Globally, chronic disease and conditions such as diabetes, cardiovascular disease, depression and cancer are the leading causes of morbidity and mortality. Why, then, are public health efforts and programs aimed at preventing chronic disease so difficult to implement and maintain? Also, why is primary care—the key medical specialty for helping persons with chronic disease manage their illnesses—in decline? Public health suffers from its often being socially controversial, personally intrusive, irritating to many powerful corporate interests, and structurally designed to be largely (...) invisible and, as a result, taken for granted. Primary care struggles from low reimbursements, relative to specialists, excessive paperwork and time demands that are unattractive to medical students. Our paper concludes with a discussion of why the need for more aggressive public health and redesigned primary care is great, will grow substantially in the near future, and yet will continue to struggle with funding and public popularity. Evidence shows that changes in the architecture, design, and decor of health care facilities can improve patient care and in the long run reduce expenses. These essays detail the state of the research, look inside two hospitals that put some of these innovations into practice, and consider how design fits into the moral mission ofhealth care.

Various social animal species have been noted to inhibit aggressive attacks when a conspecific displays submission cues. Blair (1993) has suggested that humans possess a functionally similar mechanism which mediates the suppression of aggression in the context of distress cues. He has suggested that this mechanism is a prerequisite for the development of the moral/conventional distinction; the consistently observed distinction in subject's judgments between moral and conventional transgressions. Psychopaths may lack this violence inhibitor. A causal model is developed showing (...) how the lack of this mechanism would explain the core behavioural symptoms associated with the psychopathic disorder. A prediction of such a causal model would be that psychopaths should fail to make the moral/conventional distinction. This prediction was confirmed. Blair, David Concerning David Tribe's rejoinder to my 'Science works better than that' , it's pleasing to see that there are some points on which we agree. Unfortunately he continues to make a strong and unjustified attack on the scientific community as a whole-essentially on the grounds that, of the conclusions of science that later turned out to be false, virtually all of them were at some time 'believed' by most scientists. In reply, I shall show that it is (...) his underlying framework that needs to be altered-quite radically. He does not understand what the activity of scientists is like-indeed what it has to be like, when faced with the task of extracting knowledge from the real world. (shrink)

This target article considers the relation of fluid cognitive functioning to general intelligence. A neurobiological model differentiating working memory/executive function cognitive processes of the prefrontal cortex from aspects of psychometrically defined general intelligence is presented. Work examining the rise in mean intelligence-test performance between normative cohorts, the neuropsychology and neuroscience of cognitive function in typically and atypically developing human populations, and stress, brain development, and corticolimbic connectivity in human and nonhuman animal models is reviewed and found to provide evidence of (...) mechanisms through which early experience affects the development of an aspect of cognition closely related to, but distinct from, general intelligence. Particular emphasis is placed on the role of emotion in fluid cognition and on research indicating fluid cognitive deficits associated with early hippocampal pathology and with dysregulation of the hypothalamic-pituitary-adrenal axis stress-response system. Findings are seen to be consistent with the idea of an independent fluid cognitive construct and to assist with the interpretation of findings from the study of early compensatory education for children facing psychosocial adversity and from behavior genetic research on intelligence. It is concluded that ongoing development of neurobiologically grounded measures of fluid cognitive skills appropriate for young children will play a key role in understanding early mental development and the adaptive success to which it is related, particularly for young children facing social and economic disadvantage. Specifically, in the evaluation of the efficacy of compensatory education efforts such as Head Start and the readiness for school of children from diverse backgrounds, it is important to distinguish fluid cognition from psychometrically defined general intelligence. The paper's thesis is that dialogue is not an adequate model for all types of argument. The position of Walton is taken as the contrary view. The paper provides a set of descriptions of dialogues in which arguments feature in the order of the increasing complexity of the argument presentation at each turn of the dialogue, and argues that when arguments of great complexity are traded, the exchanges between arguers are turns of a dialogue only in an extended or metaphorical (...) sense. It argues that many of the properties of engaged back-and-forth exchanges of paradigmatic argument dialogues are not found in ‘solo’ arguments, and that at least some of the norms appropriate to the former type of argument, such as some of the pragma-dialectical rules of van Eemeren and Grootendorst's model, do not apply to the latter. An argument is a proposition and a reason for it. and argumentation is an interchange involving two or more parties resulting in the assertion of one or more arguments coupled with anticipated or actual critical responses. A logically good argument has grounds adeq uate for the purposes at hand (true, probable, plausible, acceptable to the audience) and the grounds provide adequate support for the conclusion. Academic and public debate is continuing about whether digital nomadism, a new Internet-enabled phenomenon in which digital workers adopt a neo-nomadic global lifestyle, represents ‘real’ emancipation for knowledge workers—or if it is, instead, the opposite. Based on a field study of digital nomadism, and accepting a pluralist approach to emancipation, we analyse the ‘emancipatory project(s)’ that digital nomads engage in. This analysis, following Weberian idealtypes, employs a tripartite structure: unsatisfactory conditions (what people want to overcome); emancipatory means (actions taken); and (...) emancipatory ends (desired outcomes). We critically compare digital nomadism to the traditional descriptions of emancipatory projects in nation-state contexts, as found in prior literature, using the same analytical framework. Juxtaposing these idealtypes, we discuss similarities and differences and analyse their inherent assumptions, logics and ethical stances. We conclude that digital nomadism generates an emancipation that is very much ‘real’ for digital nomads, whose experience cannot be disregarded, but with a ‘postmodern’ ethos that is at odds with modernity and its ethos originating from the Enlightenment. This paper examines some of the gendered insecurities informing some of the livelihood practices of Zimbabwean migrants in northern South Africa from 2004-2011, the period in which I carried out almost annual ethnographic research in this region. Situating these practices within wider policy shifts and changing migration patterns at the national and local scales, this paper shows the importance of attending to gendered dependencies and insecurities when analysing migrant livelihoods in southern Africa. These include those found within humanitarian organizations targeting (...) Zimbabwean migrants in their programs and policies in the border area. These gendered insecurities, which are woven into the fabric of travel, work and accommodation for these migrant Zimbabwean women in northern South Africa, should be examined in struggles for social justice. By drawing on the lens of social critique to engender a wider sense of the social justice needs for Zimbabwean women migrants in South Africa, this essay aims to broaden the focus of activism on women migrants to also attend to gendered insecurities in their everyday economic and shelter-seeking activities. Empathy is a lay term that is becoming increasingly viewed as a unitary function within the field of cognitive neuroscience. In this paper, a selective review of the empathy literature is provided. It is argued from this literature that empathy is not a unitary system but rather a loose collection of partially dissociable neurocognitive systems. In particular, three main divisions can be made: cognitive empathy , motor empathy, and emotional empathy. The two main psychiatric disorders associated with empathic dysfunction are (...) considered: autism and psychopathy. It is argued that individuals with autism show difficulties with cognitive and motor empathy but less clear difficulties with respect to emotional empathy. He argues that, although we might still wish to incarcerate such (...) individuals to prevent them inflicting further harm on others, we should be less enthusiastic to blame them for their actions.In many respects, I agree with much of Levy's treatise. I certainly do not regard ascriptions of blame as useful in my own work with this population. I believe that our goal should be to fully understand the nature of their emotional deficit to be able to treat it similarly to other researchers who work to treat other emotional disorders. To me, Levy's argument appears sound.However, I offer three considerations and raise questions within these considerations that might further the debate. First, not all rules involve care-based emotional responses to victims. As Levy notes, this is the origin of the moral/conventional distinction. The problem here is that we do consider someone responsible if they break a conventional transgression. The student talking in class is generally considered blameworthy for this action as long as they knew that the rule of not talking during the lesson applied to that classroom. Yet, there is no victim and no care-based emotional response. Here, the only thing necessary to ascribe blame and responsibility is that the individual knew that a rule exists. Given this holds for conventional transgressions, why must we necessarily make a special case, with respect to responsibility, for moral transgressions?I believe that Levy is fully aware of this concern. He specifically makes reference to moral responsibility and writes that:the kind and degree of wrongness, and therefore blame, which attaches to infringements of the rules [against double parking or line-jumping] is very different, and usually much less significant, than the kind and degree attaching to moral wrongs. For psychopaths, all offences are merely conventional, and therefore—from their point of view—none of them are all that serious.(2007, p. 132)But the danger here is that issues of blame and responsibility may become highly relativistic and hinge on the strength of the emotional reaction. This is a problem because, although we might judge someone as more blameworthy if he intentionally [End Page 149] runs over five people rather than one, do we really consider that there is a distinction in moral responsibility between these two cases? If not, might not responsibility relate to factual knowledge of the existence of the rule against the transgression rather than the emotional response to the transgression?Second, there is considerable debate regarding whether psychopathy represents a taxon or a continuum (e.g., Vasey et al. 2005; Edens et al. 2006). Assuming it does represent a continuum, then we have an additional concern. How little of an emotional response to the transgression reduces one's moral responsibility and blame? Is it only the very reduced (although usually not absent) levels of emotional response that are shown by individuals with psychopathy? Or should we consider moral responsibility and blame proportional to the level of the emotional response? If this latter option is adopted, then what should we do about individuals who show heightened emotional responses, for example, patients with specific anxiety disorders (Rauch, Shin, and Wright 2003)? There are already some data that inhibited children (a developmental precursor to some anxiety disorders; Kagan and Snidman 1999) show heightened conscience development (Asendorpf and Nunner-Winkler 1992). If such children commit moral transgressions, should we regard them as more morally responsible and more blameworthy than regular folks?Third, the issue of intention received little attention in this paper. On the one hand, the basic functions of selecting, summarizing, storing and sorting information garnered from reading, listening, observing and thinking can be identified in most literate contexts in some form or other. On the other hand, Renaissance humanists emphasized with unprecedented success the virtues of stockpiling notes on large scales and for the long term, thanks to the availability of paper and a new abundance of books, but also to (...) their ability to transmit their own keen motivation to avoid any future loss of learning. We continue to share many early modern ideals for insuring the collection and retrievability of information and have built on early modern practices that facilitate the accumulation and the organization of information, including collaborative work and the use of rearrangeable slips. The question, “What norms are appropriate for the evaluation of the probative merits of visual arguments?” underlies the investigation of this paper. The notions of argument and of multimodal visual argument employed in the study are explained. Then four multimodal visual arguments are analyzed and their probative merits assessed. It turns out to be possible to judge these qualities using the same criteria that apply to verbally expressed arguments. Since the sample is small and not claimed to be representative, this (...) finding can at best be regarded as suggestive for the probative assessment of multimodal visual arguments in general. Given our own group's efforts to develop national guidelines, we have rethought the issue of palliative sedation therapy several times over the past year.The use of clear and concise definitions is fundamental to the development of any consensus guidelines on this topic. This paper will address the political and ethical ramifications of Derrida's concern for friendship in relation to his concerns with the future of democracy, rights of hospitality and cosmopolitics. The questions addressed read as follows: Is there a way we can get beyond this stance which not only consolidates a friendship of the ‘perhaps’ with a friendship of the promise, but also implicates their consolidation with the very future of what we today call democracy? Is there a way in which (...) we can substantiate something more than a romanticized call for a future integration of friendship and democracy while avoiding the pitfalls of on one hand, substantiating a model of friendship for politics or, on the other, offering a disguised and naïve return to a metaphysics of friendship as the saving grace of social unity? Through a close reading of the conclusion to Politics of Friendship as well as his concerns with friendship in Spectres of Marx and Rogues: Two Essays on Reason it will be argued that Derrida's insistence on the future of friendship is bound up with the notion of an ethical promise to the thought of friendship as the condition for its political and ethical relevance. Fifty years ago this summer, the Uniform Anatomical Gift Act was adopted by the National Conference of Commissioners on Uniform State Laws and approved by the American Bar Association. The UAGA has provided a sound and stable legal platform on which to base an effective nationwide organ donation system. The cardinal principles of altruism, autonomy, and public trust are still important. At a time when confidence and trust in our government and many private institutions has declined, maintaining trust and confidence (...) in our health care system and its commitment to “first, do no harm” has never been more important. Any policies that override these core ethical principles could cause irreparable damage to the public's faith in our transplant system. While progress has been made to increase organ registration and the number of organs transplanted, much more must be done to realize the potential of life‐saving therapy without jeopardizing ethical principles. In this paper, we will consider the neuro-cognitive systems involved in mediating morality. Five main claims will be made. First, that there are multiple, partially separable neuro-cognitive architectures that mediate specific aspects of morality: social convention, care-based morality, disgust-based morality and fairness/justice. Second, that all aspects of morality, including social convention, involve affect. Third, that the neural system particularly important for social convention, given its role in mediating anger and responding to angry expressions, is ventrolateral prefrontal cortex. Fourth, that the (...) neural systems particularly important for care-based morality are the amygdala and medial orbital frontal cortex. This article challenges the view that rhetoric, dialectic and logic are three perspectives on argument, relating respectively to its process, its procedure, and its product. It also questions the view that rhetorical arguments represent a distinctive type. It suggests that, as related to argument, rhetoric is the theory of arguments in speeches, dialectics the theory of arguments in conversations, and logic the theory of good reasoning in each.

This appointment was different though; it came about as a result of request by a psychology professor who was conducting a follow–up study on individuals treated by the team of endocrinologists I saw as a child.The professor had reached out to me through my parents, since that was the only contact information available. It had been more than ten years since I had seen any of the doctors who treated me as a child. I was living in another state when I received the call from my mother informing me of the request for my participation in the study. I was really quite shocked; in all the years I was treated in early childhood and adolescence, no one had ever asked me how I felt about the treatment I received. I knew I had a medical condition, but no one ever explained to me why I needed so many appointments, and why I had to take medicine three times a day. No one told me why, when I was two or three years old, I had surgery that left me with memories of gauze and a catheter between my legs, or why medical residents still wanted to examine me, or why I had to be humiliated and ashamed, again and again. I was in my early 30’s by this time. Surely if my condition was so important that someone wanted to talk to me now, so many years later, the conversation would have already happened?!My parents dropped me off at the scheduled location for the interview. The professor introduced himself and his colleague. I didn’t recognize them, which meant they weren’t the doctors who had treated me. I remember feeling somewhat relieved, as I never liked the doctors who treated me as a child. I remembered my last appointment there, and feeling that I was escaping from captivity, that I was free to go on with my life.I wasn’t sure what to expect from this meeting. The professor told me that to date no one had completed any follow–up on the pediatric patients treated by the hospital’s endocrinology team to see how they were faring in life. He began by asking, “so, D_____, [I went by another name then] how are you doing at this point in your life?”I felt a sudden surge of rage, and, to my own surprise, I found myself responding angrily, “Why the hell do you care? The notion that God and the world are mutually interdependent is generally taken to be unique to twentieth-century process theology. Largely, process thinkers have focused on classical theists, rather than the mystics. My thesis, however, is that, centuries before process came along, there were Western mystical concepts stressing that God needed the universe in order to become conscious and complete. In support of my thesis, I will provide a synopsis of the doctrines of God as found in mystics such as (...) Boehme, Dionysius, Eckhart, and then show how Whitehead’s aesthetic provides a coherent philosophical psychology of ecstasy. In Argumentation Theory: A Pragma-Dialectical Perspective (2018), Professor van Eemeren suggests that it might be worthwhile for pragma-dialectics and informal logic to join forces, given that there is a “considerable amount of common ground” between the two. In this paper, we explore that common ground by considering both the ways logic is understood and incorporated in the pragma-dialectical model and the ways informal logic has developed since its inception in the 1970s. Built in the centre of Copenhagen, and noted for its equestrian stairway, the Rundetaarn, was intended as an astronomical observatory. Part of a complex of buildings that once included a university library, it affords expansive views of the city in every direction, towering above what surrounds it. The metaphor of the towering figure, who sees what others might not, whose vantage point allows him to visualize how things fit together, and who has an earned-stature of respect and authority, fits another (...) Danish stalwart, Hans Vilhelm Hansen, whose contributions to the fields of informal logic and argument theory have earned the gratitude of his colleagues, and inspired this collection of essays, written to express the appreciation of its authors and of the many, many colleagues they represent. This paper’s thesis is that the fallacies should not be taught to undergraduates. Besides some bad influences, this is not only because doing so steals time more valuably spent elsewhere, but also because the field is now so complex (overlapping concepts, theories and disciplines), that we lack knowledgeable instructors and sophisticated students. The study of theories involving fallacies, however, remains viable.

This article uses interview data to examine changes over time in the cultural constructions of executive women's family responsibilities. The author delineates two gendered cultural structures: the family devotion schema and the work devotion schema. Respondents are caught in the conflict between each schema's competing vision of a worthwhile life. Older respondents are more likely to accept the devotion schema's definition of an irreconcilable conflict between work and family, prompting many to avoid marriage or childbearing. In contrast, many members of (...) the youngest cohort, who came of age after the early 1970s women's movement, are refashioning the family devotion schema by subcontracting out domestic responsibilities while maintaining demanding careers. Yet, the family devotion schema continues to haunt all cohorts, enduring in its cognitive, normative, and emotional power. Gender, as expressed in cultural models of family, remains a powerful constraint on the hearts and minds of even professionally successful women. (shrink)
