The current dominant perspective on addiction as a brain disease has been challenged recently by Marc Lewis, who argued that the brain-changes related to addiction are similar to everyday changes of the brain. From this alternative perspective, addictions are bad habits that can be broken, provided that people are motivated to change. In that case, autonomous choice or “free will” can overcome bad influences from genes and or environments and brain-changes related to addiction. Even though we concur with Lewis that (...) there are issues with the brain disease perspective, we also argue that pointing to black swans can be important, that is: there can be severe cases where addiction indeed tips over into the category of brain disease, but obviously that does not prove that every case of addiction falls into the disease category, that all swans are black. We argue that, for example, people suffering from Korsakoff’s syndrome, can be described as having a brain disease, often caused by alcohol addiction. Moreover, the brain changes occurring with addiction are related to choice-behaviour, habit formation and insight, hence essential mental abilities to break the addiction. We argue for a more graded perspective, where both black swans and white swans are rare, and most cases of addiction come as geese in different shades of gray. (shrink)

The doctor-patient relationship is built on an implicit covenant of trust, yet it was not until the post-World War Two era that respect for patient autonomy emerged as an article of mainstream medical ethics. Unlike their medical forebears, physicians today are expected to furnish patients with adequate information about diagnoses, prognoses and treatments. Against these dicta there has been ongoing debate over whether placebos pose a threat to patient autonomy. A key premise underlying medical ethics discussion is the notion that (...) the placebo effect necessitates patient deception. Indeed, the American Medical Association guidelines imply that placebo treatment necessary entails a form of deception. As a consequence of this assumption, the fulcrum of debate on the use of placebo treatment has hinged on whether that deception is ever justified. Recently performed experiments with open-label transparently prescribed placebos have begun to challenge the notion that deception is necessary in eliciting the placebo effect and such effects necessarily involve a binary distinction between autonomy and beneficence. In this article we focus on the content of disclosures in distinctive open-label, transparently disclosed placebo studies and inquire whether they might be said to invoke deception in clinical contexts, and if so, whether the deception is unethical. We find that open placebos may be said to involve equivocation over how placebos work. However, drawing on surveys of patient attitudes we suggest that this equivocation appears to be acceptable to patients. We conclude that open placebos fulfil current American Medical Association guidelines for placebo use, and propose future research directions for harnessing the placebo effect ethically. (shrink)

Abe and his friend Sol are out for a walk together in a part of town they haven't been in before. Passing a Christian church, they notice a curious sign in front that says "$1,000 to anyone who will convert." "I wonder what that's about," says Abe. "I think I'll go in and have a look. I'll be back in a minute; just wait for me." Sol sits on the sidewalk bench and waits patiently for nearly half an hour. Finally, (...) Abe reappears. "Well," asks Sol, "what are they up to? Who are they trying to convert? Why do they care? Did you get the $1,000?" Indignantly Abe replies, "Money. That's all you people care about." Ted Cohen thinks that's not a bad joke. But he also doesn't think it's an easy joke. For a listener or reader to laugh at Abe's conversion, a complicated set of conditions must be met. First, a listener has to recognize that Abe and Sol are Jewish names. Second, that listener has to be familiar with the widespread idea that Jews are more interested in money than anything else. And finally, the listener needs to know this information in advance of the joke, and without anyone telling him or her. Jokes, in short, are complicated transactions in which communities are forged, intimacy is offered, and otherwise offensive stereotypes and cliches lose their sting—at least sometimes. _Jokes_ is a book of jokes and a book about them. Cohen loves a good laugh, but as a philosopher, he is also interested in how jokes work, why they work, and when they don't. The delight at the end of a joke is the result of a complex set of conditions and processes, and Cohen takes us through these conditions in a philosophical exploration of humor. Hallucination: the perception of an impossible image. That which can never appear suddenly does so anyways - a private world that appears only to the eye of the one imagining it... until now. Ted Serios, psychic photographer, claimed he could project images directly from his mind onto photographic film. Under the sign of the psychic photograph, “Hallucinating Ted Serios” is a theorization of the dominant forms of uncertainty that persist in postmodern evaluations of representation, interpretation and identity. The central thesis (...) of this paper is that the imaginary and the real have collapsed into the spectral hallucination of one another, rendering impossible the rhetorical separation of hallucination from image. With the collapse of the boundary between the fictive and the real, the world of representation becomes intelligible only as an imaginary phenomenon; with the collapse between the object and the observer, interpretation is rendered deceptively magical; and, with the collapse between the self and its anonymous double, identity itself is relegated to a state of impossibility. Consequently, under the persisting signs of the imaginary, the magical and the impossible, “Hallucinating Ted Serios” asserts that in our contemporary world there no longer exists the possibility of failed performativity – psychic, imaginary or otherwise. Intellectualism about knowledge-how is the view that practical knowledge is a species of propositional knowledge. I argue that this view is undermined by a difference in properties between knowledge-how and both knowledge-that and knowledge-wh. More specifically, I argue that both knowledge-that and knowledge-wh are easily transmitted via testimony while knowledge-how is not easily transmitted by testimony. This points to a crucial difference in states of knowledge. I also consider Jason Stanley's attempt to subsume knowledge-how under an account of de se (...) knowledge. I argue that there are crucial differences between de se knowledge and knowledge-how. Thus, this paper advances both the discussion of intellectualism and the literature on the nature of de se knowledge. (shrink)

In the course of developing his arguments against making genetic enhancements to one's children, Habermas assumes that a clear line can be drawn between the natural and the manufactured. But given the current state of medical science, this is precisely what we can no longer take for granted.

We are often uncertain how to behave morally in complex situations. In this controversial study, Ted Lockhart contends that moral philosophy has failed to address how we make such moral decisions. Adapting decision theory to the task of decision-making under moral uncertainly, he proposes that we should not always act how we feel we ought to act, and that sometimes we should act against what we feel to be morally right. As is clear to any user of software, quality control of software has not reached the same levels of sophistication as it has with traditional manufacturing. In this paper we argue that this is because insufficient thought is being given to the methods of reasoning under uncertainty that are appropriate to this domain. We then describe how we have built a large-scale Bayesian network to overcome the difficulties that have so far been met in software quality control. This exploits a (...) number of recent advances in tool support for constructing large networks. We end the paper by describing how the network was validated and illustrate the range of reasoning styles that can be modelled with this tool. (shrink)

Health worker shortages in many countries are reaching crisis levels, exacerbated by factors associated with the COVID-19 pandemic. In New Zealand, the medical specialists union has called for a health workforce emergency to be declared, yet at the same time, many foreign-trained healthcare workers are unable to stay in the country or unable to work. While their health systems differ, countries such as New Zealand, the USA and the UK at least partially rely on international medical graduates (IMGs) to ensure (...) access to health services, particularly in underserved communities. This paper focuses on the challenges faced by many IMGs, particularly those that constrain their capacity to live and work in the countries that rely on their skills. These challenges give rise to two ethical problems. First, they represent a failure of reciprocity towards IMGs; second, they represent a source of harm, both to IMGs themselves and to communities whose healthcare service depends on contributions made by IMGs. We argue that brain waste and disvaluing the contributions of IMGs and other foreign-trained health professionals have significant costs for maintaining a functional healthcare system, costs which must be adequately recognised and counted. The time is ripe for a greater interrogation of assumptions and commitments underlying an emerging common ground on the ethics of animal research as well on the 3 R (replacement, refinement, reduction) approach that parallels, and perhaps even further shapes, it. Recurring pressures to re-evaluate the moral status of some animals in research comes as much from within the relevant sciences as without. It seems incredible, in the light of what we now know of such animals as chimpanzees, to deny (...) that these animals are properly accorded high moral status. Barring the requirement that they be human, it is difficult to see what more animals such as chimpanzees would have to possess to acquire it. If the grounds for ascribing high moral status are to be non-arbitrary and responsive to our best knowledge of those individuals who possess the relevant features, we should expect that a sound ethical experimental science will periodically reassess the moral status of their research subjects as the relevant knowledge demands. We already can observe this reassessment as scientists committed to humane experimental science incorporate discoveries of enrichment tools and techniques into their housing and use of captive research animals. No less should this reassessment include a critical reflection on the possible elevation of moral status of certain research animals in light of what is discovered regarding their morally significant properties, characteristics or capacities, or so I will argue. To do anything short of this threatens the social and moral legitimacy of animal research. (shrink)

Among the "hard cases" of captive animal research is the continued use of chimpanzees in harmful experimental science. In a recent article I contend that contemporary animal welfare science and chimpanzee behavioral studies permit, if not require, a reappraisal of the moral significance of chimpanzee dissent from participation in certain experiments. In what follows, I outline my earlier argument, provide a brief survey of some central concepts in pediatric research ethics, and use these to enrich an understanding of chimpanzee dissent (...) useful for research ethics. Locked-in syndrome (LIS) is a severe neurological condition that typically leaves a patient unable to move, talk and, in many cases, initiate communication. Brain Computer Interfaces (or BCIs) promise to enable individuals with conditions like LIS to re-engage with their physical and social worlds. In this paper we will use extended mind theory to offer a way of seeing the potential of BCIs when attached to, or implanted in, individuals with LIS. In particular, we will contend that functionally integrated BCIs (...) extend the minds of individuals with LIS beyond their bodies, allowing them greater autonomy than they can typically hope for in living with their condition. This raises important philosophical questions about the implications of BCI technology, particularly the potential to change selves, and ethical questions about whether society has a responsibility to aid these individuals in re-engaging with their physical and social worlds. It also raises some important questions about when these interventions should be offered to individuals with LIS and respecting the rights of these individuals to refuse intervention. By aiding willing individuals in re-engaging with their physical and social worlds, BCIs open up avenues of opportunity taken for granted by able individuals and introduce new ways in which these individuals can be harmed. These latter considerations serve to highlight our emergent social responsibilities to those individuals who will be suitable for, and receive, BCIs. (shrink)

“Neurodiversity” is associated with the struggle for the civil rights of all those diagnosed with neurological or neurodevelopmental disorders. Two basic approaches in the struggle for what might be described as “neuro-equality” are taken up in the literature: There is a challenge to current nosology that pathologizes all of the phenotypes associated with neurological or neurodevelopmental disorders ); there is a challenge to those extant social institutions that either expressly or inadvertently model a social hierarchy where the interests or needs (...) of individuals are ranked relative to what is regarded as properly functioning cognitive capacities. In this paper, we explore some of the reasons justifying which make it an important tool for achieving greater neuro-equality, while still recognizing its limitations for achieving this goal. Particularly, we explore how an appeal to functionality and neurological diversity can support a re-seeing of at least certain forms of ASD. (shrink)

The legacy of the Frankfurt Institute for Social Research has been a powerful force for critically understanding social reality. Erich Fromm was one of the early and best known members of the Institute. Fromm emphasised the centrality of culture and interpersonal relations in the contruction of the psyche. The unconscious was not only the location for buried repressed matter but also for the imaginative potential of the human person. He is a forgotten and neglected contributor to the story of the (...) Institute having been written out of this history. This retrospective of his ideas explores his work in the light of the recent work of Jürgen Habermas who is also an active but less controversial engager with the psychoanalytic tradition. The implications for adult education will be addressed. The paper outlines Fromm’s radical reinterpretation of psychoanalysis emphasising the importance of social existence as distinct from the impact of instincts; key concepts of the market, commodity fetishism and automaton conformity; The implications for adult education in the tradition of radical (Freire) and transformative learning theory (Mezirow) and addressed and make connections between Habermas and Fromm that further the project of critical theory. Both attempted in different times to identify and realise the potential of (though neither used the term) of lifelong learning as part of the process of bringing about a more just and caring society and a shared attention to the importance of having free conversations about how the emancipated life might be created and sustained. In this book, Ted Peters explores the fallacies of the "gene myth" and presents a resounding array of arguments against this kind of all-encompassing genetic determinism. On the scientific side, he correctly points out that genetic influences on behavior are in most instances relatively modest. Does anyone deny that identical twins are still able to practice individual free will? After dispatching some of the sweepingly deterministic conclusions of the "science" of evolutionary psychology with a particularly effective set of rebuttals, Peters (...) arrives at the conclusion that the nature of humanness is an interaction of three things- not one, not two, but three: genetics, environment, and free will. Neglecting any one of these three leads down a path of fuzzy thinking and dangerous consequences. (shrink)

Paul Tillich's eternal now is the ground from which all things emerge and perish in each and every moment. A Tillichean eschatology involves the gathering of all things finite into the eternity of the present moment, into God. Salvation is present moment. But is the “eternal now” enough? This essay offers biblical and theological critiques of Tillich's present eschatology and posits an eschatology that combines Tillich's “eternal now” with Wolfhart Pannenberg's “end‐oriented eschatology.” The result is an eschatology that recognizes the (...) eternal now in which all things (including all time) belong to God yet with an eye toward the God‐given possibilities of the next moment, the future. The end of being is not cessation; rather, it is the fulfillment of time, the consummation of all things. (shrink)

Public policy debates such as we find in the Untied Nations, the Singapore Bioethics Advisory Committee, and the US President’s Council on Bioethics reflect behind-the-scenes theological debates. Although religious spokespersons agree nearly universally that human reproductive cloning should be banned; moral ambivalence rises when confronting human embryonic stem cell research. Rather than focus on beneficence (medical benefits), religious bioethicists focus on nonmalificence (embryo protection). The Vatican claim that stem cell research should be banned because it destroys embryos appears at first (...) to rely upon ensoulment at conception; but a closer analysis shows that the Vatican position relies upon genetic uniqueness. Appeal to genetic uniqueness is inadequate for Christian anthropology; what needs to be added is a relational and proleptic understanding of human dignity. (shrink)

Briefly summarized here are some reasons why the DNA/genetic model of life appears to be inadequate (for more details see previous OJPP paper). Given its foundational role with regards to our understanding of life and its challenges, this is significant. The traditional dualistic reincarnation model is introduced. That model is then considered, along with the DNA model, as potential explanations for our natural religious instincts. Next, the two models are considered as possible bases for the phenomenon of kin orientation or (...) bias. Together the case is made that a soul-based reincarnation model offers some explanatory advantages therein over the genetic-based (material-only) model. Finally, and of potential practical and philosophical significance, the corresponding imports for life and meaning are considered. (shrink)

Contemporary television has been marked by such exceptional programming that it is now common to hear claims that TV has finally become an art. In Appreciating the Art of Television, Nannicelli contends that televisual art is not a recent development, but has in fact existed for a long time. Yet despite the flourishing of two relevant academic subfields—the philosophy of film and television aesthetics—there is little scholarship on television, in general, as an art form. This book aims to provide scholars (...) active in television aesthetics with a critical overview of the relevant philosophical literature, while also giving philosophers of film a particular account of the art of television that will hopefully spur further interest and debate. In Thinking of Others, Ted Cohen argues that the ability to imagine oneself as another person is an indispensable human capacity--as essential to moral awareness as it is to literary appreciation--and that this talent for identification is the same as the talent for metaphor. To be able to see oneself as someone else, whether the someone else is a real person or a fictional character, is to exercise the ability to deal with metaphor and other figurative language. The underlying faculty, (...) Cohen argues, is the same--simply the ability to think of one thing as another when it plainly is not. In an engaging style, Cohen explores this idea by examining various occasions for identifying with others, including reading fiction, enjoying sports, making moral arguments, estimating one's future self, and imagining how one appears to others. Using many literary examples, Cohen argues that we can engage with fictional characters just as intensely as we do with real people, and he looks at some of the ways literature itself takes up the question of interpersonal identification and understanding. To what extent, and on what grounds, can we ethically evaluate art from a generative context that is at some significant distance from our present reception context—at enough distance, at least, so that the two contexts differ, in important ways, in aspects of their moral outlooks? This paper has four aims. The modest task of the paper is to show that this question is much more difficult than has been recognised. The somewhat more ambitious goal is a methodological intervention: it (...) is to highlight the need for interdisciplinary research that illuminates the socio-historical specificities of remote generative contexts in a way that allows us to see the relevant ways in which the moral outlooks overlap with ours (or do not). In relation, the third aim of the paper is to argue, with reference to Bernard Williams’s ‘relativism of distance’, that there are some cases in which the moral outlooks of a work’s generative and reception contexts differ so significantly that the warrant for our moral judgments of remote artworks is attenuated—if it exists at all. Finally, we try to demonstrate how the sort of interdisciplinary collaboration we’re advocating might inform our moral judgments of art—more specifically, in a way that invites a degree of quietude or uncertainty about remote artworks that are deeply morally troubling in our present reception context. Ingmar Persson and Julian Savulescu argue that non-traditional forms of cognitive enhancement (those involving genetic engineering or pharmaceuticals) present a serious threat to humanity, since the fruits of such enhancement, accelerated scientific progress, will give the morally corrupt minority of humanity new and more effective ways to cause great harm. And yet it is scientific progress, accelerated by non-traditional cognitive enhancement, which could allow us to dramatically morally enhance human beings, thereby eliminating, or at least reducing, the threat from the (...) morally corrupt minority. I argue that this apparently intractable dilemma is less difficult to resolve than Persson and Savulescu suppose. Their analysis of non-traditional cognitive enhancement overstates the risks and undervalues the benefits that such enhancement might provide. Once the benefits are better described, it is clear that non-traditional cognitive enhancement could be the means of our survival, not of our destruction. (shrink)
