In simple terms, it is the removal or excision of the foreskin of the penis. Seems so simple, right? In some families, it is that simple. In other families, it is a religious exercise. I am a doctorally-prepared Family Nurse Practitioner and started my nursing career in the NICU. I have seen my fair share of circumcisions. (...) As a nurse, our role was to prep the room with whichever instrument the OB/GYNS preferred and then soothe the baby. Some OB/GYNS prescribed Sucrose, which is ultimately sugar water, for the babies to have while undergoing circumcision, while others wouldn't prescribe anything. What I learned in the NICU and by watching circumcisions being performed was that the doctor's preference determined the pain management protocol. Some doctors cared more about pain control, some cared [End Page 85] about presentation and ensuring the foreskin was cut properly, and some were just old school and had their preferences. I wanted to comfort the baby and make his world as comfortable as possible.Watching circumcisions being performed, I never formed an opinion about them. I really looked at circumcision like any other procedure. However, I did see some parents really toil over the decisions, especially fathers who did not want to "see their son tortured or mutilated like that" as one told me once. I didn't quite understand the comparison to mutilation, but I understood why he did not want his son to go through the procedure. It wasn't until years later that I understood the gravity of the decision when I became pregnant with our son and knew immediately it was a boy. Call it a mother's intuition, but I just knew. To circumcise my son was never a decision I had to deliberate very long because it was something I was accustomed to in my family. It wasn't until one of my relatives got married and had her first son that I became aware that this would ever be a weighty decision for someone in my family.I took it for granted all those years before working in the NICU because circumcision was superficial to me. I realized that it's not just a piece of foreskin to many people. For some circumcision is done for religious reasons, for others making the choice to circumcise is about sameness, or hygiene. For my extended family member, it happened to be about sameness. She didn't want her sons to look different from their dad. Since potty training was mostly going to be taught by their father, they wanted their sons to look like him. Many NICU parents told me they wanted their sons to look like their fathers, so this notion was familiar to me. After my extended family member explained why sameness mattered, I understood the reasons why some parents felt this way.There are other stories like this which we pondered upon when we got pregnant with our son, and I really began to reflect on whether we should circumcise or not. Then two additional frames of thoughts arose; one, I ensured that my husband and I jointly made the decision together, but I really leaned on him for his opinion because he was a male and most of the male teaching would come from him. Second, we considered if the decision to circumcise should come from him once he became an adult. His father and I discussed whether he should have a choice regarding being circumcised. He may have chosen a different decision than us and we didn't want to take away his right to choose. However, waiting to have a circumcision as an adult comes with its own sets of challenges, including the possibility of infection, increased pain, missed work, edema, etc. So, ultimately, we made the decision to go ahead with the circumcision.In our case, our pediatrician performed the circumcision. WilsonI am a doctorally-prepared nurse practitioner with over 20 years in healthcare, and I am currently pursuing my MS in Bioethics and Medical Humanities at Tulane University, with an anticipated graduation of May 2023. In the fall of 2022, I had the pleasure of taking a Clinical Ethics course. As part of that course, I shadowed (...) a Clinical Ethicist for a week. It was a valuable experience, and I gained some insights about a role I didn’t know about previously. The last two years have been difficult for my family and me. I lost my dad in November 2020 and then a “sister cousin” in May 2021. In both instances, we had the team involved in their care and helping with the decision-making process.The role of the Clinical Ethicist Consultant (CEC) service is to help families process medical decisions—most of those decisions deal with endof-life care. The CEC works as the mediator between the family and the medical team. This is how I saw the CEC role play out in my experience.During my shadowing experience, my preceptor was phenomenal at his role because 1) he listened to the patient and the family, and 2) he consulted with the appropriate team members. My shadowing experience was virtual due to the restrictions of the healthcare facility. However, I was able to attend ethical team meetings. I was able to listen to multiple family calls and consultations. During my experience, my clinical ethicist was very conscientious of taking inventory of the feelings of everyone involved. Before he provided an opinion on a case, he did a deep dive into the patient’s chart to gain the patient’s historical context. He always asked for my opinion while we discussed a case, which made me feel included even as a student. It made me feel like my opinion had some weight, and it did in one of the cases. When I returned the next day, the feelings I had about a patient and her family using her for financial gain turned out to be true. Elder abuse was taking place.My personal experiences were more complicated, in which we didn’t always have a medical team that always heard what we needed for our loved ones. In the case of my sister cousin, she entered the hospital and was diagnosed with COVID-19. Our family never thought she would die from it, so it was shocking to us when she was placed on a ventilator and never came off. When the critical care MD stated that “there was nothing else he could do for her,” I could not understand how that could be when lung transplants were being done all over the US and even less than 100 miles away.We needed a CEC team then. I was the medical expert for my family, and I felt the medical team was not listening to what we wanted. The medical team believed that to continue treatment would be futile and their belief about this exceeded their need to continue treating my sister cousin. However, having a CEC mediate by first understanding the clinical team’s perspective, explaining those considerations to us, and then helping our family relay back our [End Page 23] wants, needs, and values to the clinical team could have helped. Unfortunately, my cousin never got this option. There was a palliative care team implemented but never a Clinical Ethicist. Having faced both, from a student perspective and from a personal experience, I would say that having a CEC present allows a family to have an advocate in the room, someone who knows the medical language and can explain to you and will explain to the medical team your perspective, which can sometimes be lost in translation or not heard.When my father passed away, he did not have a Clinical Ethicist as a part of his medical team either. Tamar Schapiro has offered an important new ‘Kantian’ account of inclination and motivation, one that expands and refines Christine Korsgaard’s view. In this article I argue that Kant’s own view differs significantly from Schapiro’s. Above all, Kant thinks of inclinations as dispositions, not occurrent desires; and he does not believe that they stem directly from a non-rational source, as she argues. Schapiro’s ‘Kantian’ view rests on a much sharper distinction between the rational and non-rational parts of the soul. Current scientific research almost always requires collaboration among several (if not several hundred) specialized researchers. When scientists co-author a journal article, who deserves credit for discoveries or blame for errors? How should scientific institutions promote fruitful collaborations among scientists? In this book, leading philosophers of science address these critical questions.

A critical property of the perception of spoken words is the transient ambiguity of the speech signal. In localist models of speech perception this ambiguity is captured by allowing the parallel activation of multiple lexical representations. This paper examines how a distributed model of speech perception can accommodate this property. Statistical analyses of vector spaces show that coactivation of multiple distributed representations is inherently noisy, and depends on parameters such as sparseness and dimensionality. Furthermore, the characteristics of coactivation vary considerably, (...) depending on the organization of distributed representations within the mental lexicon. This view of lexical access is supported by analyses of phonological and semantic word representations, which provide an explanation of a recent set of experiments on coactivation in speech perception (Gaskell & Marslen–Wilson, 1999). This article considers financial risk management practice using a virtue ethics lens, in response to ongoing critiques of risk management from within business ethics. Risk management should be seen as embedded within a complex system of cultures, organizations and regulations that are underpinned by a quantitatively reductive or ‘mechanistic’ economic paradigm, where dominant logics of self-interest, profit maximization and short-termism prevail. Building on recent work applying virtue ethics in finance, an alternative to the values, normative expectations and priorities in financial (...) risk management is presented in the form of a refined ‘taxonomy’ of the virtues that can be applied to individuals and organizations engaged in financial risk management. This article aims to contribute to and extend debate on the ethical aspects of risk management and presents an alternative to more technically based critiques which do not engage with the underlying socially constructed foundations of risk management practice. It thereby provides a basis to critique the mechanistic and narrow approach used in the current regulation of the sector. (shrink)

To provide rigour when preparing a research design, the researcher needs to carefully consider not only the methodology but also the philosophical intent of the study. This, however, is often absent from reported research and provides the reader with little evidence by which to judge the merits of the chosen methodology and its influence on the study. The purpose of this paper is to set out the case for critical realism as a framework to guide appropriate action in practice development (...) and realistic evaluation for understanding the consequences of those actions.It is evident that critical realism and critical social science share common ground. Emancipatory practice development (ePD) is based on the philosophy of critical social science and therefore by virtue is linked to the tenets of critical realism. Until now, the evaluation of ePD programme has been well served by 4th-generation evaluation. However, this paper outlines the need for a different approach to evaluation, one that is based on critical realism, that is concerned with emancipation, and that can be used in the ever-changing environment of clinical practice. Realistic evaluation not only links strongly to ePD programmes, but also serves as the basis for effective research questions that will test the outcomes of the research and inform the transferability of ePD mechanisms into differing contexts. We examine whether investors value the disclosure of first-time standalone corporate social responsibility reports, and whether market valuations differ between government-controlled and privately controlled firms. Using a matched sample of Chinese publicly listed firms, we find that CSR initiators have higher market valuations than matched CSR non-initiators, and CSR initiators controlled by the central and local governments have lower market valuations than CSR non-initiators and CSR initiators controlled by private shareholders. Additional analyses demonstrate that CSR initiators with high CSR reporting (...) quality and perceived credibility have higher market valuations than CSR initiators with low CSR reporting quality and medium or low perceived credibility of CSR reporting. We do not find convincing evidence that CSR mandate, litigation risk, and prior stock returns affect market reactions to CSR reporting. Overall, we find that the market values standalone CSR reports, and that CSR reporting quality and perceived credibility are important factors in market valuation. (shrink)

Fodor's thinking on modularity has been influential throughout a range of the areas studying cognition, chiefly as a prod for positive work on modularity and domain-specificity. In _The Mind Doesn't Work That Way_, Fodor has developed the dark message of _The Modularity of Mind_ regarding the limits to modularity and computational analyses. Recent discussions of genomics and international justice have adopted the concept of ‘global public goods’ to support both the view of genomics as a benefit and the sharing of genomics knowledge across nations. Such discussion relies on a particular interpretation of the global public goods argument, facilitated by the ambiguity of the concept itself. Our aim in this article is to demonstrate this by a close examination of the concept of global public goods with particular reference to its use in (...) the context of genomic databases. We contend that the argument for construing genomics as a global public good depends on seeing it as a natural good by focusing on features intrinsic to genomics knowledge. We shall argue that social and political arrangements are relevant and that recognising this opens the door to construing the use of global public goods language as a strategic one. (shrink)

Siguiendo las tesis que plantea Jaspers en el Problema de la culpa, se intenta mostrar que si se interpreta el concepto de culpa política propuesto por Jaspers en la línea del filósofo liberal John Locke, (...) se puede arrojar una nueva luz sobre lo que la responsabilidad política significa. Esto también nos permitirá mostrar que las llamadas obligaciones políticas de los ciudadanos se extienden más allá de los requerimientos institucionales, asunto que ha sido el foco de recientes discusiones entre aquellos estudiosos que tratan la cuestión de la legitimidad política desde la perspectiva del contractualismo moderno. Approaches to self‐management traditionally focus upon individual capacity to make behavioural change. In this paper, we use Bourdieu's concepts of habitus and capital to demonstrate the impact of structural inequalities upon chronic illness self‐management through exploring findings from 28 semi‐structured interviews conducted with people from a lower socioeconomic region of Adelaide, South Australia who have type 2 diabetes. The data suggests that access to capital is a significant barrier to type 2 diabetes self‐management. While many participants described having sufficient cultural (...) capital to access and assess health information, they often lacked economic capital and social capital in the form of support networks who promote health. Participants were often involved in social networks in which activities which are contrary to self‐management have symbolic value. As a consequence, they entered relationships with health professionals at a disadvantage. We conclude that structural barriers to self‐management arising from habitus resulting in the performance of health behaviours rooted in cultural and class background and limited access to capital in the form of economic resources, social networks, health knowledge and prestige may have a negative impact on capacity for type 2 diabetes self‐management. We have two main objections to Kerr and Godfrey-Smith's (2002) meticulous analysis. First, they misunderstand the position we took in Unto Others – we do not claim that individual-level statements about the evolution of altruism are always unexplanatory and always fail to capture causal relationships. Second, Kerr and Godfrey-Smith characterize the individual and the multi-level perspectives in terms of different sets of parameters. In particular, they do not allow the multi-level perspective to use the individual fitness parameters i and i. (...) We don't see why the multi-level perspective prevents one from thinking in these terms. Kerr and Godfrey-Smith's argument that Uyenoyama and Feldman's (1980, 1992) definition of altruism belongs more to the individualist perspective than it does to the multi-level perspective is an artifact of their choice of parameters; the same point applies to their argument about the individualism inherent in the idea of Class I and Class II fitness structures. It is known that there exists a first-order sentence that holds in a finite group if and only if the group is soluble. Here it is shown that the corresponding statements with ‘solubility’ replaced by ‘nilpotence’ and ‘perfectness’, among others, are false.These facts present difficulties for the study of pseudofinite groups. However, a very weak form of Frattini’s theorem on the nilpotence of the Frattini subgroup of a finite group is proved for pseudofinite groups.

The purpose of this paper is, in the context of the distinction between Nietzsche's notions of cultural philistine (Bildungsphilister) and free spirit (Freigeist), to discuss which of these two types we, professors at Brazilian higher education institutions, are more similar. The philistine is the opposite of the genuine man of culture and the artist. The free spirit, on the other hand, is the exception, he is one who is disconnected from the values and habits in force, he doesn't cling to (...) anything and walks freely. A woman presents to her physician with a newly diagnosed condition that in her considered and informed judgment requires an elective surgical procedure. The physician, after speaking with her, agrees that this is an acceptable option. The procedure in question is in fact one of the commonest surgeries performed on American women. The physician is also aware that although the procedure is deemed elective in this and in most cases, research has shown that the consequences of not providing the procedure (...) when it is requested can be severe in terms of both physical and emotional sequelaewhich itself can add to the complexity and risk of the procedure. Presented with this option, the patient expresses dismay but also her resigned commitment to follow through with this referral, and leaves the office in tears. (shrink)

This research project defines the concept of an ethical incident pit and explores how qualitative and quantitative research into corporate ethical failurescan be conducted using this concept. Factors on an organizational, departmental and individual level are explored.

In six chapters of uneven length, Peter Lewis Allen, a former literature professor and public policy activist, offers a highly readable romp through two millennia of beliefs and attitudes regarding sin, sex, and disease. In particular, Allen draws on religious, medical, and popular literature from different eras in order to exemplify how particular “diseases”—lovesickness, leprosy, syphilis, bubonic plague, and masturbation—were causally connected with thoughts of punishment for sinful behavior. He then extends this theme into a lengthy chapter describing how the (...) same belief contributed to the chasm between conservative and activist perspectives on AIDS in the United States over the past two decades.Allen, who is admittedly not a historian, picks and chooses a “series of portraits of disease,” offering little contextualization as he creates a “tableau” of perspectives drawn from European and American discourse throughout Christianity. He flavors his chronological narrative with personal experiences drawn from his work in gay health groups. The most significant personal experiences he intersperses stem from thoughts about his one‐time lover, now dead from complications associated with AIDS. He interjects key episodes from this relationship into the book to affirm that “in its fight about how to respond to AIDS,” American society has reenacted a “drama that had been written centuries before” .Allen's engaging writing stands much more firmly as chronologically arranged comparative literature than as historical scholarship. His greatest historiographical faux pas lies in his anachronistic writing about disease. For example, he introduces his chapter on medieval leprosy with a description of the pathological destruction caused by the leprosy bacterium. Then, in selecting situations drawn from Herodotus through the sixteenth‐century “contagionist” Girolamo Fracastoro, he erroneously assumes that “leprosy” implied a singular, consistent disorder throughout this long period. Similar concerns arise regarding his coverage of syphilis. Such errors may have originated from his drawing too exclusively from pre‐1990 historical writings. Works like Charles E. Rosenberg and Janet Golden's Framing Disease: Studies in Cultural History and Linda Merian's edited volume The Secret Malady: Venereal Disease in Eighteenth‐Century Britain and France , and the voluminous literature on the history of the body, would have provided contextual concepts within which to place more concrete views about the intersections between diseases and culture. The author's desire to flash forward from particular historical periods to AIDS in our society too often leads to weakly substantiated connections between time periods.Allen's strengths lie in his ability to draw on works of literature, especially popular literature. His particular emphasis on French works reflects his own background in medieval French literature. His arguments are also strong when comparing church and society in the medieval period. For example, his chapter on lovesickness clearly delineates the persistent position of the Church Fathers against medics who regularly prescribed a “coital cure” for the disorder, without specifying what partners to select, instead of welcoming disease as a divine corrective.Allen's writing on plague, the shortest chapter of the book, clearly depicts the once‐common view that this epidemic disaster was inflicted by God as a punishment for mankind's sinful nature. However, the omission of any discussion of sex in a chapter of a work subtitled “Sex and Disease, Past and Present,” is somewhat puzzling. Even more conspicuous is the author's failure to expand upon the 1530 poem regarding the plight of the impious shepherd, Syphilis, from whom the disease in question received its name. Why would a literary scholar prone to draw on popular writings not give more than a single‐sentence mention to this historically relevant poem?Of the works Allen has drawn upon, he offers a pleasant mix of primary and secondary accounts that, like all good literary criticism, gives original sources a clear voice. Any work so selectively focused on a few diseases will leave some readers wondering about others that might have been perceived in terms of sex and sin. Were such associations discussed in regard to madness in the eighteenth century, tuberculosis in the nineteenth century, or the wide range of twentieth‐century STDs prior to AIDS? Why was no mention made of leprosy in the twentieth‐century context? Its AIDS‐like stigma has been historically contextualized in several works, including Stephanie J. Castillo's award‐winning 1992 documentary Simple Courage. Similarly, one wonders why the author chose not to drawn on standard medical history accounts on AIDS, including Elizabeth Fee and Daniel M. Fox's edited volume AIDS: The Burden of History and, perhaps more important for the Francophile Allen, Mirko D. Grmek's Histoire du SIDA: Début et origine d'une pandémie actualle .Apart from my historiographical criticisms, The Wages of Sin is a most delightful popular rendering of key relations between disease and Christianity. Works like this are extremely useful in reminding the reading public in an articulate, informative, and entertaining manner that they are part of humanity's continuum that has long sought connections between physical health and moral living. Indeed, Allen's apt description of the underlying usefulness of history underscores the importance of his own writing: “We value history because, whatever its distortions and inaccuracies, it is a way of understanding and giving value to our collective human experience and collective human memory”.
