The central theme of this book is that technological advances in medicine have created a multitude of choices for each individual -- choices that can influence how we live and die. These choices are difficult ones, and the book provides a better understanding of the issues. Thus, the implications of each choice become clearer. Such decisions remain inherently very difficult and personal. Thoughtful, compassionate societies must consider these difficult problems. Jane Stein makes a stimulating effort to clarify these subjects in this book. Corporations are developing more cost‐effective payment arrangements with health care providers and are shifting costs away from expensive forms of treatment. Although employees may have fewer options as a result, quality of care need not be affected.

Effective communication and interpersonal skills have long been recognized as fundamental to the delivery of quality health care. However, there is mounting evidence that the pressures of communication in high stress work areas such as hospital emergency departments present particular challenges to the delivery of quality care. A recent report on incident management in the Australian health care system cites the main cause of critical incidents, as being poor and inadequate communication between clinicians and patients. This article presents research that (...) describes and analyses spoken interactions between health care practitioners and patients in one ED of a large, public teaching hospital in Sydney, Australia. The research aimed to address the challenges and critical incidents caused by breakdowns in communication that occur between health practitioners and patients and by refining and extending knowledge of discourse structures, to identify ways in which health care practitioners can enhance their communicative practices thereby improving the quality of the patient journey through the ED. The research used a qualitative ethnographic approach combined with discourse analysis of audio-recorded interactions. Some key findings from the analysis of data are outlined including how the absence of information about processes, the pressure of time within the ED, divergent goals of clinicians and patients, the delivery of diagnoses and professional roles impact on patient experiences. Finally, the article presents an in-depth linguistic analysis on interpersonal and experiential patterns in the discursive practices of patients, nurses and doctors. (shrink)

Edith Stein is honored today not only because of her sainthood but because of what is now seen as important and groundbreaking work in phenomenology done under especially arduous conditions. Thus it may be said with some accuracy that Stein is, among philosophers, in the comparatively rare category of being acknowledged both for her work and her exemplary life. Writing on Stein has standardly proceeded with an emphasis on the biographical factors that caused her to live and (...) write as she did. This book challenges traditional approaches to research ethics based on moral principles and advocates a new, relationship-based paradigm for research ethics. The book begins with an explanation of the editors' reservations about the principalist approach pervading current regulations governing research. The editors' concerns are three-fold. First, they cite the continuing errors and abuse of human subjects of research in America despite federal regulations. Secondly, they argue that American regulations, grounded in the principles of autonomy, beneficence and justice, should not be (...) applied in all cultures. Finally, the editors argue that regulation is not the answer to all ethical questions concerning human subject research. I explore two walks, one by Henry Thoreau on a hot day in 1851 and one by a line as it winds its way into a doodle today. Walks, I contend, generate circuits of energies and affects, some issuing from people, some from elsewhere. The goal is to accent how ahuman energies and affects inscribe themselves upon selves and inflect their positions and dispositions. Borrowing a term from Lorenz Engell, I call this inscriptive inflection an ›ontographic‹ procedure. Ontography will mark (...) the operations of a creative cosmos, of a more-than-human world continuously impressing itself upon us. At the end, I leave the ontographic to return to the linguistic, to human attempts to ›write up‹ the ahuman ontographies they experience. (shrink)

Breath is invisible and yet ever present and vital for living beings. The concept of invisibility in relation to breath operates in concrete and metaphorical ways to extend ideas about breath and breathlessness across disciplines, in clinical spaces and in life experience. Using a critical medical humanities approach, I demonstrate that the poverty of narrative accounts and language for breath outside the health context have had a crucial influence enabling clinically mediated interpretations and accounts to dominate. These third-person accounts are (...) important in the articulation of the ‘lived body’, but I balance this with a consideration of the subjective sensation of interoception, which has important implications for the visibility of breathlessness in both clinical and lay contexts. This article illustrates the rich potential of the subjects of breath and breathlessness within body studies and this special issue is a key step in making breath such an emergent topic. (shrink)

Many people believe that a person's sexual orientation is genetic. Given the widespread prejudice against, and hatred of, homosexuals in many societies, it seems likely that many parents will be interested in using genetic technologies to prevent the birth of children who will not be heterosexual. This paper considers the moral and legal implications of such procedures (whether or not they would work). It is argued that the availability of procedures to select the sexual orientation of children would contribute to (...) discrimination and prejudice against lesbians, gay men and bisexuals and, more generally, undermine the maintenance of a just society. These effects carry significant weight in determining whether genetic technologies should be developed and whether their use is, or should be, legally permissible and morally acceptable. (shrink)

An analysis of the specific yogurt and phone microcredit schemes in Bangladesh is made along three lines of argument. It is important to note that these schemes are pulled together by NGO’s (non-governmental organizations) to assist women and children in developing areas to attain financial independence—the first line employs leftist criticism of the corporate constructs, and an additional line of inquiry compares some of the programs to those in other nations. A final line of argument analyzes the specific cultural views (...) of Bengali Islam and the long tradition of Bengali literacy. In this article, I outline three critiques of analytic philosophy; that it is irrelevant to individuals and society; unconstructive; and excessively technical. These critiques are linked to skepticism about the educational value of analytic philosophy. In response, I suggest that if analytic philosophy provides constructive guidance about prominent and pressing questions, then it holds potential educational value. I identify a body of prominent and pressing questions that are addressed by analytic philosophy as a discipline. Because analytic philosophy is often concerned (...) with the analysis of ordinary concepts, there is at least some potential educational value to analytic philosophy insofar as it provides guidance about the meaning of ordinary concepts that ground ordinary questions. Taking P. F. Strawson's paradigmatically analytic account of philosophy, I show that analytic philosophy can directly address ordinary questions in a constructive manner. Analytic philosophy is neither irrelevant, unconstructive, nor excessively technical. (shrink)

The pervasive use of contracts in healthcare is a source of unease for many healthcare ethicists and patient advocates. This commentary examines the use of such contracts with individuals in rehabilitation settings who have complex medical and behavioural issues. The goals of this paper are to examine the many factors that can lead to contract use, to discuss some legal and ethical implications of contract use, and to assess contract use in light of concerns about health equity. A Special Issue of the Journal of Philosophy of Education, 2005, issue 2, contains an interesting ‘Philosophy of the Teacher’ by Nigel Tubbs. It rejects attempts in pedagogical traditions to ignore or avoid the contradiction between the teacher as master and as servant, and ends with an interpretation of ‘upbuilding’, a central concept in Søren Kierkegaard’s writings. According to Tubbs’ reading, the teacher’s patient struggle with herself in doubt is the basic condition for upbuilding, whereby the eternal’s perfect gift of (...) love is given to the students. I maintain that the original Danish text does not fully support this interpretation and defend an alternative reading. The text says that the condition for receiving the perfect gift is itself a gift of God. Doubt is necessary, but if our doubt were sufficient, we could manage our lives without God, the condition for receiving the gift would not be a gift, and the possibility for receiving the gift would be spoiled both for teachers and students. Biological development is about history, the history of an individual through time. Historically, the dominant epigenetic tradition has seen the developmental process as an unfolding of potential or in terms of the emergence of new organization that becomes an individual organism over time. The concept of development has included differentiation, growth, and morphogenesis; since the mid-nineteenth century, it has been seen in terms of cell division. Along the way have come explorations of such issues as the extent to which development (...) is driven by hereditary determination rather than flexible regulation in response to changing conditions. Some researchers have focused specifically on examining the capacity for regeneration in response to injury or loss, or on the extent to which parts are self-organizing individually rather than determined segments of a whole. This piece, framed by sight and sound, is an (un)written essay on repetition, memory, rhythm, and marks made by the passage of time. The authorship condenses at once in the music, the initial creation, and then in the movement of the image, created with the memory of music spooling out in the silence of a train through the Rhône-Alpes. The result, an attempt— une tentative —a temptation, marks moments of feeling kept aloft through seeing what was once heard and marking (...) the passage of thought through memory's repetition. Do we remember what we listened to when we see what we saw when we listened? What remains calls back. Are you still there? Responding to thoughts, memories arise with rhythmic pulses in a mute endeavor. The voice we answer evaporates as we turn to greet it. We can replay but not repeat. We repeat without reporting, calling up a memory— une histoire —repeatedly, searching the scene. Each time new evidence, new non-evidence, new absence. Sight and sound. A parting with no meeting. Tentatively embracing the past, a passing memory in the present, big with its future. Its flowing rhythm that we take apart and keep a part of imparts a reminiscence which shapes as it passes, always departing. We think directly as the activity of memory marks the tempo. Interrupting our thoughts in their repetition, memory ignites with the slightest sound, smell, touch. A synaptic explosion which reshapes our memory of how we once thought, if we thought. Perhaps we only felt, but reflection impresses it upon present thought. Once, we have thought—or at least that is how I remember it, how you told me about it. (shrink)

Discussion at the symposium, and subsequent correspondence with participants, have raised a series of critical questions that seem to me to merit discussion. The issues raised have also led me to consider further some of the literature commenting on Newton’s work and on related matters in the history of optics. What was initially intended as a brief supplement to the foregoing paper [On Metaphysics and Method in Newton, item 10631] has thus evolved into a new article of considerable length.
