The authors use the term “principlism” to refer to the practice of using “principles” to replace both moral theory and particular moral rules and ideals in dealing with the moral problems that arise in medical practice. The authors argue that these “principles” do not function as claimed, and that their use is misleading both practically and theoretically. The “principles” are in fact not guides to action, but rather they are merely names for a collection of sometimes superficially related matters for (...) consideration when dealing with a moral problem. The “principles” lack any systematic relationship to each other, and they often conflict with each other. These conflicts are unresolvable, since there is no unified moral theory from which they are all derived. For comparison the authors sketch the advantages of using a unified moral theory. (shrink)

The pessimistic induction plays an important role in the contemporary realism/anti-realism debate in philosophy of science. But there is some disagreement about the structure and aim of the argument. And a number of scholars have noted that there is more than one type of PI in the philosophical literature. I review four different versions of the PI. I aim to show that PIs have been appealed to by philosophers of science for a variety of reasons. Even some realists have appealed (...) to a PI. My goal is to advance our understanding of what the various PIs can teach us about science and the threat posed by PIs to scientific realism. (shrink)

Many conditionals seem to convey the existence of a link between their antecedent and consequent. We draw on a recently proposed typology of conditionals to argue for an old philosophical idea according to which the link is inferential in nature. We show that the proposal has explanatory force by presenting empirical results on the evidential meaning of certain English and Dutch modal expressions.

Many languages allow for “fake” uses of their past tense marker: the marker: can occur in certain contexts without conveying temporal pastness. Instead it appears to bear a modal meaning. Iatridou :231–270, 2000) has dubbed this phenomenon Fake Tense. Fake Tense is particularly common to conditional constructions. This paper analyzes Fake Tense in English conditional sentences as a certain kind of ambiguity: the past tense morphology can mark the presence of a temporal operator, but it can also signal a specific (...) modal operator. The ambiguity is proposed to be the result of recategorization: the Simple Past develops a second, modal meaning because of structural similarities between the temporal and the modal/epistemic domain. The proposal is spelled out in the generative semantics framework, using the restrictor approach to conditionals Semantics from different points of view, 1979; in: A. von Stechow and D. Wunderlich Semantics: an international handbook of contemporary research, 1991), and building on von Stechows et al.’s recent work on the English tense system The expression of time in language, 2010; Romero and von Stechow, Tense: introduction, 2008). The author argues for three interconnected theses which provide a cognitive account for why humans intuitively believe that others survive death. The first thesis, from which the second and third theses follow, is that the acceptance of afterlife beliefs is predisposed by a specific, and already well-documented, imaginative process - the offline social reasoning process. The second thesis is that afterlife beliefs are social in nature. The third thesis is that the living imagine the deceased as socially embodied in such (...) a way as to continue to fulfill on-going social obligations with others. The author further suggests six reasons why the fantasy/reality distinction breaks down for the imaginer such that the continued existence of the decedent in the afterlife is believed to be real. Finally, the author suggests avenues for further research which would support this cognitive account. (shrink)

What are the criteria for reasonable clinical judgements? The reasonableness of macro-level decision-making has been much discussed, but little attention has been paid to the reasonableness of applying guidelines generated at a macro-level to individual cases. This paper considers a framework for reasonable clinical decision-making that will capture cases where relevant guidelines cannot reasonably be followed. There are three main sections. (1) Individual claims on healthcare from the point of view of concerns about equity are analysed. (2) The demands of (...) responsibility and equity on professional clinical performance are discussed, and how the combination of these demands emerges into seven requirements that constitute the framework is explored. Since this framework is developed to assist in reasonable clinical decision-making, practical implications of all these requirements are also suggested. (3) Challenges concerning the framework are discussed. First, a crucial presumption that the framework relies upon is considered—namely, clinicians’ willingness to justify their decisions as requested. Then how public deliberation may influence clinical decision-making is discussed. Next is a consideration of how clinicians’ need to have confidence in their own judgements in order to perform in a manner worthy of trust would be compatible with adherence to the framework supported by public deliberation. It is concluded that fair distribution in the interplay between macro- and micro-level considerations can be secured by legitimising procedures on each level, by ensuring well-organised and continuing public debate and by basing individual clinical judgements upon well-justified and principled normative bases. I examine the influence of James B. Conant on the writing of Kuhn’s Structure of Scientific Revolutions. By clarifying Conant’s influence on Kuhn, I also clarify the influence that others had on Kuhn’s thinking. And by identifying the various influences that Conant had on Kuhn’s view of science, I identify Kuhn’s most original contributions in Structure. On the one hand, I argue that much of the framework and many of the concepts that figure in Structure were part of Conant’s picture (...) of science, a picture that figured prominently in the general education natural science courses that Conant taught at Harvard. On the other hand, I show that Kuhn’s Structure contains important contributions that do not figure in Conant’s picture of science. I argue that the following three themes in Structure do not originate with Conant: (1) the concept of “normal science”; (2) “the problem of scientific revolutions,” that is, the apparent threat posed by radical changes of theory in science; and (3) Kuhn’s emphasis on the social dimensions of science, specifically the social structure of research communities. Usually, modular construction involves the off-site manufacturing of standard building components in a factory before the components are assembled on the construction site. It is common to use terms like "prefabrication," "off-site construction," and "modular construction" interchangeably. The construction of modular constructions nowadays is flourishing all over the globe. The roots of the Albanian prefabricated constructions are extended for the first time around the 1970s. This paper will indeed analyze some recently built modular construction in Albania, considering and comparing it (...) with international case studies. The demand for new construction in Albania is rising rapidly. Currently, the most common structures are cast-in-place beam-column designs, driven by both traditional mindsets and industrial capabilities. However, with Albania's economic growth, there is an increasing need for affordable, quickly built, and technologically advanced buildings. Modular construction, which assembles prefabricated modules on-site, offers a solution. This method aligns with bioclimatic design principles, reducing environmental impact and optimizing resources, marking a shift toward sustainability in the construction industry. The purpose of this article is to explore the potential integration of “modular construction techniques” with “bioclimatic design concepts” to create buildings that are environmentally friendly, energy-efficient, and multipurpose by function. This study explores the use of modular construction techniques in conjunction with bioclimatic techniques to increase “building sustainability”. Combining these tactics encourages energy efficiency and a reduced environmental effect, making people far more resilient and weather-adaptable. The synergy between the two of them is necessary. The primary aim of this research was to investigate the intricate relationship between the structural elements of experiences and their essential role in meaning formation. The analysis focused on understanding the nature of mental representations and the subjective, phenomenal qualities that emerge within experiences. To achieve this, an integrated approach, combining cognitive semantics with phenomenological analysis, was employed to examine the compositional complexities of the dynamic interaction between a priori and immediate experiences and their significance in meaning formation. The study (...) highlights the interconnectivity of structural elements within experience as a critical factor in shaping the phenomenal qualities of mental representations. Another key contribution of this study is the introduction of the “fulfiller” concept, which underscores the importance of absent qualities in meaning formation-an often-overlooked aspect in traditional models that focus solely on present attributes. The “fulfiller” concept emphasizes how absence, in addition to presence, influences meaning assignment. This inclusion enhances our understanding of meaning formation by considering both the tangible and intangible dimensions of the experiential-intentional process, offering a more comprehensive framework for understanding how meaning emerges from the complex interaction of present and absent qualities. Analyses of biological concepts of disease and social conceptions of health indicate that they are structurally interdependent. This in turn suggests the need for a bridge theory of illness. The main features of such a theory are an emphasis on the logical properties of value terms, close attention to the features of the experience of illness, and an analysis of this experience as action failure, drawing directly on the internal structure of action. The practical applications of this theory are outlined (...) for a number of problems in each of the three main practical areas, clinical work, teaching and research. In each case the resources of the theory suggest new models and generate new results. The full practical significance of the theory, however, is shown to consist in the way in which it ties together biological and social theories into an integrated picture of the conceptual structure of medicine as a whole. It is argued, finally, that practical efficiency of this kind is a test of theory not only in the philosophy of medicine but also in general philosophy. (shrink)

This paper presents a natural deduction system for orthomodular quantum logic. The system is shown to be provably equivalent to Nishimura’s quantum sequent calculus. Through the Curry–Howard isomorphism, quantum $$\lambda $$ -calculus is also introduced for which strong normalization property is established.

This paper reports a study of planning processes in the five-disc Tower of London (TOL) task in 20 younger and 20 older adult participants. A concurrent direct ''think-aloud'' method was used to obtain data on planning processes prior to moving discs in the TOL. A check was made of the effects of verbalising by comparing performance data from the experimental groups with data from control groups who did not verbalise during planning or moving. Verbalising slowed down planning and moving but (...) did not appear to distort the participants' approaches to the task. Older and younger participants did not differ in average moves taken to solve the tasks. However, older participants' planning was less complete and more error-prone than that of younger participants. The planning processes were characterised as showing a means-ends ''goal selection'' strategy. In this strategy participants (1) identify a single active goal disc at the start, (2) select moves and move sequences to enable the placing of the current goal disc in its target position, and (3) continue in this way until all discs are in their target positions. Age differences were found in the planning stage, during which there was no stimulus support and hence a substantial working memory load. During the move phase there was stimulus support and hence little loading of working memory. Age differences in moves required were not found in the move phase. As older participants tend to have depleted working memory capacity the present results suggest that working memory is heavily loaded in TOL planning but less so in the move phase of TOL. (shrink)

I examine the value and limitations of Kuhn’s Structure of Scientific Revolutions. In the interests of developing a social epistemology of science, I argue that we should draw on Kuhn’s later work, published in The Road since Structure. There, Kuhn draws attention to the important role that specialty formation plays in resolving crises in science, a topic he did not discuss in Structure. I argue that we need to develop a better understanding of specialty research communities. Kuhn’s later work provides (...) a research agenda for those who want to study scientific specialization. I briefly discuss some recent studies that promise to provide valuable insight into the social structure of science. (shrink)

We examine the emergence of the field of life-history strategies during the 1950s. (We consider a 'field' an area of scientific activity consisting of a theoretical core, a subject of research, a vocabulary and research tools). During the late 1940s and early 1950s, population ecology faced many problems, concerning its conceptual framework, its mathematical models, experimental deficiencies, etc. Research on life-history characteristics remained descriptive, lacking explanations about the causes and significance of phenomena. This was due to the deficiencies of the (...) theoretical framework of population ecology up to the 1950s. The catalyzing factor for the emergence of the new field was the interdisciplinary impacts, and especially the impact of neodarwinism. The elaboration of a new theoretical core, invoking also methodological shifts, was the triggering factor, conditioning the emergence of the new field. (shrink)

Background Policy instruments based on the working of markets have been introduced to empower consumers of healthcare. However, it is still not easy to become a critical consumer of healthcare. Objectives The aim of this study is to analyse the possibilities of the state to strengthen the position of patients with the aid of a new financial regime, such as personal health budgets. Methods Data were collected through in-depth interviews with executives, managers, professionals and client representatives of six long-term care (...) institutions. Results With the introduction of individual budgets the responsibility for budgetary control has shifted from the organisational level to the individual level in the caregiver-client relationship. Having more luxurious care on offer necessitates a stronger demarcation of regular care because organisations cannot simultaneously offer extra care as part of the standard care package. New financial instruments have an impact on the culture of receiving and giving care. Distributive justice takes on new meaning with the introduction of financial market mechanisms in healthcare; the distributing principle of ‘need’ is transformed into the principle of ‘economic demand’. Conclusion Financial instruments not only act as a countervailing power against providers insufficiently client-oriented, but are also used by providers to reinforce their own positions vis-à-vis demanding clients. Tailor-made finance is not the same as tailor-made care. (shrink)
