According to Ron Mallon (2004), any adequate account of race must meet three constraints: passing, no-traveling, and reality. "Passing" describes the fact that persons who are treated by others as belonging to one race, may "actually" belong to a different race. "No traveling" refers to the fact that racial concepts such as "white" may pick out different sets of persons in different cultures. "Reality" refers to the fact that racial designations enter into explanations of how people's lives go. However, Mallon (...) argues that no account can simultaneously satisfy all three constraints. I argue that an account of race as an institutional fact, based on Searle's theory of constitutive rules, can satisfy all three constraints. RATIONALE: The concept of evidence-based medicine is important in providing efficient health care. The process uses research findings as the basis for clinical decision making. Evidence-based practice helps optimize current health care and enables the practitioners to be suitably accountable for the interventions they provide. Little work has been undertaken to examine how allied health professionals change their clinical practice in light of the latest evidence. The use of opinion leaders to disseminate new evidence around the management of low back (...) pain into practice has been proposed. AIMS: The aim of this study was to investigate if physiotherapists' clinical management of patients with low back pain would change following an evidence-based education package, which utilized local opinion leaders and delivered the best evidence. METHOD: Thirty musculoskeletal physiotherapists from a Community Trust in North Staffordshire were cluster randomized by location of work, to two groups. The intervention group received an evidence-based programme on the management of low back pain, including advice regarding increasing activity levels and return to normal activity and challenging patients' fears and beliefs about their pain. The control group received a standard in-service training package on the management of common knee pathologies. The physiotherapists' clinical management of patients with low back pain was measured prior to training and 6 months post training. Outcome measures were based on physiotherapists completing 'discharge summary' questionnaires, which included information relating to the use and importance of therapies for treating their low back pain patients. RESULTS: There were few significant differences in treatment options between the intervention and control groups post training. Whilst there was some indication that physiotherapists were already utilizing aspects of psychosocial management for patients with low back pain, there was little change in what physiotherapists perceived to be important to patient recovery and actual clinical practice following the intervention. CONCLUSIONS: Psychosocial factors have been identified as an important factor in the recovery of patients with low back pain. This project incorporated the latest evidence on the management of low back pain and utilized the theory of opinion leaders to disseminate this evidence into clinical practice. Whilst there were some limitations in the overall size of the study, the results help to give an insight into the challenges faced by the health care system and researchers alike to ensure quality evidence is actually utilized by practitioners for the benefits of patient care. In this “global information age” accessing, disseminating, and controlling information is an increasingly important aspect of human life. Often these interests are expressed in the language of human rights—e.g., rights to expression, privacy, and intellectual property. As the discipline concerned with, “Facilitating the effective communication of desired information between human generator and human user” (Belkin, 1975, 22), Library and Information Science (LIS) has a central role in facilitating communication about human rights and ensuring the respect for human rights in information (...) services and systems. This paper surveys the literature at the intersection of LIS and human rights. To begin, an overview of human rights conventions and an introduction to human rights theory is provided. Then the intersections between LIS and human rights are considered. Three central areas of informational human rights—communication, privacy, and intellectual property—are discussed in detail. It is argued that communication rights in particular serve as a central linchpin in the system of human rights. (shrink)

In this paper we present an equivalence between the category of commutative regular rings and the category of Boolean-valued fields, i.e., Boolean-valued sets for which the field axioms are true. The author used this equivalence in [12] to develop a Galois theory for commutative regular rings. Here we apply the equivalence to give an alternative construction of an algebraic closure for any commutative regular ring.Boolean-valued sets were developed in 1965 by Scott and Solovay [10] to simplify independence proofs in set (...) theory. They later were applied by Takeuti [13] to obtain results on Hilbert and Banach spaces. Ellentuck [3] and Weispfenning [14] considered Boolean-valued rings which consisted of rings and associated Boolean-valued relations. To the author's knowledge, the present work is the first to employ the Boolean-valued sets of Scott and Solovay to obtain results in algebra.The idea that commutative regular rings can be studied by examining the properties of related fields is not new. For several years algebraists and logicians have investigated commutative regular rings by representing a commutative regular ring as a subdirect product of fields or as the ring of global sections of a sheaf of fields over a Boolean space. These representations depend, as does the work presented here, on the fact that the set of central idempotents of any ring with identity forms a Boolean algebra. The advantage of the Boolean-valued set approach is that the axioms of classical logic and set theory are true in the Boolean universe. Therefore, if the axioms for a field are true for a Boolean-valued set, then other properties of the set can be deduced immediately from field theory. (shrink)

A narrow conception of who counts among the marginalized can blind ethicists to the precarious position of groups who function as middle agents between elites and the lower class. The imposition of middle agency on such groups is a form of oppression that leaves them vulnerable to abandonment and attack. In Rwanda, discourses emanating from colonialism, classism, and racism obscured the Tutsi as middle agents, despite white Catholics' dedication to the poor. By neglecting to recognize middle agency as a type (...) of marginalization, missionaries contributed negatively to the genocide. Liberatory practices are recommended so that ethicists can expose and challenge the dynamics of middle agency and include all the marginalized in liberation strategies. (shrink)

We give an examination of the automorphism group Aut of a countable recursively saturated model M of PA. The main result is a characterisation of strong elementary initial segments of M as the initial segments consisting of fixed points of automorphisms of M. As a corollary we prove that, for any consistent completion T of PA, there are recursively saturated countable models M1, M2 of T, such that Aut[ncong]Aut, as topological groups with a natural topology. This paper draws on Weber’s theodicy problem to define organizational corruption as the emerging discrepancy between experience and normative expectation. Theodicy describes the attempts to explain this discrepancy. The paper presents four normative principles enlisted by observers to respond to perceived corruption: moral dilemma, detachment, systematic regulation, and normative controls. Consistent with social construction, these justifications work to either reaffirm or challenge prevailing social norms in the face of confusing events. An exemplar case involves perceived corruption in the business of (...) mountain climbing as represented through the 1996 Mt. Everest climbing disaster. The events illustrate how theodicy informs descriptive accounts of corruption and expose two limitations of normative models of ethics. (shrink)

The recent “turn to matter” evident in material feminist theories of the more‐than‐human world offers distinct posthuman understandings of the world as continuously relationally entangled, emergent or materializing. In this paper, I consider how these premises both trouble conventional understandings of matter and/or materials, but likewise potentially revise and revitalize understandings of the political for health and inequalities, and for nursing. This is both timely and much needed given contemporary contexts of austerity‐driven neoliberalism in health care and the unprecedented growth (...) in disparities of wealth and well‐being. I wish to explore whether material feminisms allow us to retheorize connections between abstract theory and material concerns like health and inequalities, differently. This is not theory in opposition to practice or activism, but theory conceptualized as sets of entangled emergent practices, but also what constitutes the political, as more fully relational to and in praxis with health‐related activism. I will argue these theories further justify how practitioners can visibly care for and care more about social and health inequalities. Drawing mainly on the work of material feminist, Karen Barad, and her bringing together of queer and feminist theory, as well as feminist new materialisms and understandings of posthumanism, I discuss how this turn to matter together with meaning might transform understandings of health and inequalities. This article presents an approach for structured reflection by a designer through journal writing. The journal writing situates the agency of the designer, using a range of internal conversations as a way to expand horizons and perspectives. Through a structured approach using journal entries, experiences of the design process are introduced as reflective internal talkback. In the approach that is described, decision points and perspectives are negotiated and potentially contested through a series of voices of self as I, Me, You, (...) and We. These voices are intertwined within the journal narrative and are proposed as a useful framework for negotiating and effectively engaging with design complexity. The article introduces the conceptual backgrounds with reference to conversation as a process of learning. The specific dynamics of the journal writing approach are explained and then illustrated in a case study. The case study describes how the approach is applied for a specific design project. (shrink)

In the 1950s and 1960s Freudian theory was deemed to be a vital part of the sociological tradition, but since then it has fallen from favor, largely because of the simplifications and misinterpretations both by Freud's sociological critics and by his supporters. Chief among such misunderstandings is the tendency to view Freud's social theory as a variant of that of Hobbes, in which a selfish and asocial human nature is made social through the imposition of external constraints; these constraints, as (...) Durkheim stated, eventually are "internalized" into the personalities of social beings. Against such a claim this paper argues that Freud's views differ profoundly from those of Hobbes and that the myth of the Hobbesian Freud has so distorted Freud's most fundamental concepts that their social theoretical significance has been largely obscured. This paper takes as its starting point the thesis that one is responsible for one’s actions insofar as one has the ability to act for good reasons. Such a view faces a challenge: it is plausible that only beings with the ability to reflect are responsible agents, and yet it seems that not only is it possible to act for reasons without reflecting, it seems to happen quite frequently. Thus, advocates of the rational-ability view of responsibility must either reject as (...) a necessary condition that responsible agents must have the ability to reflect, or locate a plausible role for reflective ability. There is an emphasis on the politics of ‘life’ through the lens of ‘marginalization’ and the giving and taking away of life with the solution being reducible to Said’s understanding of the marginalized making a “voyage in” from the periphery to reclaiming the center. While clearly these are useful political questions, they reach an impasse, which Zizek’s work while is in favor of immigration and open borders, correctly raises some anxieties about the politics of life from a completely different perspective. This is what I want to explore in this paper, what does Zizek’s work make possible in thinking beyond the ideological way that the ‘politics of life’ has been framed, thus opening up discourses that have been ‘set’ by a limited ‘post/de-colonial-biopolitical’ set of signifiers, which to my reading of Zizek, he is right to point out, have become the ‘master’-signifiers in the ideological matrices of how the question of life is currently framed. Ludwig Wittgenstein was a brilliant, intense, complex man, and this novel, a work of historical fiction but based on fact, explores his early thinking, which led him to publish one of the most important works of logic ever written. The story is told by David Pinsent, a student in mathematics who meets Wittgenstein at Trinity College in Cambridge, England, just before World War I. Despite Wittgenstein’s odd mannerisms and difficult personality, David is attracted to him, recognizing his genius immediately and (...) forming a friendship that is as volatile as it is rewarding. Wittgenstein, under the tutelage of the eminent mathematician and philosopher Bernard Russell, has developed a picture theory of meaning to explain human language through the lens of logic, and he is tormented by the idea that he will die before he is able to put it into book form and present it to the world. -/- Both David and Ludwig are welcomed into the elite intellectual circles at Cambridge, which consist of some of the most notable philosophers, mathematicians, and logicians of the day, including not just Russell but also men like John Maynard Keynes and G.E. Moore, as well as the famous Bloomsbury Set. However, David is troubled by the mysteries that surround Wittgenstein. Ludwig, he learns, has developed an innovative propeller that can be used on a flying bomber, and David finds himself caught up in international intrigue as both England and Germany vie for the design to give them an advantage in the impending war. -/- But David and Ludwig have become both emotionally and physically intimate, and as war looms, David is forced to confront the role that their forbidden relationship plays in the escalating risks to Wittgenstein’s safety. Responsible Research and Innovation (‘RRI’) is a cross-cutting priority for scientific research in the European Union and beyond. This paper considers whether the way such research is organised and delivered lends itself to the aims of RRI. We focus particularly on international consortia, which have emerged as a common model to organise large-scale, multi-disciplinary research in contemporary biomedical science. Typically, these consortia operate through fixed-term contracts, and employ governance frameworks consisting of reasonably standard, modular components such as management committees, advisory (...) boards, and data access committees, to co-ordinate the activities of partner institutions and align them with funding agency priorities. These have advantages for organisation and management of the research, but can actively inhibit researchers seeking to implement RRI activities. Conventional consortia governance structures pose specific problems for meaningful public and participant involvement, data sharing, transparency, and ‘legacy’ planning to deal with societal commitments that persist beyond the duration of the original project. In particular, the ‘upstream’ negotiation of contractual terms between funders and the institutions employing researchers can undermine the ability for those researchers to subsequently make decisions about data, or participant remuneration, or indeed what happens to consortia outputs after the project is finished, and can inhibit attempts to make project activities and goals responsive to input from ongoing dialogue with various stakeholders. Having explored these challenges, we make some recommendations for alternative consortia governance structures to better support RRI in future. (shrink)

Discussion around the increasing use of AI in healthcare tends to focus on the technical aspects of the technology rather than the socio-technical issues associated with implementation. In this paper, we argue for the development of a sustained societal dialogue between stakeholders around the use of AI in healthcare. We contend that a more human-centred approach to AI implementation in healthcare is needed which is inclusive of the views of a range of stakeholders. We identify four key areas to support (...) stakeholder involvement that would enhance the development, implementation, and evaluation of AI in healthcare leading to greater levels of trust. These are as follows: (1) aligning AI development practices with social values, (2) appropriate and proportionate involvement of stakeholders, (3) understanding the importance of building trust in AI, (4) embedding stakeholder-driven governance to support these activities. Biobanks are proliferating rapidly worldwide because they are powerful tools and organisational structures for undertaking medical research. By linking samples to data on the health of individuals, it is anticipated that biobanks will be used to explore the relationship between genes, environment and lifestyle for many diseases, as well as the potential of individually-tailored drug treatments based on genetic predisposition. However, they also raise considerable challenges for existing legal frameworks and research governance structures. This book critically examines the current governance (...) structures in place for biobanks in England and Wales. It shows that the technologies, techniques and practices involved in biobanking do not always conform neatly to existing legal principles and frameworks that apply to other areas of medical research. Using a socio-legal approach, including interview data gathered from the scientific community, this book provides unique insights and makes recommendations about appropriate governance mechanisms for biobanking in the future. It also explores the issues around the secondary use of information, such as consent and how to protect privacy, when biobanks are accessed by a number of different third parties. As soon as he had observed labour to be ‘first of all, a process between man and nature', Marx turned to conscious determination. ‘Man not only affects a change of form in the materials of nature, he also realises his own purpose in these materials. And this is a purpose he is conscious of. It is purpose which distinguishes labour from the activities of animals. Marx called the purposive character of labour ‘an exclusively human characteristic’ and the term indicates its (...) fundamental importance in his thought. As it is purposive activity, so labour is ‘a specific productive activity appropriate to its purpose, a productive activity that assimilate[s] particular natural materials to a particular human requirement'.3 Since ends are specific by definition, this has to be the case, and neither absence of skill nor indifference effect the issue one way or the other. Work does not cease to be particular because it demands no special capacities - tightening screws and stacking shelves do not stop being different kinds of activity by virtue of the fact that anyone can perform them; or because no store its set by its distinctive qualities. In its accounts, capital may treat different kinds of labour uniformly as a cost of production, but this does not alter the fact that the labour it employs comprises different types of labour: ‘the fact that the production of use-values or goods is carried on under the control of a capitalist and on his behalf does not alter the general character of that production'.4 In which case, we ask, what is labour which is not particular? If labour is always and necessarily a specific productive activity, what is abstract labour — ‘homogenous labour’ which, by definition, is not specific? (shrink)

Obesity is now commonly recognised to be a significant public health issue worldwide with its increasing prevalence frequently described as a global epidemic. In the United Kingdom, primary care nurses are responsible for weight management through the provision of healthy eating advice and support with lifestyle change. However, nurses themselves are not immune to the persistent and pervasive global levels of weight gain. Drawing on a Gadamerian informed phenomenological study of female primary care nurses in England, this paper considers the (...) complex gendered understandings and experiences of being overweight, and of food and eating. The nurses’ emotional and injurious experiences of being large is found to be capable of producing embodied caring practices, involving a fusion of horizons with patients over how it feels to inhabit a large body. Yet, even though subjected to similar derogatory stereotypes as patients, they simultaneously reinforce the dominant and damaging individualising psychopathology inherent to anti‐obesity discourses. This suggests an urgent need to expose and challenge harmful discourses surrounding women’s body size and weight in order to avoid nursing practices that unthinkingly reproduce culturally dominant and gendered understandings of weight, body size, food and eating. Current debate on business ethics in Australia continues apace as the excesses of the 1980s are exposed. Codes of Ethics have been a high profile instrument in the American business scene. A survey of Australia''s largest business corporations reveals a different situation. Codes are not as commonly used, tend to refer to legal requirements and do not have as high a profile within the corporation. Given the changing legal framework in Australia a greater role for Codes of Ethics may emerge.

We provide a suitable theoretical foundation for the notion of the quantum coherent state which describes the electrostatic field due to a static external macroscopic charge distribution introduced by the author in 1998 and use it to rederive the formulae obtained in 1998 for the inner product of a pair of such states. (We also correct an incorrect factor of 4π\documentclass[12pt]{minimal} \usepackage{amsmath} \usepackage{wasysym} \usepackage{amsfonts} \usepackage{amssymb} \usepackage{amsbsy} \usepackage{mathrsfs} \usepackage{upgreek} \setlength{\oddsidemargin}{-69pt} \begin{document}$$4\pi$$\end{document} in some of those formulae.) Contrary to what one might expect, (...) this inner product is usually non-zero whenever the total charges of the two charge distributions are equal, even if the charge distributions themselves are different. We actually display two different frameworks that lead to the same inner-product formulae, in the second of which Gauss’s law only holds in expectation value. We propose an experiment capable of ruling out the latter framework. We then address the problem of finding a product picture for QED—i.e. a reformulation in which it has a total Hamiltonian, arising as a sum of a free electromagnetic Hamiltonian, a free charged-matter Hamiltonian and an interaction term, acting on a Hilbert space which is a subspace (the physical subspace) of the full tensor product of a charged-matter Hilbert space and an electromagnetic-field Hilbert space. (The traditional Coulomb gauge formulation of QED isn’t a product picture in this sense because, in it, the longitudinal part of the electric field is a function of the charged matter operators.) Motivated by the first framework for our coherent-state construction, we find such a product picture and exhibit its equivalence with Coulomb gauge QED both for a charged Dirac field and also for a system of non-relativistic charged balls. For each of these systems, in all states in the physical subspace (including the vacuum in the case of the Dirac field) the charged matter is entangled with longitudinal photons and Gauss’s law holds as an operator equation; albeit the electric field operator (and therefore also the full Hamiltonian) while self-adjoint on the physical subspace, fails to be self-adjoint on the full tensor-product Hilbert space. The inner products of our electrostatic coherent states and the product picture for QED are relevant as analogues to quantities that play a rôle in the author’s matter-gravity entanglement hypothesis. Also, the product picture provides a temporal gauge quantization of QED which appears to be free from the difficulties which plagued previous approaches to temporal-gauge quantization. (shrink)

Jakob Friedrich Fries is one of the most important representatives of the Critical Philosophy, someone who built immediately on the original Kantian philosophy. -/- Fries was born in 1773 in Barby (on the Elbe). In 1805 he was extraordinary professor for philosophy in Jena and in the same year was ordinary professor for philosophy in Heidelberg. Returning to Jena in 1816, one year later he was compulsorily retired because of his participation at the nationalistic and republican Wartburg Festival. In 1924 (...) he obtained a professorship for physics and mathematics, and in 1838 he was given back a professorship for philosophy. He died in 1843 in Jena. -/- The book summarizes the research results of the DFG-Project "Jakob Friedrich Fries' Influence on the Sciences of the 19th Century". The research project was carried out by Dr. Kay Herrmann (Institute of Philosophy, Jena University) and Prof. Dr. Wolfram Hogrebe (Institute of Philosophy, Bonn University). Such a study has special importance. There is available a large amount of literature about the "speculative contemporaries" of Fries, like Fichte, Schelling, and Hegel. In contrast to the "speculative philosophy", there has been published only a few studies about the Friesian natural philosophy. Fries was, in his natural-philosophical studies, looking for a link between philosophy and modern sciences, wheras his "speculative philosophical" contemporaries felt obligated to stick primarily to a descriptive, phenomenal view of nature. So far the question "How was mathematical natural philosophy regarded by scientists and mathematicians of the 19th century?" has hardly been investigated. Archival studies showed that this gap in Fries-research can be filled. The Friesian correspondence turned out to be a rich gold mine. -/- The present publication is more than a research report. The monographic first part is intended to introduce the foundations of the Friesian theory of cognition, the Friesian methodology, and the Friesian natural philosophy. This should facilitate entry into Friesian philosophy. -/- The Friesian theory is analyzed from two points of view: •How did Fries suceed in continuing and improving the Kantian approach? Is Fries able to remove the weak points of Kantian philosophy? -/- •What is the current significance of the Friesian approach? There are some interesting similarities between the Friesian approach and modern philosophical theories (such like Chomsky's theory of "universal grammer"). The lasting core of the Friesian theory of cognition is: To use empirical studies for working on philosophical problems. -/- Chapters 3 and 4 are scientific-historically oriented. These chapters analyze the Friesian position in scientific and mathematical debates (debates about the a priori foundations of physics, the problem of the identification of physics as an independent discipline, the problem of the boundary between chemistry and physics, the problem of mathematization of the sciences, the theory of the imponderabilia, the systematics and structure of sciences and mathematics, problems of infinity, the differential calculus, the theory of parallel lines) and the relation between Fries and the scientists of the 19th century. The book contains the latest findings gained by evaluation of the Friesian unpublished work (for example the correspondence with W. Weber, C. F. Gauß, E. F. Apelt, O. Schlömilch, Ch. Reichel, B. A. v. Lindenau, L. Gmelin, E. G. Fischer, A. N. Scherer, J. S. C. Schweigger) -/- One result of the research project is that some important scientists took a favourable view of the Friesian theory, but the influence of the Friesian philosophy on the sciences of the 19th century was very limited. The causes are very complex: An anti-natural-philosophical spirit of age, the limits of the Kantian inspired philosophy and some unfavourable aspects in the biography of Fries. -/- For the first time the voluminous Fries-Reichel-correspondence was evaluated. In this paper I take up a critical position in regard to the theme of debility around which this collection is framed. I argue that theorisations of ‘debility’ do little to progress theory and policy in regard to disability and share many of the problems inherent to the social model. I also suggest that the theorisation of debility is rooted in and reinforces ablebodied privilege. I begin with a critical analysis of the social model of disability and explore the dualisms (...) by which it either negates the body altogether or can only conceive the disabled body in negative terms. I then go on to explore how Puar's work on debility continues this negation of the disabled body. From this position I use the work of Inahara to excavate the foundations of ablebodied privilege. In Inahara's work gender is the analytic starting point, but for me white privilege is a much more effective mechanism through which to understand the impact and reproduction of ablebodied privilege— what McRuer refers to as ‘compulsory ablebodiedness'—which I argue underpins Puar's work. I conclude with some reflections upon how a critical analysis of ablebodied privilege might function and I reiterate its importance for a critical theory that goes beyond the mere repetition of binary structures of ablebodiedness and disability. “You Have Seen Their Faces”: Gisèle Freund, Walter Benjamin and Margaret Bourke-White as Headhunters of the Thirties -/- Abstract This paper concentrates on one work by each of three authors: Walter Benjamin’s Deutsche Menschen (Germans), an anthology of twenty-five 18th and 19th century German personal letters; Margaret Bourke-White and Erskine Caldwell’s You Have Seen Their Faces; and Gisèle Freund’s collection of photographic portraits of writers and artists, compiled between 1936 and 1939. The purpose of this paper is to compare the (...) politics of representation of these three figures and to examine the challenge they addressed to the dominant iconography of the 1930s. At the heart of each one’s work was a desire to bring about a shift in contemporary cultural politics by reimagining the body politic. Benjamin’s and Freund’s aim was to displace the monumentalizing, distancing, ‘heroic’ forms of fascist art and writing. They construct and project an intimate, or ‘domestic’ image of humanism as a form of social practice, past and present. Bourke-White’s aim was to bring to visibility an America concealed beneath the glossy homogenized images of white American advertising. All three authors aligned themselves with a documentary tradition that believed that representation should function as a constructed record of the traces of lived experience. This paper provides an analytical overview of the apostolic networks in Britain. There are several ways to understand the success of the apostolic networks. They can be seen as an extreme answer to an extreme problem an answer to a profound secularisation leading to declining church attendance and influence within society. The apostolic networks appear to fit into post-modern society better than conventional denominational structures. In terms of their organization, despite appearances, these networks are however hierarchical like the denominational structures (...) which they challenge. The difference is in the characteristic informality with which these networks can collaborate with each other even when they grow. (shrink)

A significant share of the U.S. federal R&D budget is devoted to large-scale, complex technological systems commonly referred to as "big science. " Over the last two decades, these systems have continued to grow in size, complexity, development time, and cost. At the same time, political changes in the United States, particularly the concern over government spending and the federal budget deficit, have made it more difficult for proponents to secure and preserve support for these programs over their lifetimes. Using (...) the U.S. space shuttle and space station programs as examples, the article shows that the political requirements for obtaining approval and funding for large and expensive research and development projects create conditions that reduce the likelihood that they will succeed technologically. The problems of technology-based projects like those of the U.S. space program are in fact deeply rooted in the American political process itself. (shrink)

This essay argues that, while much has been gained by medicine's focus on the spatial aspects of disease in light of developments in modern pathology, too little attention has been given to the temporal experience of illness at the subjective level of the patient. In particular, it is noted that there is a radical distinction between subjective and objective time. Whereas the patient experiences his immediate illness in terms of the ongoing flux of subjective time, the physician conceptualizes the illness (...) as a disease state according to the measurements of objective time. A greater understanding of this disparity in temporal experiencing provides insights into the lived experience of illness and can preclude difficulties in communication between physician and patient. Background Randomized controlled trials are central to generating knowledge about effectiveness of interventions as well as risk, protective and prognostic factors related to diseases in emergency newborn care. Whether prospective participants understand the purpose of research, and what they perceive as the influence of the context on their understanding of the informed consent process for RCTs in emergency obstetric and newborn care are not well documented. Methods Conceptual review. Discussion Research is necessary to identify how the illnesses may be prevented, (...) to explore the causes, and to investigate what medications could be used to manage such illness. Voluntary informed consent requires that prospective participants understand the disclose information about the research, and use this to make autonomous informed decision about participation, in line with their preferences and values. Yet the emergency context affects how information may be disclosed to prospective research participants, how much participants may comprehend, and how participants may express their voluntary decision to participate, all of which pose a threat to the validity of the informed consent. I challenge the claim that the ‘understanding’ of research is always necessary for ethical informed consent for research during emergency care. I argue for reconceptualization of the value of understanding, through recognition of other values that may be equally important. I then present a reflective perspective that frames moral reflection about autonomy, beneficence and justice in research in emergency research. Conclusion While participant ‘understanding’ of research is important, it is neither necessary nor sufficient for a valid informed consent, and may compete with other values with which it needs to be considered. In this book, Lynn Kaye examines how rabbis of late antiquity thought about time through their legal reasoning and storytelling, and what these insights mean for thinking about time today. Providing close readings of legal and narrative texts in the Babylonian Talmud, she compares temporal ideas with related concepts in ancient and modern philosophical texts and in religious traditions from late antique Mesopotamia. Kaye demonstrates that temporal flexibility in the Babylonian Talmud is a means of exploring and resolving (...) legal uncertainties, as well as a tool to tell stories that convey ideas effectively and dramatically. Her book, the first on time in the Talmud, makes accessible complex legal texts and philosophical ideas. We provide an introduction to network theory, evidence to support a connection between molecular network structure and neuropsychiatric disease, and examples of how network approaches can expand our knowledge of the molecular bases of these diseases. Without systematic methods to derive their biological meanings and inter‐relatedness, the many molecular changes associated with neuropsychiatric disease, including genetic variants, gene expression changes, and protein differences, present an impenetrably complex set of findings. Network approaches can potentially help integrate and reconcile these findings, as (...) well as provide new insights into the molecular architecture of neuropsychiatric diseases. Network approaches to neuropsychiatric disease are still in their infancy, and we discuss what might be done to improve their prospects. (shrink)

Inducement prizes have been long used to stimulate individuals and groups to accomplish diverse goals. Lately, governments have become more and more interested in these prizes and sought to include this kind of incentives within the set of policy tools available to promote science, technology, and innovation. To date, however, there has been little empirically-based scientific knowledge on how to design, manage, and evaluate innovation prizes. This note discusses aspects of the prize phenomenon and the opportunities and challenges related with (...) the use of innovation prizes as a government policy instrument. Compared to other incentive mechanisms, prizes are likely to present advantages to, for example, accelerate the development and commercialization of technologies that are held back for diverse reasons and help to leverage public money with external ideas, collaborative efforts, and the participation of diverse individuals and organizations. Still, despite these advantages and other interesting features of prizes, there are key questions that policy-makers and scholars must address to better understand this kind of incentives and further improve prize designs and implementations before governments move forward to a more widespread use of innovation prizes in science and technology policies. This article examines the involvement of neonatal nurses in ethical issues, achieved through a survey of Australian neonatal nurses. The aim was to discover if nurses were involved in ethical decisions, to examine various categories of neonates and the concerns that nurses felt about them, and to determine the extent to which nurses saw themselves as advocates. A response rate of 65% was achieved from nurses in two states who worked in intensive care and special care nurseries. The findings show (...) that nurses were more likely to be involved in clinical decision making than in ethical decision making, showed the greatest concern for infants who had an uncertain prognosis, and saw themselves as advocates for their patients. Much of the mathematics with which Felix Klein and Sophus Lie are now associated (Klein’s Erlangen Program and Lie’s theory of transformation groups) is rooted in ideas they developed in their early work: the consideration of geometric objects or properties preserved by systems of transformations. As early as 1870, Lie studied particular examples of what he later called contact transformations, which preserve tangency and which came to play a crucial role in his systematic study of transformation groups and differential equations. This note examines Klein’s efforts in the 1870s to interpret contact transformations in terms of connexes and traces that interpretation (which included a false assumption) over the decades that follow. The analysis passes from Klein’s letters to Lie through Lindemann’s edition of Clebsch’s lectures on geometry in 1876, Lie’s criticism of it in his treatise on transformation groups in 1893, and the careful development of that interpretation by Dohmen, a student of Engel, in his 1905 dissertation. The now-obscure notion of connexes and its relation to Lie’s line elements and surface elements are discussed here in some detail. (shrink)

This paper suggests that the paradigm of lived body (as it is developed in the works of Merleau-Ponty, Sartre and Zaner) provides important insights into the experience of illness. In particular it is noted that, as embodied persons, we experience illness primarily as a disruption of lived body rather than as a dysfunction of biological body. An account is given of the manner in which such fundamental features of embodiment as bodily intentionality, primary meaning, contextural organization, body image, gestural display, (...) lived spatiality and temporality, are disrupted in illness causing a concurrent disorganization of the patient's self and world. The paradigm of lived body has important applications for medical practice. It provides a fuller account of illness than does the prevailing reductionist Cartesian paradigm of body, more directly addresses the existential predicament of illness, and orients the clinical focus around the personhood of the patient. Chaotic itinerancy is useful for illustrating transitions in attractor dynamics seen in the olfactory system. Cantor coding is a good model for information processing, but so far it lacks perceptual proof. The theories presented provide a large step toward bridging the use of chaos as an interpretive tool and hard examination of chaotic neural activity during perception.
