Half of the 33.2 million people living with HIV today are women. Yet, responses to the epidemic are not adequately meeting the needs of women. This article critically evaluates how prevention of mother-to-child transmission (PMTCT) programs, the principal framework under which women's health is currently addressed in the global response to AIDS, have tended to focus on the prevention of HIV transmission from HIV-positive women to their infants. This paper concludes that more than ten years after their inception, PMTCT programs (...) still do not successfully ensure the adequate treatment, care and support of HIV-infected women. Of particular concern is the continued widespread use of single-dose nevirapine despite World Health Organization recommendations to employ more effective combination therapies that do not potentially jeopardize women's future treatment outcomes. In response, the article calls for a more comprehensive approach that places women's health needs at the centre of AIDS responses. This is critical in settings where the pandemic is generalized and there is a push to greatly expand PMTCT programs, as a more effective and equitable way of meeting the needs of women in the context of HIV. Without such a comprehensive approach, women will continue to be impacted disproportionately by the pandemic, and current strategies for prevention, including PMTCT, and treatment will not be as effective and responsive as they need to be. Moses Mendelssohn (1729-1786) was the premier Jewish thinker of his day and one of the best-known figures of the German Enlightenment, earning the sobriquet 'the Socrates of Berlin'. He was thoroughly involved in the central issue of Enlightenment religious thinking: the inevitable conflict between reason and revelation in an age contending with individual rights and religious toleration. He did not aspire to a comprehensive philosophy of Judaism, since he thought human reason was limited, but he did see Judaism as compatible (...) with toleration and rights. David Sorkin offers a close study of Mendelssohn's complete writings, treating the German, and the often-neglected Hebrew writings, as a single corpus and arguing that Mendelssohn's two spheres of endeavour were entirely consistent. German Jewish philosopher Moses Mendelssohn was one of the most influential thinkers of the Enlightenment. Until now, attention was focused on Mendelssohn’s German works—such as his groundbreaking _Jerusalem—_which have been duly translated into English. Edward Breuer and David Sorkin assert that his Hebrew works are essential for understanding both his biography and his oeuvre. This volume offers expertly translated and generously annotated selections from the entire corpus of Mendelssohn’s published Hebrew writings. Mendelssohn wrote in Hebrew throughout his life, but (...) these works—mainly grounded in biblical and other Hebrew classical works—have been hitherto inaccessible to most scholars. In this volume, Breuer and Sorkin make an important contribution to modern Jewish and religious thought, refuting the notion that Mendelssohn led a bifurcated intellectual and spiritual existence and demonstrating Mendelssohn’s ability to transform traditional religious genres into vehicles for philosophical argumentation. The report is dedicated to modern understanding of the correlation between science and religion that is based on the analysis of certain ideas formulated by Newton, Berkeley and Mach. Newton proceeded from the existence of infinite (absolute) Space that he interpreted as the Sensory of the intelligent omnipresent Being (God) who sees things themselves intimately, and throughly perceives and comprehends them. Human being also has his little “Sensoriums” perceiving the images of things, the Order and the Beauty of their arrangement. (...) Mach emphasized that since Newton’s period space and time have become “immaterial substances that form the most important basis of our sensual world outlook”. Apparently, this “immateriality of substances” manifests itself in the way Machinterprets our perceptions, conceptions, will, feelings, i.e. all inner and outer world, which he understands as small number of homogeneous elements called sensations (Empfindungen). These sensations are compared in the report to what Berkeley called ideas while he denied the existence of the real absolute noncreated space that is part, or attribute, of God. If we accept the idea that beside space and time inseparable from matter as it is scientifically comprehended, there exist absolute space and time as Newton interpreted them, then these space and time must exist outside our universe or parallel to it. This brings us to the panentheistic model (Eduard I. Sorkin, ХХIst World Congress of philosophy, Abstracts 2003, pp. 374‐375). According to Mach the law of causality is separated from space and time while the laws of nature are just limitations that our experience dictates to our expectations. The report shows that if the Mach’s concept had been supplemented by the “idealistic” views of Newton and Berkeley, it would have been more convincing – something contrary to fideism. (shrink)

Binding of a growth factor (GF) to its specific receptor on the cell surface causes the initiation of a signal transduction cascade which eventually results in mitosis. GF:receptor complexes are removed from the cell surface via receptor‐mediated endocytosis, a process which involves clathrin‐coated pits. After internalization into the endosomal compartment, a significant pool of GFs and GF receptors escape recycling to the cell surface and are sorted to the degradation pathway. The ligandinduced internalization and lysosomal degradation of GF receptors result (...) in the dramatic loss of surface receptors, a phenomenon termed receptor down‐regulation. In this review, we discuss relevant biochemical, morphological and kinetic studies of the mechanism of GF endocytosis, and the possible role of this process in mitogenic signaling by growth factor receptors. The strong sensorimotor account of perception gives self-induced movements two constitutive roles in explaining visual consciousness. The first says that self-induced movements are vehicles of visual awareness, and for this reason consciousness ‘does not happen in the brain only’. The second says that the phenomenal nature of visual experiences is consists in the action-directing content of vision. In response I suggest, first, that the sense in which visual awareness is active should be explained by appeal to the role of attention (...) in visual consciousness, rather than self-induced movements; and second, that the sense in which perceptual consciousness does not happen in the brain only should be explained by appeal to the relational nature of perceptual consciousness, appeal to which also shows why links with action cannot exhaust phenomenal content. These scientists have produced landmark studies on the dangers of DDT, tobacco smoke, acid rain, and global warming. But at the same time, a small yet potent subset of this community leads the world in vehement denial of these dangers. -/- Merchants of Doubt tells the story of how a loose-knit group of high-level scientists and (...) scientific advisers, with deep connections in politics and industry, ran effective campaigns to mislead the public and deny well-established scientific knowledge over four decades. Allan's monograph successfully takes on most of the (many) significant criticisms of the play, in particular those concerning its alleged disunity, and argues that the play is (...) more interesting than it has been taken to be. Allan sets out to show that the play merits more attention than it has received, and he succeeds, at least with this reviewer; a classicist reviewer who has written on the play is, however, hardly a test case. Moreover, the assertion of interest is not a strong claim, and, given the constraints of academic writing, scholarly arguments for a quirky play like the Andromache are often less compelling than the literary work itself. [End Page 126]Allan's thesis, that "every play represents a distinct thematic and dramatic complex which merits individual attention" (267), is undeniably true. He further asserts that the play is calculated to surprise and that the noticeable dissonances in it are purposeful as well. The book is divided into chapters on myth, structure, characterization, rhetoric, the spread of Attic tragedy, gender, the chorus, and the gods, in that order. Allan highlights Euripides' creative use of myth, drawing attention to the elevation of Andromache and the simultaneous way in which Euripides turns Neoptolemus, who was the center of epic treatments of the myth, into a corpse. There are connections between Allan's argument about the structure of the unexpected and Euripides' handling of myth. In part, Euripides builds the structure of surprise that Allan detects through his use of novel mythic variations.The elements of flexibility and surprise that Allan emphasizes also appear in a different guise in his considerations of gender (chap. 6) and the gods (chap. 8). According to Allan, female characters are useful for exposing the tensions that emerge when heroic society encounters crisis (162), while he uses Euripides' treatment of the gods to contribute to the overall argument about the multiplicity of Euripidean tragedy.Allan sounds a similar note in discussing characterization. Here he emphasizes dissonance, the contrast of traditional and invented story, and warns against expecting the depth of characterization typical of narrative from later centuries. This is a salutary reminder of the aesthetic conventions of tragedy too often overlooked by readers of the modern era.Characterization not only grows out of structure, as Allan argues, but is developed by rhetoric. His goal in the chapter on rhetoric is "to reappraise the functions and importance of Euripidean rhetoric (defined more broadly than the mere use of forensic topoi) by placing it within the larger dramatic context of the plays, and by viewing them within the wider rhetorical context of fifth-century Athenian culture" (119). Allan's contention is that Euripidean tragedy shows the failure of persuasion because of the intervention of political power. He argues that Euripides uses the Sophistic arsenal for his own dramatic ends, never for its own sake; as he notes, rhetoric is closely related to characterization through the characters' self-presentation. As he also shows, in the Andromache and other plays of Euripides, the rhetoric cannot be separated from the fictional world of the play. We model a black hole spacetime as a causal set and count, with a certain definition, the number of causal links crossing the horizon in proximity to a spacelike or null hypersurface Σ. We find that this number is proportional to the horizon's area on Σ, thus supporting the interpretation of the links as the “horizon atoms” that account for its entropy. The cases studied include not only equilibrium black holes but ones far from equilibrium.

It is commonly assumed that grounding relations are asymmetric. Here I develop and argue for a theory of metaphysical structure that takes grounding to be nonsymmetric rather than asymmetric. Even without infinite descending chains of dependence, it might be that every entity is grounded in some other entity. Having first addressed an immediate objection to the position under discussion, I introduce two examples of symmetric grounding. I give three arguments for the view that grounding is nonsymmetric (I call this view (...) ‘metaphysical interdependence’). These arguments are: (i) that metaphysical interdependence is the only theory able to reconcile competing intuitions about grounding; (ii) that it is the only theory consistent with both ‘gunk’ and ‘junk’; and (iii) that offers a satisfactory solution to the problem concerning whether or not grounding is itself grounded. The transient cellular organelles known as migrasomes, which form during cell migration along retraction fibers, have emerged as a crutial factor in various fundamental cellular processes and pathologies. These membrane vesicles originate from local membrane swellings, encapsulate specific cytoplasmic content, and are eventually released to the extracellular environment or taken up by recipient cells. Migrasome biogenesis entails a sequential membrane remodeling process involving a complex interplay between various molecular factors such as tetraspanin proteins, and mechanical properties like membrane tension and (...) bending rigidity. In this review, we summarize recent studies exploring the mechanism of migrasome formation. We emphasize how physical forces, together with molecular factors, shape migrasome biogenesis, and detail the involvement of migrasomes in various cellular processes and pathologies. A comprehensive understanding of the exact mechanism underlying migrasome formation and the identification of key molecules involved hold promise for advancing their therapeutic and diagnostic applications. Attempts to elucidate grounding are often made by connecting grounding to metaphysical explanation, but the notion of metaphysical explanation is itself opaque, and has received little attention in the literature. We can appeal to theories of explanation in the philosophy of science to give us a characterization of metaphysical explanation, but this reveals a tension between three theses: that grounding relations are objective and mind-independent; that there are pragmatic elements to metaphysical explanation; and that grounding and metaphysical explanation share a (...) close connection. Holding fixed the mind-independence of grounding, I show that neither horn of the resultant dilemma can be blunted. Consequently, we should reject the assumption that grounding relations are mind-independent. (shrink)

This paper develops an account of metaphysical explanation according to which metaphysical explanations are answers to what-makes-it-the-case-that questions. On this view, metaphysical explanations are not to be considered entirely objective, but are subject to epistemic constraints imposed by the context in which a relevant question is asked. The resultant account of metaphysical explanation is developed independently of any particular views about grounding. Toward the end of the paper an application of the view is proposed that takes metaphysical explanations conceived in (...) this way to characterize reality's structure. Verification and validation of numerical models of natural systems is impossible. This is because natural systems are never closed and because model results are always nonunique. Models can be confirmed by the demonstration of agreement between observation and prediction, but confirmation is inherently partial. Complete confirmation is logically precluded by the fallacy of affirming the consequent and by incomplete access to natural phenomena. Sometime around their first birthday most infants begin to engage in relatively sustained bouts of attending together with their caretakers to objects in their environment. By the age of 18 months, on most accounts, they are engaging in full-blown episodes of joint attention. As developmental psychologists (usually) use the term, for such joint attention to be in play, it is not sufficient that the infant and the adult are in fact attending to the same object, nor that the one’s attention (...) cause the other’s. The latter can and does happen much earlier, whenever the adult follows the baby’s gaze and homes in on the same object as the baby is attending to; or, from the age of six months, when babies begin to follow the gaze of an adult. We have the relevant sense of joint attention in play only when the fact that both child and adult are attending to the same object is, to use Sperber and Wilson’s (1986) phrase, ‘mutually manifest’. Psychologists sometimes speak of such jointness as a case of attention being ‘shared’ by infant and adult, or of a ‘meeting of minds’ between infant and adult, all phrases intended to capture the idea that when joint attention occurs everything about the fact that both subjects are attending to the same object is out in the open, manifest to both participants. Antirealism about metaphysical explanation is relatively underexplored. This paper maps out the territory for the antirealist, explaining what it would take to be an antirealist given various different conceptions of metaphysical explanation, and of the relationship between metaphysical explanation and grounding.

Socrates was not a moral philosopher. Instead he was a theorist who showed how human desire and human knowledge complement one another in the pursuit of human happiness. His theory allowed him to demonstrate that actions and objects have no value other than that which they derive from their employment by individuals who, inevitably, desire their own happiness and have the knowledge to use actions and objects as a means for its attainment. The result is a naturalised, practical, and demystified (...) account of good and bad, and right and wrong. Professor Reshotko presents a freshly envisioned Socratic theory residing at the intersection of the philosophy of mind and ethics. This paper explores a middle way between realism and eliminativism about grounding. Grounding-talk is intelligible and useful, but it fails to pick out grounding relations that exist or obtain in reality. Instead, grounding-talk allows us to convey facts about what metaphysically explains what, and about the worldly dependence relations that give rise to those explanations.

In this paper, I take seriously calls for public engagement in human genome editing decision-making by endorsing the convening of a “Citizens Jury” in conjunction with the International Commission on the Clinical Use of Human Germline Genome Editing’s next summit scheduled for March 6–8, 2023. This institutional modification promises a more inclusive, deliberative, and impactful form of engagement than standard bioethics engagement opportunities, such as comment periods, by serving both normative and political purposes in the quest to offer moral guidance (...) on gene editing. In examining evidence from the Australian Citizens’ Jury on Genome Editing convened in 2021, I argue that Citizens’ Juries should work in tandem with governing institutions to preserve the role of expertise while ensuring that the diverse views of the public are incorporated into their final reports as well. First, early inclusivity allows “the people” to hold agenda setting power through helping to set resource priorities. This also makes a downstream deliberative event, such as the called for Global Citizens’ Assembly, both more likely to occur and more influential on policy. Second, Jury’s diverse composition promises substantive contributions to the Commission’s work. Third, Citizens’ Juries could help to cultivate the Commission’s trustworthiness. (shrink)

Currently, value sensitive design (VSD) does not commit to a particular ethical theory. Critiques contend that without such an explicit commitment, VSD lacks a methodology for distinguishing genuine moral values from mere stakeholders-preferences and runs the risk of attending to a set of values that is unprincipled or unbounded. We argue that VSD practitioners need to complement it with an ethical theory. We argue in favour of a mid-level ethical theory to fulfil this role.

Grounding talk has become increasingly familiar in contemporary philosophical discussion. Most discussants of grounding think that grounding talk is useful, intelligible, and accurately describes metaphysical reality. Call themrealistsabout grounding. Some dissenters reject grounding talk on the grounds that it is unintelligible, or unmotivated. They would prefer to eliminate grounding talk from philosophy, so we can call themeliminitivistsabout grounding. This article presents the framework Capability Sensitive Design (CSD), which consists of merging the design methodology Value Sensitive Design (VSD) with Martha Nussbaum's capability theory. CSD aims to normatively assess technology design in general, and technology design for health and wellbeing in particular. Unique to CSD is its ability to account for human diversity and to counter (structural) injustices that manifest in technology design. The basic framework of CSD is demonstrated by applying it to the hypothetical design case of a (...) therapy chatbot for mental health. By applying CSD to a design case, the merits of this new framework over the standard VSD approach become apparent. This chapter argues that a central division among accounts of joint attention, both in philosophy and developmental psychology, turns on how they address two questions: What, if any, is the connection between the capacity to engage in joint attention triangles and the capacity to grasp the idea of objective truth? How do we explain the kind of openness or sharing of minds that occurs in joint attention? It does so by putting centre stage the notion of a ‘bipolar second person relation’, and examining ways of giving it substance suggested by the authors of these papers. In particular, it focuses on claims made in these papers about the existence and/or nature of second person (...) thought, second person reasons for action and second person reasons for belief and about possible connections among thought-theoretical, ethical and epistemological issues and debates in this area. Paul Hoyningen-Huene argues that what makes scientific knowledge special is its systematic character, and that this can be used to solve the demarcation problem. He labels this STDC: “Systematicity Theory’s Demarcation Criterion.” This paper argues that STDC fails, because there are areas of intellectual activity that are highly systematic, but that the great majority of scientists and historians and philosophers of science do not accept as scientific. These include homepathy, creationism, and climate change denial. I designate these activities “facsimile sciences” (...) because they mimic the appearance of science but are not, by the standards of philosophers and scientists, scientific. This suggests that we need additional criteria to demarcate science from non-science and/ or nonsense. (shrink)
