The last few years have seen dramatic progress in the development of HIV pre-exposure prophylaxis (PrEP). These developments have been met by ethical concerns. HIV interventions are often thought to be ethically difficult. In a context which includes disagreements over human rights, controversies over testing policies, and questions about sexual morality and individual responsibility, PrEP has been seen as an ethically complex intervention. We argue that this is mistaken, and that in fact, PrEP does not raise new ethical concerns. Some (...) of the questions posed by PrEP are not specific to HIV prophylaxis, but simply standard public health considerations about resource allocation and striking a balance between individual benefit and public good. We consider sexual disinhibition in the context of private prescriptions, and conclude that only unjustified AIDS-exceptionalism or inappropriate moralism about sex supports thinking that PrEP raises new ethical problems. This negative conclusion is significant in a context where supposed ethical concerns about PrEP have been raised, and in the context of HIV exceptionalism. (shrink)

Mental health professionals who care for asylum seekers in Western European countries increasingly encounter problems for which standard diagnostic and therapeutic protocols and institutional healthcare policies offer no ready answers. In the following case vignettes some of these problems can be identified.

Burt formulates her critique at a general level of abstraction that highlights the methodological deficiencies of sociogenomics without also calling attention to precisely the same deficiencies in the social science model she seeks to defend against its encroachments. What might have been a methodological bulwark against the excesses of sociogenomics is instead a one-sided critique that merely renews its charter.

Upshot: Massumi innovatively interlinks poststructuralist theory with ideas from cognitive psychology and Luhmann’s systems theory to deconstruct rational choice as the founding myth of the liberal economy. His politically charged constructivism explores socio-economic reproduction as a process of constant re-stabilization between the openness of affective response and the closure of rationality. Defying social determination, Massumi shows how affect can constitute a source of potential change when modulated trans-individually in response to political events.

Konkrete Hinweise zur Textauswahl und zur Aufbereitung eines Themas helfen ganz direkt bei der Strukturierung und Planung von Unterricht. Leitend ist dabei insgesamt die Idee, dass fachliche Souveranitat und philosophische Denkerfahrung genauso unabdingbare Grundlage fur das Lehramt sind, wie die Reflexion eigener Bildungsprozesse im Studium. This paper looks at a dispute decision theory about how best to characterize expected utility maximization and express the logic of rational choice. Where A1, … , An are actions open to some particular agent, and S1, … , Sn are mutually exclusive states of the world such that the agent knows at least one of which obtains, does the logic of rational choice require an agent to consider the conditional probability of choice Ai given that some state Si obtains, (...) Prob(Ai/Si). Or, is the logic of choice better represented by considering the probability of the counterfactual if Ai then Si, Prob(Ai ⟥-> Si). Causal decision theory, developed by Allan Gibbard, William Harper, and David Lewis defend the counterfactual analysis; whereas, Richard Jeffrey and others defend the conditional probability analysis, evidential decision theory. I argue that the problems posed by cases of decision instability favor evidential decision theory. (shrink)

To examine the ethical issues involved in governmental decisions with potential health risks, we review the history of the decision to raise the interurban speed limit in Israel in light of its impact on road death and injury. In 1993, the Israeli Ministry of Transportation initiated an “experiment” to raise the interurban speed limit from 90 to 100 kph. The “experiment” did not include a protocol and did not specify cut-off points for early termination in the case of adverse results. (...) After the raise in the speed limit, the death toll on interurban roads rose as a result of a sudden increase in speeds and case fatality rates. The committee's decision is a case study in unfettered human experimentation and public health risks when the setting is non-medical and lacks a defined ethical framework. The case study states the case for extending Helsinki type safeguards to experimentation in non-medical settings. (shrink)

Over the past decade, clinical ethics has received growing attention in Germany as in most European countries. In the mid-1990s, most European countries made efforts to establish healthcare ethics committees and clinical ethics consultation services. The development of clinical ethics discourse and activities in Germany, however, was delayed and, consequently, is still in its natal phase. Until the end of the 1990s, the only institutionalized bodies of ethical reflection were the research ethics committees at university medical centers and at the (...) State Physician Chambers. In March 1997, the Catholic and Protestant hospital association in Germany recommended the implementation of HECs, modeled after the American HECs. Consequently, the establishment of clinical ethics consultation in the form of HECs started in Germany in denominational hospitals, followed by a small but increasing number of community hospitals. Twenty years ago, the biopsychosocial model was proposed by George Engel to be the new paradigm for medicine and psychiatry. The model assumed a hierarchical structure of the biological, psychological and social system and simple interactions between the participating systems. This article holds the thesis that the original biopsychosocial model cannot depict psychiatry's reality and problems. The clinical validity of the biopsychosocial model has to be questioned. It is argued that psychiatric interventions can only stimulate but not determine their target (...) systems, because intervention and outcome are only loosely coupled. Thus, psychiatric interventions have in principle limited ranges which differ according to the type of intervention and according to the system to be stimulated. Psychosocial interventions face far more obstacles to be overcome than psychopharmacogical therapy. (shrink)

Objectives—The study was performed in order to investigate how end-of-life decisions are influenced by cultural and sociopolitical circumstances and to explore the compliance of doctors with patient wishesParticipants and measurement—Five hundred and thirty-five physicians were surveyed in Sweden , Germany , and in Russia by a questionnaire. The participants were recruited according to availability and are not representative. The questionnaire is based on the one developed by Molloy and co-workers in Canada which contains three case vignettes about an 82-year-old Alzheimer (...) patient with an acute life-threatening condition; the questionnaire includes different levels of information about his treatment wishes. We have added various questions about attitudes determining doctors' decision making process .Results—Swedish physicians chose fewer life-prolonging interventions as compared with the Russian and the German doctors. Swedish physicians would perform cardiopulmonary resuscitation in the event of a cardiac arrest less frequently, followed by the German doctors. More than half the Russian physicians decided to perform CPR irrespective of the available information about the patient's wishes. Level of dementia emerged as the most powerful determining attitude-variable for the decision making in all three countries.Conclusions—The lack of compliance with patient wishes among a substantial number of doctors points to the necessity of emphasising ethical aspects both in medical education and clinical practice. The inconsistency in the treatment decisions of doctors from different countries calls for social consensus in this matter. (shrink)

The first important reaction in favor of generic criticism here was that of the Chicago neo-Aristotelians, whose feisty polemics against the "New" critics must have seemed, in the 1940s and 1950s, like voices crying in the wilderness. The popularity of Northrop Frye's Anatomy of Criticism—also ostensibly based upon Aristotle's example—won the concept of genre broader support. And today, if the books covering my desk are anything to go by, genre criticism has emerged in force. The flood has brought forth historical (...) studies of Renaissance genres, analyses of traditional genres like the picaresque or of new ones like "the fantastic," ambivalently generic essays in "thematics," efforts to systemize the genres of narrative fiction, and even attempt, through the philosophic analysis of dozens of generic systems, to go "beyond genre." Indeed, the late sixties spawned a journal entitled Genre entirely devoted to theoretical and practical criticism employing the concept. David H. The relevance of Wittgenstein for ethics depends on which Wittgenstein we mean. I argue that we should distinguish not only between Wittgenstein's personal opinions and his philosophy, but also, within his philosophical work, between broadly methodological remarks and what Wittgenstein might call genuinely philosophical remarks (which are not about philosophy but try to bring clarity to the mind bewitched by language). Wittgenstein's personal opinions will be considered irrelevant by most philosophers (although I try to show that they are not as (...) crazy as some people might think), and his philosophical remarks consist only of what he calls ?boring truisms?. It is to his methodology, therefore, that we might expect philosophers to look for ethical implications. If that methodology is well conceived, though, it should be ethically neutral. I argue that it is neutral in this way, that Wittgensteinian philosophy says nothing about ethics, but that its proper use will not likely leave our philosophical beliefs about ethics unscathed. Nach einem Vierteljahrhundert deutscher Einheit ist es an der Zeit einmal zu diskutieren, was uns Deutsche eigentlich ideell verbindet, was wir gemeinsam anstreben, m.a.W. worin unsere Identität besteht. Dabei geht es nicht um Abgrenzung gegenüber anderen Nationen, sondern um den besonderen Beitrag, den wir für die Zukunft der gefährdeten Menschheit leisten können. Nun brauchen wir, wenn uns diese Frage gestellt wird, gottseidank gar nicht zu spekulieren, sondern können an gegebene Tatsachen anknüpfen. Denn wie sich beim G-7-Treffen in Elmau wieder gezeigt (...) hat, ist die Bundesrepublik faktisch in der Umweltpolitik der Vorreiter unter den großen Industrienationen. Der Ausstieg aus der Kernenergie ist beschlossene Sache und das Erneuerbare-Energien-Gesetz ist bisher so erfolgreich gewesen, dass über 65 Länder in und außerhalb Europas es nachgeahmt haben. Allerdings ist die Frage bisher unbeantwortet, welche historischen Gründe es denn für diese deutsche Sonderrolle gibt. Zwar ist bekannt, dass der Schlüsselbegriff "Nachhaltigkeit" aus der deutschen Forstwirtschaft des 17. Jahrhunderts stammt oder dass die Deutschen stark durch die Romantik geprägt sind. Das vorliegende Buch möchte jedoch eine umfassendere und tiefere Antwort geben, indem es zeigt, dass sich die deutsche konfessionelle und philosophische Tradition im Naturverständnis deutlich von der angelsächsischen unterscheidet. Von der Reformation bis hin zur Relativitäts- und zur Quantentheorie hat sie überwiegend einen ökologischen Sinn. Reinhart Koselleck is among the most original German theorists of history and historiography. His international reputation is due in part to his contributions as theorist and editor of the remarkable lexicon Geschichtliche Grundbegriffe (GG). The GG is an exceptional reference work that goes far towards realizing Koselleck's program and distinctive version of Begriffsgeschichte (the history of concepts, conceptual history). What is presented here is a translation in full of Koselleck's own entry on Krise (crisis). The first step in planning a lexicon of European political and legal concepts is to decide upon how it is to be organised. Among the principal alternatives are the formats of three German reference works on the history of concepts (Begriffsgeschichte) and the methods associated with John Pocock and Quentin Skinner. Although these German and Anglophone styles are often regarded as incompatible, on closer inspection, they turn out to be in many respects complementary, as Skinner has recently acknowledged.
