The use of a placebo arm in clinical trials is unethical if there is an active comparator that is acceptably safe and effective. We argue that reasonable evidence of effectiveness and safety of an inexpensive alternative to an expensive therapy is sufficient to require that the inexpensive therapy be included as a comparator when the expensive therapy is tested, and that use of an inactive placebo comparator only is not ethical. For example, studies of the expensive drug eplerenone for congestive (...) heart failure have not included a spironolactone arm, although there is reasonable evidence that spironolactone would be safe and effective, and spironolactone is inexpensive. The requirement to study inexpensive therapies is based on avoidance of unnecessary cost in medical care as an example of non-maleficence. Several ethical actors in the design, conduct, and publication of clinical trials and their results bear responsibility for the appropriate conduct of clinical trials. That responsibility includes protecting study subjects from being asked to participate in clinical trials that serve primarily to promote the use of new and expensive therapies. (shrink)

In the German academies of Nietzsche's period of writing, the Kantian tradition was largely displaced in favor of two independent schools that have since been labeled "Neo-Kantianism." This paper presents four key theses about philosophy of history from four Neo-Kantian thinkers, how they follow from their adaptation of the Kantian tradition, and how Nietzsche critically engaged the very same issues in the formation of his own historical theory. [Richard Glauser] Shaftesbury's theory of aesthetic experience is based on his conception of a natural disposition to apprehend beauty, a real 'form' of things. I examine the implications of the disposition's naturalness. I argue that the disposition is not an extra faculty or a sixth sense, and attempt to situate Shaftesbury's position on this issue between those of Locke and Hutcheson. I argue that the natural disposition is to be perfected in many different ways in order to be exercised in (...) the perception of the different degrees of beauty within Shaftesbury's hierarchy. This leads to the conclusion that the exercise of the disposition depends, from case to case, on many different cognitive and affective conditions, that are realised by the collaborative functionings of our ordinary faculties. Essential to Shaftesbury's conception of aesthetic experience is a disinterested, contemplative love, that causes (or contains) what we may call a 'disinterested pleasure', but also an interested pleasure. I argue that, within any given aesthetic experience, the role of the disinterested pleasure is secondary to that of the disinterested love. However, an important function of the disinterested pleasure is that, in combination with the interested pleasure, it leads one to aspire to pass from the aesthetic experience of lower degrees of beauty to the experience of higher ones in the hierarchy. /// [Anthony Savile] (1) If Shaftesbury is to be seen as the doyen of modern aesthetics, his most valuable legacy to us may not so much be his viewing aesthetic response as a sui generis disinterested delight as his insistence on its turning 'wholly on [experience of] what is exterior and foreign to ourselves'. Not that we cannot experience ourselves, or what is our own, as a source of such admiration. Rather our responses, favourable or no, are improperly grounded in any essentially reflexive, or first-personal, ways of taking what engages us. The suggestion is tested against the case of Narcissus. (2) Glauser interestingly emphasizes Shaftesbury's neo-Platonic conception of a hierarchy of aesthetic experience that culminates in the joyful contemplation of God. That hierarchy must be something that is less unitary and systematic than Shaftesbury himself had supposed, even when his emphasis on the tie between aesthetic pleasure and contemplative experience is allowed to extend beyond perception and to encompass episodes of thought itself. Without the Spirit, therefore, the church would not be the living Body of Christ; it (...) would rather be a corpse, which, left to its own devices, could only putrefy and decay. Hence, the church is the workshop of the Holy Spirit, and through his graciousness, the church and its members are blessed with myriads of pneumatic enablement that have been called charisms. Charisms 'are the manifold ways in which the graciousness of God is manifested in the lives of individual Christians, especially by making them effective instruments of grace to others in the body of Christ'. Anthony Everett gives a philosophical defence of the common-sense view that there are no such things as fictional people, places, and things. He argues that our talk and thought about such fictional objects takes place within the scope of a pretense, and that we gain little but lose much by accepting fictional realism.

Contemporary biologists are not agreed on the question of whether there are any human races, despite the widespread scientific consensus on the underlying genetics. For most purposes, however, we can reasonably treat this issue as terminological. What most people in most cultures ordinarily believe about the significance of “racial” difference is quite remote, I think, from what the biologists are agreed on. Every reputable biologist will agree that human genetic variability between the populations of Africa or Europe or Asia is (...) not much greater than that within those populations; though how much greater depends, in part, on the measure of genetic variability the biologist chooses. If biologists want to make interracial difference seem relatively large, they can say that “the proportion of genic variation attributable to racial differences is … 9 – 11%.”1 If they want to make it seem small, they can say that, for two people who are both Caucasoid, the chances of difference in genetic constitution at one site on a given chromosome are currently estimated at about 14.3 percent, while for any two people taken at random from the human population, they are estimated at about 14.8 percent. The statistical facts about the distribution of variant characteristics in human populations and subpopulations are the same, whichever way the matter is expressed. Apart from the visible morphological characteristics of skin, hair, and bone, by which we are inclined to assign people to the broadest racial categories—black, white, yellow—there are few genetic characteristics to be found in the population of England that are not found in similar proportions in Zaire or in China; and few too which are found in Zaire but not in similar proportions in China or in England. All this, I repeat, is part of the consensus . A more familiar part of the consensus is that the differences between peoples in language, moral affections, aesthetic attitudes, or political ideology—those differences which most deeply affect us in our dealings with each other—are not biologically determined to any significant degree.[…]In this essay, I want to discuss the way in which W. E. B. Du Bois—who called his life story the “autobiography of a race concept”—came gradually, though never completely, to assimilate the unbiological nature of races. I have made these few prefatory remarks partly because it is my experience that the biological evidence about race is not sufficiently known and appreciated but also because they are important in discussing Du Bois. Throughout his life, Du Bois was concerned not just with the meaning of race but with the truth about it. We are more inclined at present, however, not to express our understanding of the intellectual development of people and cultures as a movement toward the truth; I shall sketch some of the reasons for this at the end of the essay. I will begin, therefore, by saying what I think the rough truth is about race, because, against the stream, I am disposed to argue that this struggle toward the truth is exactly what we find in the life of Du Bois, who can claim, in my view, to have thought longer, more engagedly, and more publicly about race than any other social theorist of our century. 1. The aesthetic domain is a social one. We coordinate our individual acts of creation, appreciation, and performance with those of others in the context of social aesthetic practices. More strongly, many of the richest goods of our aesthetic lives are constitutively social; their value lies in the fact that individuals are engaged in joint aesthetic agency, participating in cooperative and collaborative project that outstrips what can be realized alone. I provide an account of nature and value of two such social (...) aesthetic goods—aesthetic communities and aesthetic traditions—and further argue that such goods are a core constituent of an aesthetically good life. At the same time, I argue that the ideals of the dominant theory of aesthetic value, aesthetic value hedonism, are incompatible with a full commitment to these social aesthetic goods; the hedonist is thereby alienated from the other participants within aesthetic communities and traditions. This sets up a dilemma for the hedonist: either the hedonist must bite the bullet, accepting that the theory leads to a problematic form of aesthetic alienation; or, we must reject aesthetic value hedonism and adopt a different theory of aesthetic value which accommodates the value of social aesthetic goods. Gittins, Anthony J It is impossible for anyone to feel the pain you feel; the most people can do is to sympathise or empathise. But because there is nothing new under the sun, all of us can at least try to 'suffer with' the sufferings of others. Our experience of the all too human failings of the church today is by no means unique: ever since the beginning, times of trauma and crisis have alternated with times of peace and (...) healing. Our challenge is to keep hope alive during times of crisis, and to learn from the past in order learn its lessons and not repeat its mistakes. In this final reflection, I offer some reasons for hope and some perspectives for the next stage of our lives if it is to be marked by faithfulness and action rather than fear and reaction. (shrink)

Sir Anthony Kenny presents a second edition of his landmark work The Aristotelian Ethics, which transformed Aristotle studies in 1978 by showing, on stylistic, historical, and philosophical grounds, that the Eudemian Ethics was a mature work with as strong a claim to be Aristotle's ethical masterpiece as the more widely studied Nicomachean Ethics. In this new edition Kenny offers a critical survey of developments in the field since The Aristotelian Ethics was first published. Kenny also addresses the criticisms of (...) his first edition, both accepting those he sees as justified and addressing and refuting those which he feels are unfounded. The book remains essential reading for anyone interested in Aristotle's ethical works, arguably the most influential ever written. (shrink)

Resolving to finish reading a novel, staying true to your punk style, or dedicating your life to an artistic project: these are examples of aesthetic commitments. I develop an account of the nature of such commitments, and I argue that they are significant insofar as they help us manage the temporally extended nature of our aesthetic agency and our relationships with aesthetic objects. At the same time, focusing on aesthetic commitments can give us a better grasp on the nature of (...) aesthetic normativity; this is because, in making aesthetic commitments, we are capable of giving aesthetic concerns the weight of obligation. I argue that appealing to aesthetic commitments allows us to account for the existence of aesthetic obligations as well as their grounding. I conclude by arguing that, although the aesthetic domain is a domain of play and freedom of choice, there is nevertheless an important place in it for both aesthetic commitments and the aesthetic obligations they generate. New technologies regularly bring about profound changes in our daily lives. Romantic relationships are no exception to these transformations. Some philosophers expect the emergence in the near future of love drugs: a theoretically achievable biotechnological intervention that could be designed to strengthen and maintain love in romantic relationships. We investigated laypeople’s resistance to the use of such technologies and its sources. Across two studies (Study 1, French and Peruvian university students, N after exclusion = 186; Study 2, Amazon Mechanical Turk (...) sample, N after exclusion = 693, pre-registered), we found that the use of love drugs designed to strengthen and maintain love in romantic relationships are considered as more morally problematic than psychological therapy with the same aim. In Study 2, we show that this last effect is partially due to the fact that the love resulting from the use of love drugs is perceived as less authentic, intense, and durable. We discuss the specific role of authenticity in the relative moral disapproval of love drugs. (shrink)

This chapter provides an overview of how existential phenomenology has influenced qualitative research methods across a range of disciplines across the social, health, educational, and psychological sciences. It focuses specifically on how the concepts of “existential structures,” or “existentials”—such as selfhood, temporality, spatiality, affectivity, and embodiment—have been used in qualitative research. After providing a brief introduction to what qualitative research is and why philosophers should be interested in it, the chapter provides clear, straightforward examples of how qualitative researchers have used (...) existentials to analyze qualitative data and design qualitative studies. The chapter is intended to introduce philosophers to existential approaches to phenomenological qualitative research, with the aim of facilitating interdisciplinary dialogue and collaboration. Augustine converted to Christianity in 386 and described his conversion in his Confessiones a decade later. Much ink has been spilled concerning the question of the specific nature of Augustine’s conversion and the ‘historical accuracy’ of his description thereof 10 years later. The Confessiones seem to describe a volte face: he radically embraced Christianity. But to what kind of Christianity did he convert? We will readdress this question, not by investigating his conversion, but by a close exegesis of Confessiones 9, (...) in which he describes the so-called Ostia-ecstasy. In the first part of our article, we discuss the description of the latter experience in Confessiones 9 and situate it within his conversion narrative, with attention to the symbolical language he used. The second part compares the Ostia-experience with the ascent of the soul as described in Plotinus’ Enneads, and with similar ‘Neoplatonic ascents’ present in the Confessiones and in writings penned by Augustine prior to or simultaneous with the Confessiones in order to elucidate the relationship between Augustine’s ‘Christian’ conversion and ‘Ostia-ecstasy’ with the Neoplatonic anabasis. Empathy is a topic of continuous debate in the nursing literature. Many argue that empathy is indispensable to effective nursing practice. Yet others argue that nurses should rather rely on sympathy, compassion, or consolation. However, a more troubling disagreement underlies these debates: There’s no consensus on how to define empathy. This lack of consensus is the primary obstacle to a constructive debate over the role and import of empathy in nursing practice. The solution to this problem seems obvious: Nurses need (...) to reach a consensus on the meaning and definition of empathy. But this is easier said than done. Concept analyses, for instance, reveal a profound ambiguity and heterogeneity of the concept of empathy across the nursing literature. Since the term “empathy” is used to refer to a range of perceptual, cognitive, emotional, and behavioral phenomena, the presence of a conceptual ambiguity and heterogeneity is hardly surprising. Our proposal is simple. To move forward, we need to return to the basics. We should develop the concept from the ground up. That is, we should begin by identifying and describing the most fundamental form of empathic experience. Once we identify the most fundamental form of empathy, we will be able to distinguish among the more derivative experiences and behaviors that are addressed by the same name and, ideally, determine the place of these phenomena in the field of nursing. The aim of this article is, consequently, to lay the groundwork for a more coherent concept of empathy and thereby for a more fruitful debate over the role of empathy in nursing. In Part 1, we outline the history of the concept of empathy within nursing, explain why nurses are sometimes warry of adapting concepts from other disciplines, and argue that nurses should distinguish between adapting concepts from applied disciplines and from more theoretical disciplines. In Part 2, we show that the distinction between emotional and cognitive empathy—borrowed from theoretical psychology—has been a major factor in nurses’ negative attitudes toward emotional empathy. We argue, however, that both concepts fail to capture the most fundamental form of empathy. In Part 3, we draw on and present some of the seminal studies of empathy that can be found in the work of phenomenological philosophers including Max Scheler, Edmund Husserl, and Edith Stein. In Part 4, we outline how their understanding of empathy may facilitate current debates about empathy’s role in nursing. (shrink)

Radical embodied cognitive science is split into two camps: the ecological approach and the enactive approach. We propose that these two approaches can be brought together into a productive synthesis. The key is to recognize that the two approaches are pursuing different but complementary types of explanation. Both approaches seek to explain behavior in terms of the animal–environment relation, but they start at opposite ends. Ecological psychologists pursue an ontological strategy. They begin by describing the habitat of the species, and (...) use this to explain how action possibilities are constrained for individual actors. Enactivists, meanwhile, pursue an epistemic strategy: start by characterizing the exploratory, self-regulating behavior of the individual organism, and use this to understand how that organism brings forth its animal-specific umwelt. Both types of explanation are necessary: the ontological strategy explains how structure in the environment constrains how the world can appear to an individual, while the epistemic strategy explains how the world can appear differently to different members of the same species, relative to their skills, abilities, and histories. Making the distinction between species habitat and animal-specific umwelt allows us to understand the environment in realist terms while acknowledging that individual living organisms are phenomenal beings. Few issues cause academics to disagree more than gender and race, especially when topics are addressed in terms of biological differences. To conduct research in these areas or comment favorably on research can subject one to scorn. When these topics are addressed, they generally take the form of philosophical debates. Anthony Walsh focuses upon such debates and supporting research. He divides parties into biologists and social constructionists, arguing that biologists remain focused on laboratory work, while constructionists are acutely aware (...) of the impact of biologists in contested territories. "Science Wars" introduces the ideas motivating the parties and examines social constructionism and its issues with science. He explores arguments over conceptual tools scientists love and constructionists abhor, and he provides a solid discussion of the co-evolution of genes and culture. Walsh then focuses his attention on gender, how constructionists view it, and the neuroscience explanation of gender differences. Moving to race, Walsh looks at how some have tried to bury the concept of race, while others emphasize it. He considers definitions of race--essentialist, taxonomic, population, and lineage--as they have evolved from the time of the Enlightenment to the present. And finally, he attempts to bring the opposing sides together by pointing out what each can bring to a meaningful discussion. (shrink)

Both proponents and critics of the desire theory of welfare have narrowly focused on the positive side of the theory while virtually ignoring its negative side. On the positive side, the desire theorist says that getting what you want is good for you. But what should the desire theorist say is bad for you? A common and plausible-sounding answer is that if getting what you want is good for you, then surely not getting what you want is bad for you. (...) I argue that this answer is a mistake that would commit desire theorists to a claim that is implausible by their own lights, namely that a state of affairs could be bad for you even if you were not at all bothered by it. The desire theorist should instead say that getting what you are averse to is bad for you. If desire satisfaction is good for you, then aversion satisfaction, not desire frustration, is bad for you. A review of Anthony Appiah's Experiments in Ethics.

According to internalism about prudential value, the token states of affairs that are basically good for you must be suitably connected, under the proper conditions, to your positive attitudes. It is commonly thought that any theory of welfare that implies internalism is guaranteed to respect the alienation constraint, the doctrine that you cannot be alienated from that which is basically good for you. The assumption is that because internalism requires a necessary connection between a subject’s positive attitudes and each state (...) of basic benefit to that subject, theories that imply internalism will only postulate goods that resonate with the subject. In this paper, I show that extant formulations of internalism do not have this desirable feature. The central defect of traditional formulations is that they do not respect an important but overlooked truth about alienation: namely, that even if a state of affairs is suitably connected to your positive attitudes, your negative attitudes can nonetheless render you alienated from it. By taking into account the relevance of the negative attitudes, I propose the new internalism—the view that x is basically good for you only if you have a net positive attitude towards it—as a way to advance our thinking about what is required to avoid alienating theories of welfare. It was this essential insight, along with the methodological prioritisation of a phenomenology of cognition and the recognition of the epistemological distinctiveness of judgment, that a young Bernard Lonergan gleaned from his study of John Henry Newman's Grammar of Assent. Given that the ‘later’, post‐Insight (1953) Lonergan enacted a more explicit transposition of his thought into a hermeneutical and existential framework, one might be tempted to assume that this coincided with a drift (...) away from his tutelage under the nineteenth‐century Englishman. Indeed, an examination of the secondary literature detailing their relationship would suggest as much. Yet, in the hope of contributing to the regrettably sparse Newman‐Lonergan scholarship and proposing a modest recalibration therein, I argue that the more existential, hermeneutical, and committed to the philosophical turn to concrete socio‐historical subjectivity Lonergan grew, the more fruit his early Newmanian formation bore. By analysing Newman's proto‐Lonerganian anticipations in the areas of self‐appropriation, conversion, the relationship of subjectivity to objectivity, and the hermeneutical nature of consciousness, I will contend that Newman—a presciently continental mind writing as one untimely born into an analytical milieu—was the wellspring from which Lonergan never ceased to draw. This paper defends and develops an argument against epiphenomenalism, broadly construed. I argue first for a definition of epiphenomenalism which includes ‘non-reductive’ materialism as well as classical dualistic epiphenomenalism. I then present an argument that if epiphenomenalism were true it would be impossible to know about or even refer to our conscious states -- and therefore impossible even to formulate epiphenomenalism. David Chalmers has defended epiphenomenalism against such arguments; I consider this defence and attempt to show that it fails. I (...) conclude that an adequate account of mental causation requires us to abandon the principle of the causal closure of the physical, and attempt to rebut charges that it would be ‘unscientific’ to do so. Judgment subjectivism is the view that x is good for S if and only if, because, and to the extent that S believes, under the proper conditions, that x is good for S. In this paper, I offer three related arguments against the theory. The arguments are about what judgment subjectivism implies about the well-being of welfare nihilists, people who believe there are no welfare properties, or at least that none are instantiated. I maintain that welfare nihilists can be benefited (...) and harmed. Judgment subjectivism is implausible because it implies otherwise. (shrink)
