Pregnant adolescents have a higher risk of poor maternal and fetal outcomes, particularly in the setting of concomitant maternal alcohol and other drug (AOD) use. Despite numerous programmes aimed at reducing overall teen pregnancy rates and the recognition of AOD use as a risk factor for unintended pregnancy in adolescents, interventions targeting this specific group have been sparse. In adult drug-using women, financial incentives for contraception have been provided but are ethically controversial. This article explores whether a trial could ethically (...) employ monetary incentives in adolescents with AOD use to promote the use of long-acting reversible contraception (LARC), with special attention to the relevant distinctions between adults and adolescents. We conclude that a trial of incentives to promote LARC in this patient population is ethically permissible if the incentives are small, are tied to completion of an educational activity to minimise the quick fix temptation and potential for non-attendance to the risks and benefits of LARC and are provided only to the adolescent after an assessment of her reasoning to rule out coercion (eg, by guardians) as motivation. Information about treatment for AOD use and follow-up care in case of problems with the contraceptive or desire for removal should also be provided. Before implementing such a trial, qualitative research with input from providers, potential patients and their parents should be conducted to inform the programme's specific structure. (shrink)

These labels we use to categorize ourselves have a huge effect on how we make our decisions–when faced with the choice of doing X vs. doing Y, whether I think of myself as someone’s who’s civic-minded and whether someone who’s civic-minded would do X can both play a huge role in influencing whether I decide (...) to do X. What does all that have to do with freedom? Our guest this month thinks that freedom just is the ability to affect, shape, or even create these sorts of labels, before then applying them to yourself. It isn’t as though anyone invents every label they apply to themselves; plenty of them are handed down to us from popular culture and other sources. Background Little is known about communication between patients, families, and healthcare providers regarding ethical concerns that patients and families experience in the course of illness and medical care. To address this gap in the literature, we surveyed patients and family members to learn about their ethical concerns and the extent to which they discussed them with their healthcare providers. Methods We surveyed adult, English-speaking patients and family members receiving inpatient care in five hospitals in the Washington DC-Baltimore metropolitan area from (...) July 2017 to March 2020. Descriptive statistics were used to determine the frequency, comfortableness, and helpfulness of discussions regarding ethical concerns experienced when sick or receiving medical care. Univariable and multivariable stepwise logistic regression models were used to identify associations between healthcare provider and respondent characteristics and attitudes and (1) the likelihood of speaking to a healthcare provider about their ethical concern and (2) their level of comfort during these discussions. Results Of 468 respondents who experienced ethical issues, 299 (64%) reported discussing the situation with a member of their healthcare team; 74% (197/265) of respondents who had such a discussion found the discussion comfortable, and 77% (176/230) of respondents found the discussion helpful. To make discussions more comfortable and helpful, respondents proposed suggestions in open-ended responses involving (1) content and quality of communication; (2) positive healthcare provider qualities such as empathy, open-mindedness, knowledge, honesty, and trustworthiness; and (3) other contextual factors including having adequate time and available resources. Conclusions Patients and families often have ethical concerns that they discuss with clinicians, and they want clinicians to be routinely receptive and attentive to such discussions. (shrink)

Collections of living organisms are reservoirs of biological knowledge that operate across times and places. From the mid-20th century, scientific institutions dedicated to the cultivation of such collections have routinized and professionalized their care. But “care,” for these collections, is focused not just on individual organisms—instead, a principal aim of a curator is to maintain the integrity of a reproducing “strain,” “variety,” “line,” or “stock,” and the composition of a collection as a whole. This paper explores the forms, the material (...) dimensions, the temporalities, and the values of that care, to recover the conditions under which scientist-custodians maintain continuity of research over many decades. This paper does so by focusing on two rather different kinds of scientific collection: that of Drosophila fruit flies on the one hand, and plant seeds on the other. Their comparison is valuable because their vastly different needs and life cycles engender very different practices of care. Comparing the materialities, life cycles, needs, and values of these divergent collections helps to draw attention to the routine and the apparently mundane. First, the paper asks: what kinds of work go into managing such collections—that is, the day-to-day management and cultivation, surveillance, and administration of information? Second, it asks: how do these practices maintain the integrity of the strains and stocks, and the collections themselves? What kinds of value does this work create? Third, what are the future imaginaries that are rhetorically drawn into the funding strategies of these collections, and how do they envision future use, ownership, and control? (shrink)

This study contributes to the growing interest in how hybrid organizations manage paradoxical social–business tensions. Our empirical case is “impact sourcing”—hybrids in global supply chains that hire staff from disadvantaged communities to provide services to business clients. We identify two major growth orientations—“community-focused” and “client-focused” growth—their inherent tensions and ways that hybrids manage them. The former favors slow growth and manages tensions through highly integrated client and community relations; the latter promotes faster growth and manages client and community relations separately. Both growth orientations address social–business tensions in particular ways, but also create latent constraints that manifest when entrepreneurial aspirations conflict with the current growth path. The recently reported cases of coerced sterilisation of women at a privately operated immigration detention facility in the USA are egregious in their disregard for human dignity and professional ethics, but sadly not surprising. These abuses represent a continuation of efforts to control the reproductive capacity of women, fueled by racist and xenophobic motives. Physicians helped create and legitimise the pseudoscientific framework for the eugenics movement, which would implement forceful sterilisation as its tool of choice to eliminate undesirable traits that (...) were thought to be biologically inherited and predominant among racial and ethnic minorities. Although state-endorsed forcible sterilisation programs have ended, incarcerated women have remained particularly vulnerable to sterilisation abuse. The intersectional vulnerabilities of racism, xenophobia and carcerality must be addressed to prevent such abuses from recurring. There are no data in this work. (shrink)

A venerable tradition in the metaphysics of science commends ontological reduction: the practice of analysis of theoretical entities into further and further proper parts, with the understanding that the original entity is nothing but the sum of these. This tradition implicitly subscribes to the principle that all the real action of the universe (also referred to as its "causation") happens at the smallest scales-at the scale of microphysics. A vast majority of metaphysicians and philosophers of science, covering a wide swath (...) of the spectrum from reductionists to emergentists, defend this principle. It provides one pillar of the most prominent theory of science, to the effect that the sciences are organized in a hierarchy, according to the scales of measurement occupied by the phenomena they study. On this view, the fundamentality of a science is reckoned inversely to its position on that scale. This venerable tradition has been justly and vigorously countered-in physics, most notably: it is countered in quantum theory, in theories of radiation and superconduction, and most spectacularly in renormalization theories of the structure of matter. But these counters-and the profound revisions they prompt-lie just below the philosophical radar. This book illuminates these counters to the tradition principle, in order to assemble them in support of a vaster (and at its core Aristotelian) philosophical vision of sciences that are not organized within a hierarchy. In so doing, the book articulates the principle that the universe is active at absolutely all scales of measurement. This vision, as the book shows, is warranted by philosophical treatment of cardinal issues in the philosophy of science: fundamentality, causation, scientific innovation, dependence and independence, and the proprieties of explanation. This article aims to show that fundamentality is construed differently in the two most prominent strategies of analysis we find in physical science and engineering today: (1) atomistic, reductive analysis and (2) Systems analysis. Correspondingly, atomism is the conception according to which the simplest (smallest) indivisible entity of a certain kind is most fundamental; while systemism, as will be articulated here, is the conception according to which the bonds that structure wholes are most fundamental, and scale and/or constituting entities are (...) of no significance whatsoever for fundamentality. Accordingly, atomists maintain that the basic entities—the atoms—are fundamental, and together with the “external” interactions among them, are sufficient for illuminating all the features and behaviors of the wholes they constitute; whereas systemists proclaim that it is instead structural qualities of systems, that flow from internal relations among their constituents and translate directly into behaviors, that are fundamental, and by themselves largely (if not entirely) sufficient for illuminating the features and behaviors of the wholes thereby structured. Systemism, as will be argued, is consistent with the nonexistence of a fundamental “level” of nondecomposable entities, just as it is consistent with the existence of such a level. Still, systemism is a conception of the fundamental in quite different, but still ontological terms. Systemism can serve the special sciences—the social sciences especially—better than the conception of fundamentality in terms of atoms. Systemism is, in fact, a conception of fundamentality that has rather different uses—and importantly, different resonances. This conception of fundamentality makes contact with questions pertaining to natural kinds and their situation in the metaphysics of the special sciences—their situation within an order of autonomous sciences. The controversy over fundamentality is evident in the social sciences too, albeit somewhat imperfectly, in the terms of debate between methodological individualists and functionalists/holists. A superselection rule advanced in the course of a quantum-mechanical treatment of some phenomenon is an assertion to the effect that the superposition principle of quantum mechanics is to be restricted in the application at hand. Superselection accounts of measurement all have in common a decision to represent the indicator states of detectors by eigenspaces of superselection operators named in a superselection rule, on the grounds that the states in question are states of a so-called classical quantity and therefore not (...) subject to quantum interference effects. By this strategy superselectionists of measurement expect to dispense with use of projection postulates in treatments of measurement. I shall argue that superselection accounts of measurement are self-contradictory, and that treatments of infinite systems, if they can avoid the contradiction, are not true superselection accounts. This article addresses the relations between ‘nature’ and ‘culture’ (and those characteristics associated with ‘the natural’ and ‘the cultural’) in the context of the debates about Prozac. Following Marilyn Strathern, I focus specifically on the contested issue of enablement - that is, on what Prozac does or does not enable, and on the relation between enablement and enhancement, normality and pathology. I argue that the implications of the model of the brain that accompanies explanations of Prozac are such that commentators (...) are obliged to address not only the nature of normality but also the nature of nature itself. Through a close analysis of these debates, I suggest that critiques of Prozac should be understood not as objections to reductionism - to a biology that closes things down - but rather to one that opens things up: that opens up the relations between nature, culture, biology and the individual, relations that are now cross-cut and thrown about by artificiality. Objections to Prozac, then, might be characterized as an attempt to put these concepts back into their ‘proper’ positions, to re-establish the relationality between them. In conclusion, I argue that the biology put forward by proponents of psychopharmacology, regardless of the desirability of the latter, challenges not only the frequent assumptions that are made about the claims of materialist science, but also some of the terms and concepts that are commonly deployed in the social sciences. This chapter focuses on finding better ways to conceptualize precaution. Precaution has now become an established principle of environmental governance, although it has not been distinguished from conventional risk assessment. It has been considered by some as the antithesis of risk assessment in the sense that it is done to avoid serious potential harm, without scientific certainty as to the likelihood, magnitude, or causation of that harm. The first and foremost task of this chapter is to show that these concepts (...) are non-overlapping. It also elaborates on the reasons why precaution should be taken seriously, and that, on the flipside, traditional risk assessment, or cost-benefit analysis, does not take precaution seriously because of its pervasive nature. This fact represents the ascendancy of a decision-making calculus that grew out of twentieth-century British moral theory and culminated in 1944 with the presentation of the doctrine now familiarly known as the theory of expected utility. This article undertakes a history of labor as seen through acknowledgment sections of the journals of the History of Science Society—Isis and Osiris—and documents what has been intentionally removed from acknowledgments over the decades. It focuses on the Society’s editorial office, with special attention to the career of manuscript editor Joan Vandegrift. Alongside a reading of select printed acknowledgments, this article offers a vernacular history of labor and identifies a paradox: the web of people and things included in acknowledgments expanded (...) amid an intentional, systematic exclusion of paid editorial staff. Over the century, acknowledgments in the Society’s journals echoed changes in the historiography—as the field shifted from stories of atomized individuals discovering truths to accounts of sociopolitical relationships through which knowledge was crafted. Historical work has always depended on relationships of various forms, but the acknowledgment of these interdependences began to appear only in the 1970s as feminist and postcolonial approaches to scholarship emerged. At the same time, acknowledgment of direct paid labor of Society employees, who improved and manifested historians’ publications, was actively removed from articles. In the process, editorial staff such as Joan Vandegrift were excluded from one of the few historical records of their labor and from the economies of credit that structure publishing industries. Overall, the aim of this piece is to support better understanding, continued rewriting, and ongoing transformation of the labor inequalities in our field. (shrink)

The radical probabilist counsels the prudent never to put away uncertainty, and hence always to balance judgment with probabilities of various sizes. Against this counsel I shall advise in favor of the practice of full belief — at least for some occasions. This advice rests on the fact that it is sometimes in a person's interests to accept certain propositions as a means of bringing it about that others recognize oneself as having accepted those propositions. With the pragmatists, therefore, I (...) shall reject the view that belief formation must in every instance be a truth-directed affair. Unlike the pragmatists, however, I shall conclude that the enterprise of belief formation is not directed exclusively, or even primarily, at attaining knowledge. In other words, pursuit of that which it profits to believe, on the one hand, and pursuit of knowledge on the other, are distinct enterprises, which overlap (when they do) only accidentally. Recent scholarship has emphasized the contributions of the great Maliki jurist Shihāb al-Dīn al-Qarāfī to Islamic legal thought. However, al-Qarāfī’s compilation of legal maxims and distinctions, al-Furūq, has not yet been studied, nor has the collection of his teacher, the prominent Shafiʿi jurist Ibn ʿAbd al-Salām, known as al-Qawāʿid al-kubrā. Furthermore, the original thought of Ibn ʿAbd al-Salām and his formative influence on al-Qarāfī have been understated. This article compares their two works to demonstrate that al-Qarāfī based his collection in (...) large part on Ibn ʿAbd al-Salām’s al-Qawāʿid and it examines the techniques that al-Qarāfī used, which included reordering, refining, and supplementing borrowed maxims, and anonymizing references to his teacher. Most salient, however, is al-Qarāfī’s “Malikization” of maxims, which entailed replacing Shafiʿi doctrines and authorities with their Maliki counterparts and deploying maxims to defend Maliki doctrines. The article concludes by explaining al-Qarāfī’s authorial choices in light of his Maliki affiliation and the politics between the legal schools in Mamluk Cairo. (shrink)

On the classical instrumental view, practical reason is an all-things-considered enterprise, concerned not merely with identifying and evaluating appropriate means to the realization of ends construed as uncriticizable, but also with coordinating achievement of their sum. The concept of a totality of ranked concerns is the cornerstone of the theory of utility. This paper discusses some of the ways that practical reasoning, on the ground, is not instrumental in this sense. The paper will demonstrate that some of what goes on (...) by way of practical reasoning on the ground involves a certain simple inference schema—to be called “imitative reasoning”—that involves mobilization of what has been alternately referred to as archetypes, scripts, stereotypes and schemas including most importantly self-schemas or self-concepts. Imitative reasoning, as the paper will argue, is especially hostile to deliberations that involve the sorts of tradeoffs that applications of utility theory routinely advise. It is therefore no expression or realization, however imperfect, of the notion of maximization. What is more, this framework routinely evokes as authoritative the norms of privilege, however removed from or irrelevant to the matter at hand, which might be as simple as where to go for dinner. For it is the basis of—among other things—class and race consciousness. Furthermore, it is highly subject to manipulation by the unscrupulous; such as, for example, by those who market consumer goods, especially to children. For this form of reasoning is employed so as to protect or enhance self-concepts: imitative reasoning is a form of motivated reasoning. Laying it out in schema form will shed light on the sort of reasoning processes that transpire in many cases of cognitive dissonance reduction. (shrink)

Talk abounds regarding the loss of public trust in such institutions as science or mainstream news media, but there is little clarity about the nature of public trust. Public trust, as this paper explains, is a correlate of a certain type of power in the sphere of communication—one enjoyed by a broadcast source (such as a scientific publication or a news outlet) in proportion to a number of recipients in its broadcast area who adopt its messages, or at least are (...) open to receiving them. This essay proposes new language and ontological tools for the analysis of this sort of power, because the present crop of concepts currently in circulation for handling questions around power broadly construed, and communicative power specifically, are inadequate. The possibilities of communication, and therefore of power in and through communication, differ—as the paper will also explain—according to whether the network through which they flow is governed by an authoritarian regime or instead a western-style democracy. Communicative power is the fundament of what will be referred to here as “people power”, consonant with its usage by Chenoweth et al. (2019), a nonviolent power that constitutes a natural and enormously potent check on the power of tyrants, and thereby forms one natural limit on tyranny. Communicative power has in recent memory been associated with Science and with the free press. The concepts about power articulated here will also contribute to clear formulation of numerous research questions in connection with the internet. (shrink)

What do we learn when we focus analysis – not so much on the content of experience – as on its universal features and functioning? Descartes believed that such focus (when exercised by someone employing his first-personal method of inquiry) held the key to the fundamental metaphysics of our universe – that it could reveal fundamental truths about the nature of substance, or at any rate could reveal some fundamental metaphysical categories and their contrasts. He believed such focus could lead (...) to a certain doctrine of dualism. Philosophers now widely hold that Descartes’ method was profoundly wrongheaded, in no way a candidate method for illuminating the material universe that is our own. In fact, however, Descartes’ method is considerably more serviceable. While unable to do what Descartes thought it could do, nonetheless it is ideal for examining a taxon that this essay will refer to as grammar or structure in the most general sense. Grammar is a contrary of content or materiality, though not the only one since materiality enjoys multiple contraries. Thus Descartes’ method can lead us to a certain dualism, but not the one that Descartes imagined. (shrink)

HD causes neurodegeneration resulting in the progressive development of physical, cognitive and emotional symptoms. The impact on sufferers worsens over time with the final stage of the disease resulting in the need for professional assistance in a long-term care facility. More rarely HD develops in children and young adults, with less than 5% of HD sufferers being (...) affected by Juvenile HD. This article considers the ethical aspects of such testing. (shrink)

Soffritti found that significant trend of increasing lymphoma and leukaemia incidence in female rats fed aspartame with a significant increase in the number of female rats affected at dosages of 20 mg/kg per day and upward, thus questioning aspartame's safety. However, the European Food Safety Authority's (EFSA's) Scientific Panel on Food Additives, Flavourings, Processing Aids and Materials in Contact with Food (AFC) argued that there was not enough (...) evidence to conclude that aspartame causes an increased incidence of lymphomas and leukaemias, also arguing that the increase in the number of rats affected with lymphomas and leukaemias could be due to chronic respiratory infections of the rats used in the study. In this paper we show how a student can learn the basics of the system databases using (W3school CSS) which was built as intelligent tutoring educational system by using the authoring tool called (ITSB). The learning material contains CSS and HTML. We divided the material in a group of lessons for novice learner which combines relational system and lessons in the process of learning. The student can learn using example of CSS, and types of CSS color. Furthermore, the intelligent tutoring (...) system supports not only lessons; but exercises of different difficult levels for each lesson. When a student finish successfully the first difficulty level in a lesson, the student is allowed to move to the next difficulty level of the exercises of the lesson. (shrink)

I shall endeavor to show that every physical theory since Newton explains without drawing attention to causes–that, in other words, physical theories as physical theories aspire to explain under an ideal quite distinct from that of causal explanation. If I am right, then even if sometimes the explanations achieved by a physical theory are not in violation of the standard of causal explanation, this is purely an accident. For physical theories, as I will show, do not, as such, aim at (...) accommodating the goals or aspirations of causal explanation. This will serve as the founding insight for a new theory of explanation, which will itself serve as the cornerstone of a new theory of scientific method. (shrink)

Social enterprises that operate in business-to-business contexts, often out of emerging economies, typically face high expectations from business clients, mainstream competition, and the challenge of operating across distances. In these contexts, social enterprises need to carefully choose which market segments to serve and how to organize their social mission accordingly. Based on the case of impact sourcing—hiring and training of disadvantaged staff for global business services—we seek to better understand this interplay. In general, we find that social enterprises in this (...) context focus on serving either domestic clients with an implicit social mission and an integrated social enterprise model or international clients with a more explicit social mission and a decoupled model. We discuss why both main configurations represent viable social enterprise models in the outsourcing industry, and why, in particular, the professional background of founders plays a key role in these strategic choices. Our findings contribute to a more nuanced and context-sensitive understanding of social enterprise model adoption in emerging economies. (shrink)

The following paper analyses this question according to Humboldt’s writings and shows its relevance for education. The co-occurrence of stress-induced posttraumatic stress disorder (PTSD) and obesity is common, particularly among military personnel but the link between these conditions is unclear. Individuals with comorbid PTSD and obesity manifest other physical and psychological problems, which significantly diminish their quality of life. Current understanding of the pathways connecting stress to PTSD and obesity is focused largely on behavioral mediators alone with little consideration of the biological regulatory mechanisms that underlie their co-occurrence. In this work, we leverage prior knowledge to (...) systematically highlight such bio-behavioral mechanisms and inform on the design of confirmatory pilot studies. We use natural language processing (NLP) to extract documented regulatory interactions involved in the metabolic response to stress and its impact on obesity and PTSD from over 8 million peer-reviewed papers. The resulting network describes the propagation of stress to PTSD and obesity through 34 metabolic mediators using 302 documented regulatory interactions supported by over 10,000 citations. Stress jointly affected both conditions through 21 distinct pathways involving only two intermediate metabolic mediators out of a total of 76 available paths through this network. Moreover, oxytocin (OXT), Neuropeptide-Y (NPY), and cortisol supported an almost direct propagation of stress to PTSD and obesity with different net effects. Although stress upregulated both NPY and cortisol, the downstream effects of both markers are reported to relieve PTSD severity but exacerbate obesity. The stress-mediated release of oxytocin, however, was found to concurrently downregulate the severity of both conditions. These findings highlight how a network-informed approach that leverages prior knowledge might be used effectively in identifying key mediators like OXT though experimental verification of signal transmission dynamics through each path will be needed to determine the actual likelihood and extent of each marker’s participation. In the early twentieth century, the United States Department of Agriculture funded international expeditions with the aim of finding plant specimens for introduction into the agricultural landscape and the new experimental projects in hybridization. One such agricultural explorer, noted for his eponymous lemon, was Frank Nicholas Meyer, an immigrant from the Netherlands whose expeditions in Asia have brought to the United States celebrated fruit and toxic weeds. Neither professional botanists nor farmers, plant hunters like Meyer worked by taking advantage of (...) historical allegiances to academic programs, while leaning on the authority of their newer national institutions. In addition to plants, through photographs that transposed Chinese landscapes to U.S. environmental counterparts, Meyer contributed to the imagination of the agricultural promise of the American West. The era of these plant explorers has ended but their material trace remains in a variety of spaces and modes of existence that have hitherto been disregarded. Reading Meyer’s letters shows the authority and discipline behind his transformation from gardener’s apprentice to professional plant collector. I argue that photographs and plants are understudied material traces that enable historians to re-examine the means by which credit was received, given, and exchanged. By drawing together these traces, I chart the continued importance of exploration and collection in the twentieth century and show the epistemic continuity between nineteenth-century natural history and twentieth-century experimental science. This article explores representations of the artist Willem de Kooning who, during the last few years of his creative life, produced a large number of paintings at the same time as he was thought to be suffering from Alzheimer's disease. My focus is on the way that these representations mobilize themes which Jane Goodall identifies as belonging to a discourse of anxiety. In a bid to suggest that the artist is uniquely positioned to adapt to the progression of the disease, (...) commentators betray a particular concern with the relation between will and intuition. This relation is negotiated, first, by privileging the hand of the artist as a body part possessed of a brain and a will of its own and, second, by drawing on neurological explanations of the nervous system which have the effect of removing `will' from the sphere of voluntarism. Although these representations go some way to refigure conventional notions of artistic identity and creative processes, the reliance on neurology supports an elitism which justifies the exclusion of certain figures (the insane, the naive, the computer) not just from the subject-position `artist', but also from the category `human'.
