalization, there is a need to provide access to qualified clinical ethicists at a broad range of medical facilities. Currently, however, there are insufficient numbers of trained clinical ethicists to staff all healthcare facilities, and many (...) facilities lack the necessary resources to hire staff clinical ethicists. In this review, we describe several models for providing expert clinical ethics support to remote facilities that lack access to qualified clinical ethicists. Based on this analysis, we recommend telemedicine as the optimal model providing expert support to local ethics committee members tasked with providing clinical ethics consultation services. ABSTRACT Aphantasia is a spectrum disorder, affecting the ability of otherwise healthy individuals to form voluntary or conscious mental images, and in some cases also any form of sensory representation. Although only discovered in 2010, it is now estimated that 2–3% of the population may have aphantasia – otherwise termed, the absence of a ‘mind’s eye,’ that aspect of conscious experience which so many people take for granted as part of their general way of experiencing the world. Aphantasia, although it (...) does not imply the absence of all forms of imagination, such as perspective taking and narrative construction, is quite literally a disorder affecting the individual’s capacity to form and work with images, and thus the absence of a certain way in which consciousness engages with the image-object, distinct both from perception and thought. This paper examines how phenomenological accounts of imagination can help us to better understand aphantasia, and also the ways in which aphantasia calls into question some aspects of an existential account of self. Aim: Understanding the effects of psychotherapy is a crucial concern for both research and clinical practice, especially when outcome tends to be negative. Yet, while outcome is predominantly evaluated by means of quantitative pre-post outcome questionnaires, it remains unclear what this actually means for patients in their daily lives. To explore this meaning, it is imperative to combine treatment evaluation with quantitative and qualitative outcome measures. This study investigates the phenomenon of non-improvement in psychotherapy, by complementing quantitative pre-post outcome scores (...) that indicate no reliable change in depression symptoms with a qualitative inquiry of patients’ perspectives. Methods: The study took place in the context of a Randomized Controlled Trial evaluating time-limited psychodynamic and cognitive behavioural therapy for major depression. A mixed methods study was conducted including patients’ pre-post outcome scores on the BDI-II-NL and post treatment Client Change Interviews. Nineteen patients whose data showed no reliable change in depression symptoms were selected. A grounded theory analysis was conducted on the transcripts of patients’ interviews. Findings: From the patients’ perspective, non-improvement can be understood as being stuck between knowing versus doing, resulting in a stalemate. Positive changes (mental stability, personal strength and insight) were stimulated by therapy offering moments of self-reflection and guidance, the benevolent therapist approach and the context as important motivations. Remaining issues (ambition to change but inability to do so) were attributed to the therapy hitting its limits, patients’ resistance and impossibility and the context as a source of distress. “No change” in outcome scores therefore seems to involve a “partial change” when considering the patients’ perspectives. Conclusion: The study shows the value of integrating qualitative first-person analyses into standard quantitative outcome evaluation and particularly for understanding the phenomenon of non-improvement. It argues for more multi-method and multi-perspective research to gain a better understanding of (negative) outcome and treatment effects. Implications for both research and practice are discussed. Strategies for effectively communicating scientific findings to the public are an important and growing area of study. Recognizing that some complex subjects require recipients of information to take a more active role in constructing an understanding, we sought to determine whether it was possible to increase subjects’ intellectual effort via “priming” methodologies. In particular, we asked whether subconsciously priming “intellectual virtues”, such as curiosity, perseverance, patience, and diligence might improve participants’ effort and performance on various cognitive tasks. In the first (...) experiment, we found no significant differences in either effort or understanding between IV-primed and neutrally-primed individuals across two different priming techniques. The second experiment measured the effect of IV-priming on intellectual effort in simpler, shorter-duration puzzles and exploration activities; here, we observed an effect, but given its low strength and short duration, we do not believe that priming of IVs is a promising strategy for science communication. Within the arena of sport, as throughout society, traditional definitions of femininity and masculinity have established and maintained gender differentiation. The authors’research examines this pattern in intercollegiate athletics by analyzing National Collegiate Athletic Association media guide cover photographs. They find gender differentiation in the depiction of women and men athletes. For example, women athletes are less likely to be portrayed as active participants in sport and more likely to be portrayed in passive and traditionally feminine poses. These differences changed little (...) between 1990 and 1997. The findings suggest that while one might expect less gender stereotyping from the teams themselves, the gendered images produced by intercollegiate athletic programs vary little from those produced by the popular press. Emerging from participant observation fieldwork in varied interfaith organizations, this essay argues that intentional interfaith engagement in the United States is a decidedly classed phenomenon that too rarely includes the presence and concerns of persons who are working poor. This dynamic is particularly problematic given religious entanglements with free-market capitalism and the specific political economic vulnerability and religious diversity of recent immigrants and refugees. Interfaith organizing models, especially with their inclusion of labor unions, offer an important balance in the ecology (...) of interfaith engagement and resistance to the civil religion of neoliberalism. Through these place-based, consociational collaborations, labor organizations help faith communities historicize, conceptualize, and navigate complex economic dynamics while expanding labor's value-frameworks, forms of organizing, and apprehension of worker's faith-filled identities. The dismantling of the welfare state across the United Kingdom (and indeed a number of other Western industrialised democracies, such as Canada and the United States) and the reductions to welfare provisions and entitlements are having a detrimental impact on women's equality and safety. Towers and Walby argue that the recent cuts to welfare provision in the United Kingdom, particularly for women's services, could lead to increased levels of violence for women and girls. This paper makes the argument that female (...) victims of domestic abuse experience violence on two levels: first, at the intimate/personal level through their relationship with an abuser and, second, at a structural level, through the state failing to provide adequate protection and provision for women who have experienced violence in intimate relationships. Using a specific example of post-violence community services delivered to both the children of women who have experienced domestic violence and the women themselves, this paper draws on empirical research carried out in 2010–2011 with London-based third-sector and public sector organisations delivering the Against Violence and Abuse Project ‘Community Group Programme’. We argue that the lack of services for women involved in, or exiting, a violent relationship can amount to state-sanctioned violence, if funding is withheld, or indeed, stretched to breaking point. This edited collection uses critical theory in order to understand the rise of the Alt-Right and the election of Donald Trump—and, in doing so, to assert the necessity and value of various disciplines within the humanities. While neoliberal mainstream culture has expressed shock at the seemingly expeditious rise of the Alt-Right movement and the outcome of the 2016 United States presidential election, a rich tradition of theory may not only explain the occurrence of this “phenomenon,” but may also chart an (...) alternative understanding of the movement, revealing the persistence of right-wing populism throughout the twentieth and twenty-first centuries. Though the humanities have seen themselves undervalued and under attack in recent years, the historical and cultural contextualization of the current moment via theory is a means of reaffirming the value of the humanities in teaching the ever-important and multifaceted skill of critical literacy. This book re-affirms the humanities, particularly the study of literature, theory, and philosophy, through questions such as how the humanities can help us understand the here and now. (shrink)

Chicanas and Mexican women share a history of colonialism that has (a) sustained oppressive constructions of gender roles and sexuality, (b) produced and reproduced them as racially inferior and as able to be silenced, conquered, and dominated physically and mentally, and (c) contributed to the exploitation of their labor. Given that colonialism has also come to shape the way young women of Mexican heritage learn in mainstream US schools, informal education from everyday women's conviviality and solidarity becomes a pivotal context (...) in which they can learn how to reconstitute colonial legacies. We examine how a group of Mexican working-class immigrant women at home and in a sweatshop fashion a girl named Ana, the main character in the popular film Real Women Have Curves, into a confident young woman who engages in what Pérez (1999, 2003) refers to as empowering and dynamic decolonial ways of seeing, knowing, doing, being, and reconstituting. In spite of its contradictions that, at times, assist in reproducing Ana as an oppressed laborer, it is their doing that helps produce her as a woman who engages in decolonial practices by facilitating her to deftly negotiate an oppressive economic and patriarchal space, a mainstream feminist space, and a space where their embodiments and creative cultural discourse, practices, and beliefs shine. As such, the film provides a powerful counterstory (Delgado 1989) that disrupts the chokehold of the logic of colonialism and how it seeks to classify, stereotype, and control young women like Ana. Este análisis permite comprender cuál es la configuración del sujeto nómade revisando las implicaciones que tiene asociar tal concepto con la búsqueda de aceptación y representación de esa diferencia en el contexto político. La lectura critica de Braidotti remite directamente a examinar los conceptos originales de Deleuze y Guattari referidos al nomadismo, como singularización, devenir y (...) subjetivación. ABSTRACT This article aims to describe the concept of nomadic subjectivity developed by Rosi Braidotti, while considering its structure with respect to sexual difference. This analysis allows us to understand the configuration of the nomadic subject by reviewing the implications of associating the concept with the search for acceptance and representation of the sexual difference in the political context. A critical reading of Braidotti means to examine the Deleuze and Guattari's original concepts about nomadism, such as singularization, becoming and subjectivation. (shrink)

Objective: Inhibitory control training has shown promise for improving health behaviours, however, less is known about its mediators of effectiveness. The current paper reports whether ICT reduces smoking-related outcomes such as craving and nicotine dependence, increases motivation to quit and whether reductions in smoking or craving are mediated by response inhibition or a devaluation of smoking stimuli.Method: Adult smokers were randomly allocated to receive 14 days of smoking-specific ICT or active control training. Participants were followed up to 3-months post-intervention. This (...) trial was preregistered.Results: There were no significant differences between ICT and active control training groups. Specifically, participants in both groups showed significant reductions in craving, nicotine dependence, motivation and a devaluation of smoking-stimuli up to 3-months follow-up compared to baseline. Inhibition and devaluation of smoking stimuli did not act as mediators. Devaluation of smoking stimuli was an independent predictor of smoking and craving at follow-up.Conclusion: Inhibitory control training was no more effective at reducing smoking-related outcomes compared to the active control group, however, significant improvements in craving, dependence indicators and evaluation of smoking stimuli were observed across both groups. A return to basic experimental research may be required to understand the most effective ICT approach to support smoking cessation. (shrink)

Much of the focus of the published 2011 symposium that inspired this work focused on the question, “When have you reduced risk enough to move from bench/animal studies to ‘first in-human’ studies?” Building applied research ethics related to nanotherapeutics requires bench and clinical scientists to have a clear vision about how to test nanotherapeutic safety, and it is clear that there is still much to be considered at the steps before “in-human” assessment. Herein, the perspective of the bench scientist is (...) brought to bear on using in vivo and in vitro models to assess the safety of nanotherapeutics. Much of this work falls under the purview of the field of nanotoxicology that aims to understand the toxicological impact of engineered nanoscale materials. Engineered nanomaterials include a wide variety of materials that are manipulated and controlled on the nanoscale level where, typically, the nanoparticle or nanomaterial has some dimension that is less than 100 nm. When Congress passed HIPAA, it did not intend to constrain public health's data sharing in the same way as clinical or payers. In fact, HIPAA recognizes data sharing with public health as a matter of national priority and shields this function from its reach. However, a health department may offer services that bring it within HIPAA's purview, such as running a Children's Health Insurance Program or a laboratory that bills electronically. When this is the case, HIPAA requires all information and (...) departments be subject to HIPAA unless the public health authority chooses to hybridize. Health departments might re-assess their coverage and elect to become a hybrid entity, thereby restricting HIPAA to only where required and removing barriers to information sharing with communities. En 2017, des spécialistes de différentes disciplines (santé, éducation et travail social) issus de cinq pays européens (France, Géorgie, Italie, Norvège et Suisse) ont créé un réseau afin de poursuivre leurs recherches sur le handicap et la migration. Le présent article expose les principaux résultats d’un premier workshop qu’ils ont réalisé et les pistes qui en découlent pour de futures recherches. D’une part, les enfants migrants en situation de handicap restent invisibles sur le plan statistique dans plusieurs pays. Leurs besoins (...) en tant que migrants et personnes en situation de handicap sont pris en compte par des politiques et des dispositifs distincts. D’autre part, dans les cinq pays, un écart important entre les normes légales et la situation des familles migrantes ayant un enfant en situation de handicap est constaté. Il en est de même en ce qui concerne la collaboration entre les différents services publics, ou entre ces services et les associations soutenant les personnes en situation de handicap, les personnes migrantes et/ou les minorités. Les futures recherches seront centrées sur les moyens d’augmenter la participation sociale des enfants migrants en situation de handicap et de leurs familles dans des contextes d’interventions interculturelles et interdisciplinaires. The endogenous opioid system has received attention and extensive research for its effects on reward, pleasure, and pain. However, relative to other neurochemicals, such as oxytocin, vasopressin and dopamine, the function of opioids in regulating human attachment, sociosexuality, and other aspects of human sociality has not received much consideration. For example, nonapeptides have been extensively studied in animals and humans for their possible roles in mother-offspring attachment, romantic attachment, fatherhood, and social cognition. Likewise, others have proposed models wherein oxytocin and (...) vasopressin are moderators of the relationship between steroid hormones and human social behaviors. Recently, opioids have generated renewed interest in relation to social pain, and importantly, the brain opioid hypothesis of social attachment, which suggests that endogenous opioids are a key implementer in primate and human bonding, has received some support. Here we focus on romantic bonds by proposing that endogenous opioids are an important mechanism mediating reproductive trade-offs through their inhibitory effects on testosterone production. The authors examine the claim that the most recent cohort of U.S. women is reluctant to identify as feminist although it has egalitarian gender attitudes. Using two national surveys, they show that the most recent generation is no less likely than prior cohorts to identify as feminist. However, Baby Bust women are less apt to identify as feminist than are older women, once background characteristics and attitudes related to feminist identification are controlled. Analyses suggest this reluctance is not due to (...) an aversion to feminism but reflects the “off” timing of the feminist movement in the lives of Baby Bust women. The authors also find important differences among cohorts in the correlates of feminist identity. Most notably, the relationships of political ideology and gender attitudes to feminist identity are stronger among Baby Boom women, who came of age during the feminist movement’s second wave, than among older and younger women. This article provides a spatial analysis of the types of microspaces, or Cultural Time Zones (CTZs), that constitute the narrative universes of chick lit from South Africa, China and India. I argue that although these books take place in ‘developing’ countries, their ‘First World’ spatial politics, and thus their political impact and cultural commentary, are both enabled and constrained by the settings of their narrative thrust. In the CTZ of the five-star hotel, or the elite spa, the books’ protagonists exemplify (...) a popular notion of choice/neoliberal feminism. What, then, do they say about the possibility of feminism for confronting patriarchal traditions in CTZs like the ‘local’ neighbourhood? The three books analysed – Angela Makholwa’s Black Widow Society, Wei Hui’s Shanghai Baby and Anita Jain’s Marrying Anita: A Quest for Love in the New India – are set in the globalising cities of Johannesburg, Shanghai and Delhi in putatively ‘developing’ countries; however, the books’ female protagonists tend to inhabit a series of ‘global’, ‘First World’ CTZs, like the high-end shopping mall, the Western music-playing nightclub or the cappuccino-serving cafe. These CTZs are geographically distant from those of their Western chick lit counterparts, yet nonetheless not very far away if measured in cultural kilometres. The neoliberal, so-called feminist ‘choices’ available to these protagonists would not be possible in geographically proximate but culturally distant, ‘traditional’ CTZs. The key question this article asks is whether these narratives’ spatial settings, confined as they are to upper middle-class, ‘global’, ‘modern’ CTZs, produce a form of spatial politics that fem-washes global capitalism while failing to confront patriarchal and traditional structures which dominate in more ‘local’, ‘traditional’ CTZs.
